정신건강의학과 의사의 리더십, 윤리의식, 사회적 역할 및 향후 진출방향

Abstract

본 연구에서는 현 시점에서 정신건강의학과의 미래를 예측해 보고 거기에 바람직한 리더십, 윤리의식의 개념을 정립해 보며, 사회적 역할 및 향후 진출방향에 대해 알아보고자하였다. 연구 주제로 정신건강 영역 전문가 7명을 대상으로면접 조사를 시행하였고, 결과를 바탕으로 정신건강의학의미래 방향에 대한 세 가지 역할 모델을 설정하였다. 대한신경정신의학회 회원에게 역할 모델에 대한 이메일 설문조사를 시행하였으며 그 결과를 분석하였다. 면접 조사에서 여러전문가들은 다른 직역과의 공존하는 리더십, 리더십 교육프로그램 도입, 윤리의식 향상 방안, 지역사회와의 적극적 소통, 입법 및 행정 분야 전문가 양성 등 주제와 관련된 여러내용들을 강조하였다. 설문 조사는 32명이 응답하였으며 가장 선호하는 역할 모델은 지역사회리더 모델(46.9%, n=15)이었으며, 가장 선호하지 않는 모델은 정신건강증진 모델(34.4%,n=11)이었다. 각 역할 모델의 실현 및 발전 가능성에 대해서는 긍정적으로 응답하였으며, 역할 모델 발전에 적합한 직역주체는 임상의학 모델은 대학교수(37.5%, n=12), 지역사회리더 모델과 정신건강증진 모델은 국공립병원 의사[(28.1%,n=9), (43.8%, n=14)]라고 답했다. 역할 모델 수행을 위한 리더십과 전략에 대한 질문에 대해서도 다양한 의견들이 제시되었다. 향후 본 연구 결과들이 추후 정신건강의학과 의사의리더십과 윤리의식 지침 마련에 도움이 되기를 바라며, 미래의 변화된 우리의 역할에 대한 논의가 활발해지는 계기가마련되기를 기대한다. Objectives: The traditional roles of psychiatrists are being threatened according to the changesof requests from society. Psychiatrists need to rapidly adapt to new upcoming circumstances,improve leadership skills, and prepare for emerging ethical problems. The aim of this study is topredict and suggest what will happen in the future in the area of mental health, what role modelsmight be required, what we should do and how we can prepare. Methods: The authors interviewed seven mental health professionals on topics of leadership,participating roles of psychiatrists. Three models for psychiatrists were suggested : clinical model,community leader model, and mental health promoting model. Afterwards, a questionnaire oneach role model was distributed to members of the Korean Neuropsychiatric Association via emailand results were analyzed. Results: Experts and stakeholders suggested that the following components from the interviewwill be fostered in the future : leadership in collaboration with other communities, introduction ofleadership programs for young psychiatrists, strategies for sensitization of ethical issues, activecommunication with community partners, training professionals in legislation and administration. Regarding the questionnaire, 32 members responded. The most preferred role model was theCommunity leader model (46.9%, n=15), followed by the mental health promoting model (34.4%,n=11) and clinical model (15.6%, n=5). Most responders recognized that these issues areemerging and answered affirmatively on the developmental potential of each role model. A questionabout primary agents who are supposed to take charge of development of a suitable modelwas answered as follows : university professors for the clinical model (37.5%, n=12), psychiatristsworking for public hospitals both for community leader (28.1%, n=9), and mental healthpromoting model (43.8%, n=14). Various opinions on leadership and strategies for fulfilling therole model were proposed. Conclusion: This study investigated leadership, ethics, social roles, and future directions ofpsychiatrists in order to provide suggestions on how to properly respond to changes of environment. It is expected that this study will be helpful in establishment of guidelines for leadershipdevelopment and improving ethical orientation of psychiatrists, and will provide an opportunityfor Korean NeuroPsychiatric Association members to have deep consideration and radical debateon our transforming roles.
