주요 정형외과 수술 후 발생하는 통증 정도의 비교

Abstract

목 적: 주요 정형외과 수술 후 발생하는 급성기 통증 정도를 전향적으로 비교 평가하였다. 대상 및 방법: 2013년 11월부터 2014년 1월까지 본원에서 시행된 척추 유합술(20명), 관절경하 회전근개 봉합술(20명), 고 관절 치환술(20명), 슬관절 전치환술(27명) 환자를 대상으로 하였다. 수술 후 6일까지 급성기의 통증 정도 및 통증에 대한 환자 만족도 및 수면의 질을 시각통증척도를 이용해 측정하여 비교 분석 하였다. 결 과: 술 후 1일째에 통증의 강도는 척추 유합술, 회전근개 봉합술, 고관절 치환술, 슬관절 전치환술의 순서로 높았다. 통증 조절의 만족도는 모든 기간에서 척추 유합술이 다른 수술보다 유의하게 낮은 만족도를 나타내었으며 통증으로 인한 수면의 질은 척추 유합술이 회전근개 봉합술과 비교하였을 때 술 후 2, 3, 6일째에 더 낮은 수면의 질을 나타내었으며, 슬관절 전치 환술과 비교하였을 때 술 후 1일부터 3일째까지 더 낮은 수면의 질을 나타내었다(P˂0.05). 결 론: 술 후 급성기 통증은 척추 유합술이 다른 군보다 심하였으며 통증 조절의 만족도와 수면의 질도 비슷한 양상으로 나타 났다. 예상되는 통증의 정도에 따라 술 후 급성기 통증 조절에 각별히 대비해야 하며 특히 척추 유합술의 경우 보다 적극적인 통증 조절 방법이 필요할 것이다. Purpose: To evaluate and compare the degree of acute pain after major orthopaedic surgeries. Materials and Methods: The study was conducted on 87 patients treated from November 2013 to January 2014. The patients were divided into four groups; Spinal fusion (20 patients), arthroscopic rotator cuff repair (20 patients), hip arthoplasty (20 patients) and total knee arthroplasty (27 patients). Visual analogue scale was used to measure the degree of acute pain, satisfaction with pain control, and sleep quality. Results: The mean pain VAS score of the first day was highest in spinal fusion group. Spinal fusion showed statistically significantly lower satisfaction with pain control during whole period of evaluation. Sleep quality was significantly lower in spinal fusion group on second, third, sixth postoperative day compared to shoulder arthroscopy group and first to third postoperative day compared to total knee arthroplasty group (P<0.05). Conclusion: Spinal fusion was the most painful surgery in acute postoperative period than other major orthpaedic surgeries. The degree of satisfaction with pain control and sleep quality showed similar pattern. Multimodal pain management strategies should be adopted for different major orthopaedic surgeries.
