골다공증성 척추 압박 골절과 비타민 D 결핍의 정도

Abstract

목적: 골다공증성 척추 압박 골절 환자에 있어 비타민 D 부족 정도를 알아보고, 비타민 D와 여러 변수 간의 상관관계에 대해 알아보고자 한다. 대상 및 방법: 2008년 10월부터 2011년 6월까지 골다공증성 척추 압박 골절로 진단 받은 총 134명의 환자를 대상으로 하였다. 25(OH)vitamin D3를 측정하여 환자의 비타민 D 상태를 평가하였고, 성별, 골절된 추체 수, 거주 환경, 계절에 따른 비타민 D 수치를 비교 분석하였으며, 비타민 D와 나이, 골밀도, 골대사 지표와의 상관관계를 조사하였다. 결과: 골다공증성 척추 압박 골절 환자의 87명(65%)에서 비타민 D 부족이 관찰되었다. 비타민 D는 겨울철에 가장 낮은 수치를 여름철에 가장 높은 수치를 보였고, 일반 가정에 거주하는 경우가 요양 시설에 거주하는 경우보다 통계학적으로 유의하게 높은 수치를 보였으며(p=0.032), 나이와는 유의한 음의 상관관계를(r=-0.201, p=0.02), 골밀도와는 유의한 양의 상관관계를 보였다(r=0.217,p=0.012). 결론: 골다공증성 척추 압박 골절 환자에 있어 비타민 D 수준을 평가하는 것이 치료를 계획하는 데 도움이 될 것으로 생각되며, 부족 시 적절한 일광 노출과 비타민 D의 보충을 고려해 보는 것이 좋을 것으로 생각된다.Purpose: The aim of the present study was to evaluate the degree of serum vitamin D deficiency in patients with osteoporotic spinal compression fracture and correlation of serum vitamin D level with several variables. Materials and Methods: The medical records of 134 patients with osteoporotic spinal compression fracture, diagnosed at our hospital between October 2008 and June 2011, were reviewed. Serum 25(OH)vitamin D3 was used to evaluate the status of vitamin D level. Serum 25(OH)vitamin D3 level was compared and analyzed according to sex, the number of fractured vertebral body, living environment, and the season of injury. The correlation between vitamin D level and age, bone mineral density, and bone turnover marker were evaluated. Results: In the serum 25(OH)vitamin D3, 87 patients (65%) associated with osteoporotic spinal compression fracture had an insufficient level. Vitamin D level was the lowest in winter, the highest in summer, and significantly higher in the living home than nursing home. Vitamin D level was negatively correlated with age (r=?0.201, p=0.02) and positively correlated with bone mineral density (r=0.217, p=0.012). Conclusion: Evaluation of vitamin D level in osteoporotic vertebral compression fracture patients may be helpful in planning the treatment of the patients. For insufficient vitamin D level, the adequate sun exposure and supplement of vitamin D may be used.
