경골 근위간부 골절에서 최소 침습적 금속판 고정술을 이용한 치료

Abstract

목적: 경골 근위간부 골절의 유용한 치료 방법으로 시행되고 있는 최소 침습적 금속판 고정술의 유용성에 대해 알아보고자 한다. 대상 및 방법: 2003년 9월부터 2008년 6월까지 경골 근위간부 골절에 대해 최소 침습적 금속판 고정술을 시행하였던 32예를 대상으로 하였다. 전 환자에서 골유합 시기, 슬관절의 기능, 합병증 등을 조사하였다. 결과: 29예에서 골유합을 이루었고, 방사선학적 골유합 시기는 평균 18.3 (10∼28)주이었다. 최종 추시상 슬관절의 평균 운동범위는 134도이었으며, 1예에서 불유합, 2예에서 지연유합에 따른 금속판 파손이 발생하여 금속판 교체 및 자가장골 이식술이 필요하였다. 1예에서 표재성 감염, 1예에서 심부 감염이 발생하였으며, 심한 분쇄 골절이 있었던 2예에서 5도 이상의 부정정렬은 보였고, 14예에서 금속판에 의한 피부 자극 증상을 호소하였다. 결론: 경골 근위간부 골절의 치료에서 최소 침습적 금속판 고정술은 임상적으로 만족할 만한 결과를 보였으나 심한 분쇄골절에서는 지연유합, 불유합, 부정유합 등의 합병증이 발생할 수 있어 세심한 수술 술기 등 보다 적극적인 치료가 필요하다.Purpose: To report the results of patients treated by minimally invasive plate osteosynthesis (MIPO) for proximal tibial shaft fractures. Materials and Methods: From September 2003 to June 2008, thirty-two patients with proximal tibial shaft fractures weretreated by MIPO. There were 22 men and 10 women and mean age was 43.8 years (range; 21∼72 years). Follow-up was available for all patients and the mean follow-up period was 19.5 months (range; 12∼40 months). Duration of union, range of knee motion and postoperative complications were evaluated. Results: Twenty-nine patients (90.6%) healed after the MIPO technique. The mean duration of radiographic union was 18.3 weeks (range; 10∼28 weeks). The mean range of knee motion was 134 degrees at the last follow-up. There were 1 non-union, 2 delayed unions, 1 superficial infection, 1 deep infection, 2 malunions with more than 5 degrees of malalignment and 14 cases of skin irritation by plate. Conclusion: MIPO is an effective treatment for closed, proximal tibialshaft fractures. More aggressive treatment such as dual plating should be considered in fractures with severe comminution or bone loss.
