하사근의 해부학적 연구

Abstract

목적: 사체해부를 통하여 통하여 하사근의 기시부, 근부, 부착부의 해부학적 구조를 알고자 하였다. 대상 및 방법: 사체 20구 40안을 대상으로 두개골 안와에서 누낭오목의 정점과 안와하연에서부터 하사근의 기시부까지의 거리를 측정하고 하사근이 안와하연과 이루는 각도와 너비를 측정하였다. 안구에서 부착부의 위치와 너비, 개수, 근부의 너비, 그리고 부착부위부터 하사근 신경이 부착하는 위치까지의 거리를 측정하였다. 결과: 남성이 11구 여성이 9구 였고 사망당시 평균 연령은 63±16세였다. 기시부는 너비가 2.72±0.78mm로 누낭오목의 정점에서 17.48±1.92mm, 안와하연에서 1.12±0.81mm 떨어진 거리에 위치하고 있었고 안와하연과 28.87±10.51도를 이루었다. 부착부는 너비가 9.27±1.35mm로 시신경으로부터 4.63±1.57mm, 외직근 부착부로부터 9.17±1.14mm떨어져 있었다. 근부의 너비는 7.08±1.14mm, 하사근 신경은 부착부로부터 17.15±2.99mm 에 부착하였다. 하사근의 기시부가 안와 하연과 이루는 각도가 클수록 하사근은 후방으로 부착하였고 2개에서 4개까지의 다수 부착부를 67.5%에서 관찰하였다. 또한 하사근의 부착 형태 및 개수는 50%에서 양안에서 차이가 있었다. 결론: 하사근 수술시 항상 다수의 근부가 있을 수 있다는 것과 양안에서 다를 수 있다는 점을 염두에 두어야 한다; Object : To define the anatomy and anatomical variations of the inferior oblique muscle (IO) and classify in insertional pattern with photographs. Method : This study included 40 intact orbits of 20 embalmed adult cadavers. The IO muscle dissected from the origin to the insertion. We measured the IO origin and insertion. Results : The distance from the apex of lacrimal fossa to the insertion of inferior oblique and the width of IO origin were 17.48mm, and 1.12mm, respectively. The angle of the IO relative to the inferior orbital rim was 28.87°. The width of IO insertion was 9.27mm. The distance from the optic nerve lateral edge to the posterior border of IO and the distance from the lateral rectus (LR) insertion to the anterior border of IO were 4.63mm and 9.17mm, respectively. As the angle of IO origin relative to inferior orbital rim was larger, IO inserted more posteriorly. 67.5% of IO had double, triple, and quadriple insertions. Insertional pattern of IO was different in both eyes in 50%. Conclusions : Ocular surgeons who not frequently perform IO surgery should be aware of the possibility of distinct bellies and examine the inferior temporal quadrant of the sclera carefully to avoid missing the medial belly of IO at the time of surgery
