전신성 홍반성 루푸스 환자에서 발생된 Group B Salmonella 균에 의한 감염성 슬관절염

Abstract

전신성 홍반성 루푸스 환자에게 감염은 가장 중요한 사망 원인이며, 박테리아에 의한 감염이 대부분을 차지하고 있다. 살모넬라는 세망내 피기관의 부전, 면역 억제 제재의 사용 등으로 면역이 저하된 전신성 홍반성 루푸스 환자에서 균혈증을 유발하는 흔한 균주 증 하나이나 화농성 관절염을 일으키는 경우는 극히 드문 것으로 보고되어 있다. 저자들은 전신성 홍반성 루프스 환자에서 B군 살모넬라균에 의한 화농성 슬관절염을 경험하였으며, 관절경하 세척술과 항생제로 치료한 증례를 보고하는 바이다. Infection is a leading cause of death in patients with systemic lupus erythematosus (SLE). Insufficiency of the reticuloendothelial system, immunosuppressive therapy and inadequate opsonization are the main predisposing factors for infection. Salmonella infection is one of the most common opportunistic bacterial infections in patients with SLE and it can provoke a bacteremia or a localized infection. We report here on a rare case of septic knee arthritis caused by Group B Salmonella in a SLE patient, and this was treated with arthroscopic irrigation and antibiotics.
