류마티스 수부의 신건 파열에서 건 이전술과 건 이식술의 장기 추시 결과

Abstract

저자들은 류마티스 수부의 신건 파열에 대한 건 이전술과건 이식술의 장기적인 임상 추시 결과에 대해 보고하고자 한다. 평균 추시 기간이 14년(범위, 10-21)인 37명, 43예의 환자를 대상으로 하였다.파열후 수술받기까지의 평균기간은 13.1주(범위,3-48)였다. 3예에서 관절의 변형,건 이식술을 시행한 3예에서 재파열이 발생하였다. 변형 및 재파열이 발생한 경우를 제외하고, 중수수지관절의 평균 신전저하는 8.7도(범위, 0-40), 평균 수지끝-손바닥 거리(tip-to-palm)는 0.3cm(범위, 0-3),전반적인 만족도의 평균은 86.5(범위, 70-100)였다. 통계적으로 건 이전술과 건 이식술의 임상 결과간에 유의한 차이를 보이지 않았다. 이번 연구를 통하여 평균 14년의 추시 기간 동안 류마티스 수부의 신건 파열에 대한 건 이전술과 건 이식술의 만족스러운 결과를 확인할 수 있었다.; We report on the long-term clinical outcomes of tendon transfer and tendon graft for extensor tendon ruptures in rheumatoid hands.We reviewed 43cases comprised of 37 patients whose mean follow-up period was 14 years (range 10-21). The mean time to surgery was 13.1 weeks (range 3-48). Three cases showed deformity of the joint, and three tendon graft cases had re-rupture events.Excluding those patients with deformity and re-rupture events, the mean extension lag of the metacarpophalangeal joint was 8.7 degrees (range 0-40), the mean tip-to-palm distance was 0.3 cm (range 0-3), and the mean overall satisfaction rate was 86.5 (range 70-100). The results showed no significant differences in the clinical outcomes between tendon transfer and tendon graft. The present study demonstrated that both tendon transfer and tendon graft for extensor tendon ruptures in rheumatoid hands show satisfactory results that are maintained for an average of 14years follow-up.
