dc.description.abstractBackground: Niemann-Pick C1-Like 1 (NPC1L1) functions as sterol transporter to mediate intestinal cholesterol absorption. Although the role of Liver X receptor (LXR) and Farnesoid X receptor (FXR) in hepatic steatosis is well known, its correlation with LXR/FXR and intestinal cholesterol circulation has not been thoroughly studied. We investigated the associations among LXR, FXR and NPC1 L1 as well as its correlation with intestinal cholesterol circulation in patients with non-alcoholic fatty liver disease (NAFLD). Methods: We evaluated clinical characteristics from 25 NAFLD and 28 control patients. We conducted upper endoscopy, colonoscopy and non-enhanced CT. Fatty liver diagnosed with decreased liver/spleen HU. We calculated the expression level of LXR and FXR in peripheral mono-nuclear cell by qPCR. Immunohistochemical analysis (ABCA1, ABCG5/8, NPC1L1, SREBP, LXR, FXR, CD36) was carried out on tissue samples of duodenum and ileum from NAFLD patients. Results: The expression of LXR (p = 0.01) and FXR (p = 0.03) in mononuclear cells increased in NAFLD group compared to the control group. Expression of LXR, FXR, NPC1L1, ABCA1, ABCG5/8, and SREBP in ileum was decreased in fatty liver disease. The expression of LXR in mononuclear cell showed a negative correlation with the expression of LXR, FXR, ABCA1, ABCG8, SREBP, NPC1L1 in ileum. Expression of NPC1L1 (p = 0.15), FXR (p = 0.30) in ileum was decreased in patients with hypercholesterolemia. Expression of NPC1L1 (p = 0.03), LXR (duodenum) and LXR (ileum) was decreased as triglyceride increased. Conclusion: We can predict the expression of NPC1 L1 in terminal ileum by measuring the expression of LXR and FXR in peripheral mononuclear cell.
