Abstract
Object. Endovascular coil embolization for intracranial aneurysms has gained popularity because of its low morbidity and mortality in short-term context. Headache is sometimes brought about or worsened however, after endovascular treatment, and this complaint may lead to perplexing situation albeit infrequently. Many facets of outcome after endovascular treatment of intracranial aneurysm were revealed and verified, but such headache was not thoroughly evaluated yet. The aim of this study is to estimate the practical incidence and risk factors of postoperative headache. Methods. Patients who underwent endovascular treatment of intracranial aneurysm were enrolled retrospectively according to our protocol. Between May 2005 and July 2011, there were 146 cases with ruptured (n=62) or unruptured (n=84) intracranial aneurysms in 137 patients who underwent endovascular embolization at the authors institution. Factors based on patients’ demography, anatomical and radiological features of the lesions, treatment, utilized devices and outcome were investigated, and statistically verified. Results. Headache occurred or exacerbated in 19 patients (13%) harboring 9 ruptured and 10 unruptured aneurysms, respectively. Among several factors, only stent-assisted coiling was significantly related with post-embolization headache (p<0.05). Conclusions. In the current study, stent-assisted coiling was important risk factor for headache in patients undergoing endovascular coil embolization. But, other factors which did not reach statistical significance in post-embolization headache. Further investigations are still necessary.
