dc.description.abstract목적: 주관절 상과염의 보존적 치료에 대한 환자들의 순응도를 측정하고 순응도에 영향을 미치는 인자를 알아보고자 하였다.대상 및 방법: 2009년부터 2011년까지 상과염에 대해 보존적 치료를 받은 환자를 대상으로 하였다. 최초 내원으로부터 6개월 이상 경과 후 전화 설문을 통해 보존적 치료 유지 여부, 타 의료기관 치료 유무, 병에 대한 인식도, 최종 증상 호전유무 등을 조사하였다.결과: 총 104명의 환자 중 본원에서 권유한 보존적 치료를 유지한 환자는 82명(78.8%)이었다. 병에 대한 인식도, 치료 방법, 최초 통증 정도 등은 순응도와 통계적으로 유의한 관계가 없었다. 한편 최초 통증 정도, 순응도는 최종 증상 호전 유무에 대한 유의한 예측인자였다.결론: 상과염의 보존적 치료에 대한 순응도에 영향을 미치는 인자는 찾을 수 없었다. 그러나 순응도가 높을수록 최종 증상 호전을 얻는 경우가 많았으므로 순응도를 높이는 방법에 대한 추가 연구가 필요할 것으로 생각된다.Purpose: To measure the level of compliance to conservative treatment of epicondylitis and to identify factors affecting compliance.Methods: This study included 104 patients who were treated non-operatively for epicondylitis of the elbow from 2009 to 2011. At the time of initial visit, allpatients had thorough explanation about diagnosis, cause and pathophysiology of the disease, and were recommended to wait-and-see or to have steroid injection. At minimum of 6 month’s follow-up, we performed telephone survey including patient’s level of perception about the disease, maintenance of initial treatment method, and final symptom relief. Statistical analysis was performed using chi-square test, Fisher’s exact test and logistic regression analysis.Results: Of the 104 patients, 82 (78.8%) patients maintained initial treatment methods. 23 (22.1%) remembered exact diagnosis and mechanism of the disease, 49 (47.1%) remembered cause or mechanism but not about the diagnosis, and 32 (30.8%) didn’t know about the diagnosis nor mechanism. Compliances of those groups were 73.9% (17/23), 79.6% (39/49), and 81.3% (26/32), respectively, and the differences were not statistically significant (p=0.7934). Initial pain (odds ratio=0.7, p=0.0293) and the compliance (odds ratio=6.13, p=0.0015) were significant predictors of final symptom relief.Conclusion: There were no factors including perception level that proved to affect the compliance. Since higher compliance led to more successful outcome,further research on factors that increase compliance may be needed.en_US
