Abstract
Purpose: The mechanism of occurrence of interstitial cystitis/bladder pain syndrome (IC/BPS) is unclear, and the treatment remains challenging. Pentosan polysulfate (PPS) is an oral treatment for IC/BPS and is known to improve the symptoms by restoring the glycosaminoglycan layer that protects the bladder epithelium, though the drug effect is not significant. A recent clinical study reported that phosphodiesterase (PDE) 5 inhibitor, an erectile dysfunction treatment, improved the symptoms of IC/BPS patients. The present study aims at comparing changes in bladder tissues after PPS monotherapy and PDE5 inhibitor treatment, respectively, using hydrochloric acid-induced IC/BPS model rats. Methods: Female Sprague-Dawley rats (8-week-old) were categorized into a sham group (n=3) and an experimental group (n=24). An interstitial cystitis model was generated by injecting hydrochloric acid into the bladder of the experimental individuals. After four weeks, six rats were orally administered with distilled water daily for eight weeks (control group), 12 rats were administered with 25 mg/kg PPS (PPS group), and six rats were administered with PPS and 2 mg/kg tadalafil (mix group). The bladder tissues were stained with hematoxylin-eosin (H&E) to evaluate the inflammation pattern of bladder tissues; the number of mast cells/mm2 was determined through toluidine blue staining. Fibrous tissues and apoptosis were observed with Masson’s trichrome staining and terminal deoxynucleotidyl transferase dUTP nock end labeling (TUNEL) staining, respectively. The color intensities recorded through image analysis was compared semi-quantitatively. Results: Edema occurrence was decreased in the PPS group compared to the control group, according to H&E staining (2.7±0.3, 1.2±0.7, respectively, P=0.007). The number of mast cells was also lower in the PPS group than in the control group (8.48±4.25 vs 3.75±1.32, P=0.010). Color intensity analysis results after Masson’s trichrome staining (93.17±5.79, 77.77±8.50, P=0.002) and TUNEL staining (127.27±14.42, 80.20±15.92, P<0.001) showed similar results. In the mix group, inflammation was reduced on average, but there was no statistically significant difference from that in the control group and the PPS group. Conclusion: It was confirmed that PPS histologically reduced the inflammatory response in the IC model. Since additional administration of tadalafil may hinder the effects of PPS, its use should be carefully considered.|연구의 목적: 간질성방광염/방광통증후군(Interstitial Cystitis/ Bladder Pain Syndrome, IC/BPS)의 발생 기전은 불분명하고 치료 또한 까다로운 편이다. 펜토산 폴리설페이트(Pentosan Polysulfate, PPS)는 IC/BPS의 경구 치료제로 방광 상피를 보호하는 Glycosaminoglycan layer를 회복시켜 IC/BPS의 증상을 개선하는 것으로 알려져 있으나 약물 효과는 크지 않다. 최근 IC/BPS 환자에 발기부전 치료제인 phosphodiesterase (PDE) 5 inhibitor를 투여해 본 결과 증상 개선에 효과가 있다는 임상 연구가 있기에 저자는 염산으로 유발한 IC/BPS 모델 rat을 이용한 동물 실험을 통하여 PPS 단독요법과 PDE5 inhibitor첨가시 방광 조직 변화를 비교해 보고자 하였다. 연구방법: 8주령 female Sprague-Dawley rat 27마리를 3마리의 sham 군과, 24마리의 실험군으로 분류하였다. 실험군은 방광내 염산을 주입하여 간질성 방광염 모델을 만들고 4주 경과 후 6마리는 control군으로 8주간 매일 증류수를 경구투여, 12마리는 PPS군으로 PPS 25 mg/kg을 투여, 6마리는 mix군으로 tadalafil 2 mg/kg을 추가 투여하였다. 투여 종료 시점에 채취된 조직은 hematoxylin-eosin (H-E) 염색 후 방광 조직의 염증 양상을 확인하였고, toluidine blue 염색을 통하여 제곱밀리미터당 비만세포의 수를 조사하였다. 또한 Masson’s trichrome 염색으로 섬유조직을, terminal deoxynucleotidyl transferase dUTP nock end labeling (TUNEL) 염색으로 세포자살을 관찰하고 이미지 분석을 통해 확인된 색 강도를 반정량적으로 비교하였다. 실험결과: Kruskal-Wallis 검정 후 사후 검정(Bonferroni test) 결과 H-E 염색에서 control군에 비해 PPS군은 부종이 감소하였다 (2.7±0.3, 1.2±0.7, P=0.007). 비만세포 수 역시 control군보다 PPS군이 적었다(8.48±4.25 vs 3.75±1.32, P=0.010). Masson’s trichrome 염색 후 색 강도 분석 결과나 (93.17±5.79, 77.77±8.50, P=0.002) TUNEL 염색 결과 역시 유사한 결과를 보였다(127.27±14.42, 80.20±15.92, P<0.001). mix군은 평균적으로 염증이 감소하였으나 통계적으로 control군 및 PPS군과 유의한 차이를 보이지 않았다. 결론: PPS는 IC 모델에서 염증반응을 감소시키는 것을 실험적으로 확인하였기에 IC 환자에서 효과적인 약물 치료이다. tadalafil 추가 투여는 유의한 염증 회복이 나타나지 않아 그 사용을 신중하게 고려해야 한다.
