Abstract
2017년 고령사회로 접어들면서 요양병원의 역할의 비중이 커짐에 따라 요양병원의 간호의 질에 대한 관심 또한 높아지고 있는 가운데, 국내의 요양병원의 간호행위는 간호사 인력부족으로 위임이 빈번하게 일어나고 있다. 기존의 연구는 요양병원의 간호행위의 위임실태 및 인식에 대해 양적 분석에만 그친 것에 반해, 본 연구는 양적 조사에 위임현상의 인식에 대해 요양병원에 근무하는 간호사와 간호조무사를 대상으로 질적으로 심층적으로 탐색하여 요양병원에서 간호행위의 체계적인 위임 지침을 마련하는데 기초자료를 제공하고자 시도되었다. 본 연구는 방법론적 Triangulation 연구설계로써, 요양병원의 간호행위 수행실태와 위임정도 및 위임가능여부에 대한 인식을 양적 연구방법으로, 요양병원 간호행위 위임 현상에 대한 간호사와 간호조무사의 인식을 질적 면담으로 파악한 연구이다. 자료수집기간은 2018년 1월 5일부터 2018년 2월 20일까지이며, 양적연구 대상자는 서울특별시, 전라북도 전주시, 경상북도 경주시, 대구광역시, 부산광역시, 울산광역시의 20개의 간호인력 1등급인 요양병원의 간호사와 간호조무사 360명을 대상으로 하였고 연구 동의를 받고 설문지를 통해 자료를 수집하였다. 질적면담 대상자는 양적연구에 참여한 대상자 중 전라북도 전주시와 울산광역시 간호사 8명, 간호조무사 8명을 편의 표집하였으며 4명씩 4군을 1시간씩 4번에 걸친 포커스그룹 면담방법을 통해 자료를 수집하였다. 연구도구로는 요양병원의 간호행위 수행실태 및 위임정도, 위임가능여부에 대한 인식을 측정하는 도구는 장은주, 김수현 (2017)이 개발한 총 14개 간호업무 영역과 119개의 세부 간호활동을 바탕으로 한 설문지로 양적자료를 측정하였고, 포커스그룹 면담을 통해 질적 자료를 수집하였다. 자료분석은 양적자료는 SPSS 21.0 통계프로그램을 사용하여 기술통계와 빈도분석, 독립표본 t-test, 카이제곱검정을 실시하였으며, 질적자료는 Berg & Lune (2004)의 주제 분석 방법을 통해 분석하였다. 연구결과로는 요양병원에서 수행되는 간호행위 중 환자정보의 수집관리 업무영역의 간호과정, 환자평가표 작성하기와 각종 천자, 동맥혈산소분압검사는 전혀 위임이 일어나지 않았고, 활력징후 측정하기와 혈당 측정하기 및 침상안정유지 돕기 항목은 100%의 위임이 일어나고 있었으며, 이를 제외한 나머지 110개의 간호행위는 7.2%-98.3% 사이에서 간호사와 위임받은 간호조무사가 수행하고 있는 것으로 나타났다. 또한, 돌봄간호를 제외하고는 기본간호, 호흡간호, 안전응급관리 등 대부분의 간호업무영역에서 간호사가 간호조무사에게 위임하고 있다고 인식하는 정도보다 간호조무사가 간호사에게 위임 받고 있다고 인식하는 정도가 더 높은 것으로 나타났다. 요양병원 간호행위의 위임가능여부에 대해서는 간호사와 간호조무사가 각자 위임 가능하다고 더 높게 인식한 업무들에 있어 인식의 차이를 보였다. 간호사가 간호조무사에 비해 위임 가능하다고 더 높게 인식한 것으로 나타난 간호행위로는 돌봄간호영역의 간호행위들과 분변매복제거하기 등의 대부분이 비침습적인 간호행위들로 제한된 반면, 간호조무사가 간호사에 비해 위임 가능하다고 더 높게 인식한 간호행위로는 기본간호, 호흡간호, 안전응급관리, 감염관리, 환자정보의 수집관리, 기본기능유지관리, 수혈과 마약관리를 제외한 투약간호영역에 속하는 간호행위들로 간호사가 위임가능하다고 인식한 간호행위보다 광범위하게 나타났다. 요양병원에 근무하는 간호사 8명, 간호조무사 8명을 대상으로 포커스그룹 면담을 실시한 결과로 요양병원 위임 현상에 대해 요양병원에서 간호행위 위임이 발생하는 원인과 요양병원에서 간호행위 위임 과정 중 경험한 문제점 및 요양병원에서 간호행위의 위임 과정에서 양질의 간호서비스제공을 위해 개선되어야 할 점이 주제로 도출되었다. 이와 관련하여 간호사와 간호조무사의 인식에는 다각도로 일치점과 차이점을 보였는데, 궁극적으로는 요양병원 간호행위의 위임절차 체계를 마련해야 한다고 진술하였다. 본 연구를 통해 요양병원의 간호행위는 다양한 수준에서 위임받은 간호조무사가 수행하고 있었고, 위임을 하는 입장과 위임을 받는 입장에 따라 위임정도도 다르게 인식될 수 있음을 알 수 있었다. 그리고 요양병원에서 수행되는 간호행위의 위임가능여부 및 위임현상에 대해서 간호사와 간호조무사의 인식의 공통점과 차이점을 확인할 수 있었다. 또한, 단순히 요양병원의 간호행위 위임실태를 아는 것을 넘어 당사자인 요양병원 간호사와 간호조무사의 경험을 바탕으로 위임 현상을 심층적으로 이해할 수 있었다. 이러한 측면에서 본 연구결과가 궁극적으로 요양병원의 위임 지침이 마련되는데 기초자료로 활용될 것으로 기대한다. 주요 개념어:요양병원, 간호사, 간호조무사, 간호행위, 위임 인식, 방법론적 triangulation; ABSTRACT Delegation of Nursing Activities and Perception in Long-term care Hospitals Kim, Sin Jung Dept. of Nursing The Graduate School Hanyang University Directed by Hong, Gwi-Ryung Son Ph.D, RN Entering the aged society in 2017, as the role of long-term care hospital increases, the quality of nursing in long-term care hospital is also increasing, and the nursing activity at long-term care hospital in Korea is being delegated due to the shortage of registered nurses. This research tries to provide basic data for giving systemic delegation-guidelines in nursing activities by conducting a qualitative and in-depth search on the registered nurses and nurse assistants at long-term care hospital with the perception of delegation to the quantitative investigation. This research is a methodological triangulation research that identifies quantitative method research, the perception about execution status of nursing activities, range of delegation and possibility of delegation between registered nurses and nurse assistants in long-term care hospital, also identifies quality interview, the perception of registered nurses and nurse assistants about the delegation status of nursing activities at long-term care hospital. The data collection period is from January 5, 2018 to February 20, 2018. The quantitative research subjects were 360 registered nurses and nursing assistants