한국판 양성 및 음성증후군 척도의 탐색적 요인 분석

Abstract

연구목적 : 양성 및 음성증후군 척도(PANSS)는 정신분열병의 정신병리를 측정하기 위한 중요한 연구 도구이며 세계적으로 널리 번역되어 이용되고 있다. 한국판 PANSS의 비교적 만족할 만한 신뢰도와 타당도(요인 타당도 포함)가 보고되었지만 아직 요인 구조에 대한 연구는 시도되지 않았다. 이 연구의 목적은 한국어판 PANSS의 탐색적 요인분석을 한국인 정신분열병 환자를 대상으로 평가하여 그 요인구조를 알아보는데 있다. 방법 : DSM-Ⅳ를 기준으로 정신분열병으로 진단받고 입원 치료중인 환자 160명을 대상으로 하여 PANSS를 평가하였고, 이를 주성분분석 방식을 사용하여 각각 직각회전(varimax, equmax) 방식으로 회전하여 요인분석을 실시하였으며, 신뢰도는 Cronbach's α로 검증하였다. 결과 : 탐색적 요인 분석에서 5개 요인구조가 전체 변량의 61.7%를 설명하는 것으로 나타났다. [음성증상(요인1), 흥분증상(요인2), 양성증상(요인3), 와해증상(요인4), 우울/불안(요인5)]. 결론 : 한국의 정신분열병 환자들을 대상으로 실시한 한국어판 PANSS의 요인분석은 이전에 행해진 기존 문헌에서 제안된 요인구조와 마찬가지로 5개의 요인 구조가 나타났다. 이는 PANSS를 이용한 임상 평가나 연구에서 3 요인 보다는 5개의 요인 구조를 유용하게 사용할 있음을 시사하는 결과이다. 추후 확정적 요인분석을 통해서 이 모델의 적합도를 확인할 필요가 있다.; Objective: Positive and Negative Syndrome Scale for Schizophrenia (PANSS) is the core research instrument for measuring psychopathology of schizophrenia and many different language versions are available worldwide. Despite excellent reliability and validity of Korean PANSS was reported, to our knowledge, factorial validity was not retested. This study tested the initial factor structure of a Korean PANSS in schizophrenic inpatient population. Methods: Principal component factor analysis was peformed from data pool of 160 schizophrenic patients. Results: Exploratory analysis revealed five factor structure of the scale with total variance of 61.7%: Negative (Factor 1), Excited (Factor 2), Positive(Factor 3), Disorganized(Factor 4) and Depression/Anxiety symptoms (Factor 5). Conclusion: Despite heterogeneity of the sample, our data support five factor model underlying schizophrenic symptomatology as assessed by the PANSS. The five-factor structure of the PANSS in Korean schizophrenic patients enables further elucidation of the various clinical aspects of schizophrenia. The Korean version of the PANSS proved the valid five factor structure and in part supported a cross cultural factorial stability of PANSS.
