대전자 동통증후군의 치료에 스테로이드 국소주사와 증식치료의 비교

Abstract

목 적: 대전자 동통 증후군에서 스테로이드 국소 주사와 증식 주사치료의 효과에 대해 비교하고자 하였다. 대상 및 방법: 2008년 5월부터 2011년 12월까지 대전자 동통 증후군 환자 137명을 국소 스테로이드와 증식 주사로 치료한 경우로 나누어, 치료 전, 치료 1, 6, 12개월 후에 visual analogue scale (VAS), Harris 고관절 점수(Harris hip score) 및 Roles and Maudsley 점수를 이용하여 치료 효과를 비교하였다. 결 과: 평균 VAS는 치료 시작 전 스테로이드 치료군, 증식 치료군에서 각각 6.2, 5.9점이었고 치료 1개월 후 각각 2.1, 4.3점으로 호전되었으며, 치료 12개월 후에 증식 치료군은 2.0점으로 호전되었으나 스테로이드 치료군은 4.9점으로 추시 1개월 후보다 악화되었다. 평균 Harris 고관절 점수는 치료 시작 전 각각 64.3, 61.0점이었으며 치료 1개월 후에는 각각 92.3, 83.4점이었고 치료 12개월 후에는 각각 80.8, 95.7점이었다. 매우 우수, 우수의 Roles and Maudsley 점수를 보인 환자 비율은 치료 1개월 후에서는 67.7%로 스테로이드 치료군에서 의미 있게 더 높았으며 치료 12개월 후에는 증식 치료군에서 83.3%로 유의하게 더 높았다. 결 론: 대전자 동통 증후군에서 증식 치료는 스테로이드 국소 주사 요법과 비교하여 더 장기간 지속되는 치료효과를 나타내어 좋은 치료 방법의 하나로 생각된다. Purpose: This study compares the effect of local corticosteroid injections and prolotherapy for the treatment of greater trochanteric pain syndrome. Materials and Methods: From May 2008 to December 2011, we divided 137 patients of greater trochanteric pain syndrome to local corticosteroid injection group and prolotherapy group. Effects of treatment were compared with visual analogue scale (VAS), Harris hip score and Roles and Maudsley score at pre-treatment, 1, 6, 12 months and post-treatment during the follow-up examinations. Results: Before the treatment, Average VAS were 6.2 and 5.9 in local corticosteroid injection group, and prolotherapy group respectively. The average VAS improved to 2.1 and 4.3 after 1 month of treatment. After 12 month of treatment, VAS in prolotherapy group was improved to 2.0 whereas VAS in local corticosteroid injection group worsened to 4.9, compared to the result of 1 month after treatment. Average Harris hip scores were 64.3 and 61.0 respectively before the ttreatment, 92.3 and 83.4 after 1 month of treatment and 80.3 and 95.7 after 12 months of treatment. The proportion of the patient who showed excellent or good Roles and Maudsley score of local corticosteroid injection group was significantly higher, 67.7%, after 1 month of treatment, but in 12 months after treatment, prolotherapy group was higher, 83.3%, than local corticosteroid injection group. Conclusion: Because prolotherapy, as a non-surgical treatment for greater trochanteric pain syndrome, showed longterm therapeutic effect and it is considered to be more effective treatment than the local corticosteroid injection.