of long-term care hospitals in the 1st grade of 20 regions. Subjects of qualitative interview were collected through the focus-group interview grouping 4 people in 4 group, 1 hour, 4 times with 8 registered nurses and 8 nurse assistants who participated in quantitative research by the convenience sampling. Research method was measuring execution status of nursing activities, range of delegation and possibility of delegation in long-term care hospital, quantitative data was measured by survey which was on the basis of 14 parts of nursing practice and 119 details of nursing activities developed by jang eunju, kim suhyun(2017) and collected qualitative data through the focus-group. The data analysis was conducted using the SPSS 21.0 statistical program to technical statistics and frequency analysis, independent sample t-test, chi-square test, and Pearson's correlation analysis for quantitative data, and for the qualitative data was analyzed through the theme analysis of Berg & Lune (2004). As a result of the study, nursing process, patient evaluation chart check and various puncture and assisting arterial blood gas analysis in the area of patients’information collection & management among the nursing activities performed at long-term care hospital were not delegated at all. 100% delegation was taking place to measure the signs of vitality, measure blood glucose and help absolute bed rest. The remaining 110 nursing activities were performed by registered nurses and delegated nurse assistants between 7.2% and 98.3%. Also, except for the nursing care, most of nursing practices like basic nursing, breathing nursing, safe emergency management and etc were recognized that being delegated by nurse assistants from registered nurses were more higher than the recognition of delegating nurse assistants from registered nurses. could find out the different opinions between registered nurses and nurse assistants about the possibility of delegation of nursing activities at long-term care hospital. Most of the nursing activities in the personal care area and fecal impaction removal were limited to non-invasive nursing activities, which showed that the registered nurses perceived to be more delegable than nurse assistants. Whereas, the nursing activities that the nurse assistants perceives as more delegable than the registered nurses include the basic nursing care, respiratory care, safety emergency procedures, infection control, patient information collection & management, basic restorative care, except for blood transfusion and drug management of nursing activities perceived as nurse 's possible delegation. 3 topics about delegation status in long-term care hospital were came out through the focus-group interview with 8 registered nurses and 8 nurse assistants who works at long-term care hospital. 3 topics are the reason why delegation of nursing activities at long-term care hospital, the problem experienced during the delegation of nursing activities in long-term care hospital and improving nursing quality delegating of nursing activities at long-term care hospital. In this regard, the perception of registered nurses and nursing assistants showed various degrees of coincidence and difference, for both, ultimately system for the delegation of nursing activities at long-term care hospitals should be prepared. Through this research, beyond knowing the delegation status of nursing activities at long-term care hospital, could understand deeply about delegation status base to experience of registered nurses and nurse assistants working in a long-term care hospital. In this aspect, the result of the research expect to using basic data to prepare guidelines for the delegation of long-term care hospitals.
