임플란트 주위염에 대한 최근 지식들-Schwarz의 논문들을 중심으로

Abstract

임플란트학의 주요 관심사가 임플란트의 식립이나 보철치료에서 임플란트의 유지관리로 변화함에 따라 최근 임플란트 주위염 (peri-implantitis)이 임상가들 사이의 화두로 떠오르고 있다1). 임플 란트에서는 치아에서 발생하는 치주염과 같은 염증성 질환이 발생하 지 않을 것이라는 근거 없는 기대들을 비웃기라도 하듯이 수많은 연구 들이 임플란트 주위 점막염(peri-implant mucositis)과 임플란트 주위염에 대한 보고들을 하루가 다르게 쏟아 내고 있다. 지금까지 밝혀 진 바에 따르면, 임플란트 주위염은 일종의 세균성 감염질환으로 임상 적으로 탐침 깊이(probing depth)는 4 mm 이상이며 탐침 시 출혈 (bleeding on probing, BOP), 심한 경우 배농(suppuration)의 소견을 보이고 방사선학적으로 임플란트 주위의 골소실을 동반하는 것으로 알려져 있다2). 교합적인 과부하도 임플란트 주위염의 진행에 상당 부분 역할을 하는 것으로 보인다3).As the supportive care of dental implants is emphasized these days, the disease of peri-implant tissue draws more attentions of implant clinicians. This article reviewed the current knowledge of peri-implant mucositis and peri-implantitis focusing on Frank Schwarz's clinical and basic researches.
