Abstract
본 연구는 노인 만성질환자를 대상으로 건강정보이해능력과 디지털리터러시가 건강증진행위에 미치는 영향을 확인하고자 한 서술적 조사연구이다. 본 연구는 서울 소재 종합병원 내과에 입원 또는 외래를 방문한 65세 이상 만성질환자를 대상으로 IRB승인 후 2021년 7월 15일 부터 9월 15일까지 연구 참여에 동의한 120명의 자료수집을 시행하였다. 최종 112명의 자료를 분석하였으며, SPSS WIN 26.0 프로그램을 이용하여 빈도와 백분율, 평균, 표준편차, t-test, ANOVA, Scheffe test, Pearson Correlation Coefficients, Multiple regression analysis로 분석을 시행하였다. 연구결과는 다음과 같다. 1) 본 연구에서 건강정보이해능력은 평균 6.02(±2.40)점이었으며, 디지털리터러시 평균은 23.63(±9.11)점, 평점 평균은 2.95(±0.47)점이었다. 건강증진행위는 총합의 평균 74.28(±11.85)점으로, 평점 평균은 2.95(±0.47)점이다. 2) 건강정보이해능력은 나이(F=16.64, p=<.001), 결혼상태(t=3.16, p=.003), 동거가족유무(t=-2.53, p=.022), 종교(t=2.98, p=.004), 최종학력(F=10.36, p=.002), 주관적 건강상태(F=6.88, p=.002), 건강관심(F=2.72, p=.009), 인터넷 사용여부(t=4.98, p=<.001), 인터넷 사용시간(F=6.35, p=.006)에 따라 통계적으로 유의한 차이가 있었다. 3) 디지털리터러시는 나이(F=19.23, p=<.001), 결혼상태(t=2.96, p=.004), 동거가족유무(t=-2.51, p=.014), 종교(t=3.00, p=.004), 최종학력(F=7.91, p=<.001), 주관적 건강상태(F=5.45, p=.006), 건강관심(F=2.65, p=.010), 인터넷 사용여부(t=7.63, p=<.001), 인터넷 사용시간(F=14.66, p=<.001), 복용중인 약물의 개수(t=2.52, p=.013)에 따라 통계적으로 유의한 차이가 있었다. 4) 건강증진행위는 나이(F=4.07 p=.020), 최종학력(F=3.78, p=.013), 주관적 건강상태(F=5.79, p=.004), 장애진단여부(t=2.17, p=.032), 건강관심(F=3.65, p=.001), 인터넷 사용여부(t=3.30, p=.001), 인터넷 사용시간(F=15.27, p=<.001)에 따라 통계적으로 유의한 차이가 있었다. 5) 디지털리터러시와 건강정보이해능력(r=.551, p<.001), 건강증진행위와 건강정보이해능력(r=.527, p<.001), 건강증진행위와 디지털리터러시(r=.522, p<.001)는 유의한 양의 상관관계를 보였다. 6) 건강정보이해능력(β=.320, p=.005), 주당 인터넷 사용시간 3시간 이상(β=.248, p=.017), 디지털리터러시(β=.225, p=.043)순으로 건강증진행위에 영향을 미치는 유의한 변수로 나타났다. 다중회귀분석은 유의한 것으로 나타났으며(F=6.83, p<.000), 40.6%의 설명력을 나타냈다. 본 연구 결과, 노인 만성질환자의 건강정보이해능력과 디지털리터러시는 건강증진행위에 영향을 미치는 것으로 나타났다. 따라서 만성질환을 가진 노인인구의 건강과 안녕을 위해 노인인구의 건강정보이해능력과 인터넷 건강정보 활용을 증진시킬 수 있도록 중재프로그램 개발과 간호학적 접근이 필요할 것으로 생각된다.|This study is a descriptive research study to confirm the impact of H ealth Literacy and Digital Literacy on Health Promoting Behavior in Old er Adults with chronic diseases. After IRB approval, from July 15 to September 15, 2021 data of 120 peo ple who agreed to participate in the study were collected. The subjects of the study were chronically ill patients aged 65 or ol der who have been admitted to the internal medicine department or visit ed outpatient clinics at a general hospital in Seoul, and the data of t he final 112 people were analyzed. SPSS WIN 26.0 program was used for f requency and proportion, mean, standard deviation, t-test, decentraliza tion, Scheffe test, Pearson correlation coefficients, and Multiple lin ear regression. The results of this study are as follows. 1) In this study, Health Literacy averaged 6.02±2.40 points out of 8 p oints. Digital Literacy averaged 23.63±9.11 points out of 40 points ad the average on the 5-point scale was 2.95±0.47 points. Health Pro moting Behavior averaged 74.28±11.85 points out of 100 points, and t he average on the 4-point scale was 2.95±0.47 points. 2) Health Literacy was statistically significant differences according to age(F=16.64, p<.001), marital status(t=3.16, p=.003), living toget her(t=-2.53, p=.022), religion(t=2.98, p=.004), education level (F=1 0.36, p=.002), subjective health status(F=6.88, p=.002), health inter est(F=2.72, p=.009), internet usage(t=4.98, p<.001), Internet usage t ime(F=6.35, p=.006). 3) Digital Literacy was statistically significant differences according to age(F=19.23, p<.001), marital status(t=2.96, p=.004), living toget her(t=-2.51, p=.014), religion (t=3.00, p=.004), education level(F=7. 91, p<.001), subjective health status(F=5.45, p=.006), health interes t(F=2.65, p=.010), internet usage(t=7.63, p<.001), internet usage tim e(F=14.66, p<.001), the number of medication (t=2.52, p=.013). 4) Health Promoting Behavior was statistically significant differences according to age(F=4.07 p=.020),education level(F=3.78, p=.013), subj ective health status(F=5.79, p=.004), sensory impairment(t=2.17, p=.0 32), health interest(=3.65, p=.001), internet usage((t=3.30, p=.001), Internet usage time(F=15.27, p<.001). 5) Digital Literacy and Health Literacy (r=.551, p<.001), Health Promot ing Behavior and Health Literacy (r=.527, p<.001), Health Promoting B ehavior and Digital Literacy (r=.522, p<.001) showed a statically sig nificant positive correlation. 6) Health Literacy(β=.320, p=.005), More than 3 hours of internet usag e a week(β=.248, p=.017) and Digital Literacy(β=.225, p=.043) were significant variables influencing Health Promoting Behavior(F=6.83, p <.000). And it shows 40.6% of the explanatory power. As a result of this study, it was found that Health Literacy and Digit al Literacy of elderly with chronic diseases had an effect on Health Pr omoting Behavior. Therefore, for the health and well-being of the elder ly population with chronic diseases, it is necessary to develop interve ntion programs and nursing approach to improve the elderly population's ability to understand health information and improve their ability to u se it on the Internet.
