췌장 낭성 종양과 감별이 필요하였던 췌장 암육종

Abstract

췌장의 낭성 종양은 현재 영상 기술의 발달로 인하여 우연히 발견되는 경우가 많다. 이러한 낭성 종양은 예후가 좋은 편으로 진단 및 치료적인 목적의 수술이 많이 행해지고 있다. 그러나 치료 방법을 결정할 때는 본 증례와 같이 암육종 등의 흔하지 않은 원인도 같이 고려할 필요가 있다. 암육종은 상피 조직과 간엽성 조직으로 구성된 악성 종양으로 현재까지 췌장에서는 흔치 않으며, 보고된 예들에서는 대부분 예후가 좋지 않아 수술 후 6개월 이내에 타 장기로의 전이와 범발성 혈관 내 응고장애 등으로 사망한 경우가 많았고, 수술 이후의 항암 치료도 명확한 치료지침이 없어 다른 췌장 샘암종과 같이 gemcitabine과 같은 치료를 하고 있다. 저자들은 췌미부에 발생한 암육종 환자 1예를 경험하였기에 문헌고찰과 함께 보고하는 바이다.A 66-year-old woman presented with severe abdominal pain. Abdominal computed tomography revealed a large multicystic mass with a solid component at the tail portion of the pancreas. Distal pancreatectomy was performed with a preoperative diagnosis of mucinous cystic neoplasm. The solid component of the lesion showed both adenomatous and sarcomatous characteristics on histological examination. Immunohistochemical studies showed epithelial cells positive for cytokeratin and an immunoreactive sarcomatous component positive for vimentin. We present here a case of carcinosarcoma of the pancreas with a review of the literature. (Korean J Med 2011;80:S131-S136)
