개방성 하지 골절에서 음압 상처 치료 후 지연성 연부조직 재건술에 대한 치료 결과

Abstract

목적: 하지의 개방성 골절에서 음압 상처 치료 후의 지연성 연부조직 재건술의 임상적 결과에 대해 알아보고자 하였다.대상 및 방법: 2007년 6월부터 2011년 2월까지 15명의 환자에서 발생한 16예의 개방성 하지 골절을 대상으로 하였다. 남성이 13명, 여성이 2명이었으며, 평균연령은 39.0세(7-65세)였다. 본 연구에서는 음압 치료 후 시행한 지연성 연부조직 재건술의 이식편 상태, 기능적 결과, 골유합 및 창상 감염 등의 합병증에 대해 확인하였다.결과: 수상 후 평균 29.3일(9-50일)간 음압 상처 치료를 시행하였으며, 수상 후 평균 36.1일(21-60일)에 연부조직 재건술을 시행하였다.5예에서 피부이식술, 8예에서 유리 피판술 그리고 3예에서 국소 피판술을 시행하였으며, 최종 추시 시 이식편의 실패는 관찰되지 않았다.골유합은 평균 5.2개월(3-8개월)에 관찰되었으며, 2예에서 지연 유합이 발생하였으나 최종 추시 시 골유합을 확인할 수 있었다.결론: 동반 손상으로 인해 조기 연부조직 재건술을 시행할 수 없는 개방성 하지 골절에서 음압 상처 치료 후 지연성 연부조직 재건술을 시행하는 것은 이식편의 실패, 골절 부위의 불유합 등의 합병증을 줄일 수 있어 유용한 치료 대안으로 생각한다.Purpose: To evaluate the delayed soft tissue coverage after previous negative wound therapy in treating wounds associated with open lower extremity fracture.Materials and Methods: Between June 2007 and February 2011, delayed soft tissue coverage of open lower extremity fractures after previous use of a negative pressure vacuum device, were performed in 15 consecutive patients. Patients included 13 male and 2 female, and their mean age was 39.0 (7-65) years old. Information on the time from injury to defi nitive wound coverage, type of coverage, functional result, bone union, and complications such as infection were collected.Results: The vacuum-assisted closure devices was applied for an average of 29.3 (9-50) days, and the mean time from injury to defi nitive wound was 36.1 (21-60) days. Skin graft coverage was performed in 5 cases, free fl ap in 8 cases, and local fl ap in 3 cases.The mean time to radiographic union was 5.2 (3-8) months and delayed union occurred in 2 cases.Conclusion: Delayed soft tissue coverage after previous negative wound therapy was considered an alternative treatment method in open lower extremity fractures associated with severe concomitant injury.
