Abstract
서론: 일부 약물의 경구생체이용률은 위산도의 영향을 받는 것으로 알려져 있다. 본 연구에서는 국내 건강 피험자를 대상으로 위산도가 난용성 약알칼리성 약물인 dipyridamole의 약동학에 미치는 영향을 알아보고자 하였다. 대상 및 방법: 정상위산도(pH ≤5.0) 18명과 저위산증(pH >5.0) 9명의 건강 피험자를 대상으로 하였고, 위내시경을 통하여 위액을 흡입 채취하여 직접 공복 위산도를 측정하였다. Dipyridamole 75 mg을 1회 경구 투여한 후 혈장 중 dipyridamole 농도를 HPLC-UV법으로 측정하였다. AUC_(t)는 사다리꼴 공식으로 산출하였고, 다른 약동학적 지표인 C_(max), T_(max) 및 t_(1/2)를 측정하였다. Wilcoxon rank sum test를 시행하여 유의수준 5%에서 양 군의 약동학적 지표들을 비교하였다. 결과: 정상위산도군에 비해 저위산증군에서 AUC_(t)와 C_(max)는 약 3.5배 감소하였고(P=0.0004), T_(max)와 t_(1/2)는 양 군간에 의미 있는 차이가 없었다. 결론: 정상위산도군과 비교하여 저위산증 피험자에서 dipyridamole의 경구생체이용률은 의미 있게 감소하였다.; Introduction: There have been reported that oral bioavailability of some drugs is variable and dependent on the gastric pH. This study was designed to examine the effect of gastric pH on pharmacokinetics of dipyridamole, a poorly soluble weak base, in Korean healthy volunteers. Methods: The study was conducted in 18 subjects with normal gastric acidity (pH £5.0) and 9 subjects with hypochlorhydria (pH >5.0). Fasting gastric pH was directly measured with aspirated gastric juice through endoscopy. The concentration of dipyridamole in plasma was determined with a HPLC-UV method after a single oral dose of 75 mg dipyridamole. AUC_(t) was determined by a trapezoidal method with extrapolation. Other pharmacokinetic parameters, such as C_(max), T_(max) and t_(1/2), were measured. Wilcoxon rank sum test was performed to ascertain differences in pharmacokinetic parameters obtained in both groups. Results: AUC_(t) and C_(max) were about 3.5 times deceased in hypochlorhydric subjects compared with subjects of normal gastric acidity (P=0.0004). There was no significant difference of T_(max) and t_(1/2) in both groups. Conclusion: Oral bioavailability of dipyridamole was significantly decreased in hypochlorhydric subjects.
