한국판 개정판 일생 스트레스 사건 체크리스트의 신뢰도 및 타당도 : 불안 장애 및 우울장애 외래 환자 대상

Abstract

Background: Lifetime traumatic events and adverse stressful experiences have placed as one of major etiological or risk factors in adult physical and mental health. Thus, the screening and assessment of past trauma are need as a routine procedure early in treatment; development of a psychological instrument that is easily administered and has good psychometric properties is warranted. This study investigated the reliability and validity of the Korean version of Lifetime Stressor Checklist-Revised (LSC-R) in psychiatric patients. Methods: A final sample of 258 outpatients with anxiety or depressive disorders patients were recruited at a university-affiliated teaching hospital. The self-report data included the Life Events Checklist (LEC), the Symptom Checklist-90-Revised, the Beck Depression Inventory, the Zung Self-rating Depression and Anxiety Scales, and the Impact of Events Scale-Revised in addition to LSC-R. Another convenient sample of 50 medical students were administered LSC-R for two occasions by three week-interval for test-retest reliability. Results: Mean Kappa for temporal stability was 0.65 and Cronbach alpha was 0.72. Concurrent validity was excellent with corresponding event items on LEC. Convergent validity was good with symptoms of PTSD, depression,and anxiety. An exploratory factor analysis revealed 11 factors explaining 64.3 % of total variance. Conclusion: This study revealed good psychometric properities of the Korean version of LSC-R further supporting its use of clinical research and practice. |배경 : 일생 외상 사건 및 부적 스트레스 경험은 서인의 신체적 정신적 건강에 있어서 하나의 주요한 병인론적 또는 위험 요소로 자리잡았다. 따라서, 치료에 있어서 과거의 외상을 평가하고 선별하는 것이 일상적인 과정이 되고 있다. 탁월한 정신 측정학적 특질을 가진 정신 측정 도구의 발달로 임상에서 이런 도구들을 쉽게 적용할 수 있다. 이번 연구는 한국판 개정판 일생 스트레스 사건 체크리스트의 신뢰도 및 타당도를 알아보고자 정신측정학적 연구를 시행하였다. 방법: 한양대학교 구리병원 정신건강의학과 외래에 진료 목적으로 방문한 외래 환자들 우울 또는 불안 장애를 가진 총 258명의 환자들의 자기보고식 평가자료를 사용하였다. 자기 보고식 검사는 개정판 일생 스트레스 사건 체크 리스트에 더하여 생활 사건 체크리스트, 벡 우울 척도, 간이 정신 진단검사, 개정판 사건 충격 척도, 자기 보고식 우울 척도, 자기 보고식 불안 척도를 이용하였다. 또한, 50명의 의과대학생을 대상으로 3주간격으로 검사-재검사 신뢰도를 측정하였다. 결과 : 안정성에 대한 평균 Kappa 값은 0.65, Cronbach alpha 값은 0.72로 측정되었다. 생활 사건 체크리스트의 사건 항목들과는 탁월한 공존 타당도를 보였다. 외상후 스트레스 장애의 증상, 우울, 불안 척도와도 양호한 공존 타당도를 보였다. 주 성분 분석에서는 총 분산의 64.3%를 설명하는 11개의 요인들이 추출되었다. 결론 : 이번 연구에서 한국판 개정판 일생 스트레스 사건 체크리스트가 임상 및 연구에 사용될 수 있는 좋은 정신 측정학적 특질을 가진 도구라는 사실을 알 수 있었다.; development of a psychological instrument that is easily administered and has good psychometric properties is warranted.
