dc.description.abstract국문요지 본 연구는 요양병원 간호사의 죽음 인식과 임종 간호 스트레스가 임종 간호 수행에 미치는 영향을 파악하는 서술적 조사연구이다. 자료 수집은 경기도에 소재한 150병상 이상 규모의 노인 요양병원에 근무하는 간호사 179명을 대상으로 2021년 7월 12일부터 2021년 8월 13일까지 자가 보고식 설문지를 이용하였고 최종 160부를 분석하였다. 연구도구로 죽음 인식은 우리나라의 문화가 반영된 이누미야 요시유키(2002)가 개발한 사생관 척도, 이연옥(2004)이 개발한 임종 간호 스트레스 측정도구, 박순주(1996)가 개발한 임종 간호 수행 도구를 사용하였다. 수집된 자료는 SPSS WIN 25.0 통계프로그램을 이용하여 빈도와 백분율, 평균과 표준편차, 독립표본 t-검정, 피어슨 상관관계, 다중회귀분석으로 분석하였다. 본 연구의 결과는 다음과 같다. 1. 대상자의 죽음 인식은 7점 척도에 평균 4.59±0.40점으로 하위영역별로는 죽음의 긍정적 의미는 4.89±0.77점, 죽음의 부정적 의미는 5.06±0.69점, 죽음 불안은 3.88±1.15점, 죽음관여도 3.38±0.97점, 생명존중의지는 4.66±0.81점이고, 임종 간호 스트레스는 5점 척도에 평균 3.74±0.46점으로 하위영역별로는 전문지식과 기술부족 3.91±0.66점, 의료한계에 대한 갈등 3.93±0.59, 업무량 과중 3.09±0.50점, 임종 환자에 대한 시간할애 어려움 3.73±0.57점, 임종 간호에 대한 부담감 3.24±0.78점, 임종 환자와의 인간적 갈등 3.83±0.55점, 환자 및 보호자의 임종에 대한 부정적 태도 3.86±0.61점이고, 임종 간호 수행의 평균은 2.36±0.45점으로 하위영역별로는 신체적 영역 2.67±0.55점, 심리적 영역 2.80±0.53점, 영적 영역 1.61±0.64점이었다. 2. 대상자의 일반적 특성에 따른 임종 간호 수행의 차이는 성별(t=-2.08, p=.039)로 여성 대상자가 남성 대상자보다 유의하게 높았다. 3. 임종간호 스트레스는 죽음 인식과 임종 간호 수행과 유의한 상관관계가 나타나지 않았고, 임종 간호 수행과 죽음인식에 죽음의 긍정적 의미(r=.274, p<.001)에서 유의한 양의 상관관계를 보였고 죽음 불안(r=-.160, p=.044)에서 유의한 음의 상관관계를 보였다. 4. 대상자의 임종 간호 수행에 영향을 미치는 요인은 총 임상경력(β=.19, p=.017)과 죽음의 긍정적 의미(β=.22, p=.004)로 확인되었다. 변수에 대한 임종 간호 수행의 설명력은 13.8%로 나타났다. 이상의 연구 결과로 보면 임종 간호 수행에 영향을 미치는 요인이 죽음 인식에 죽음의 긍정적 의미이고, 임종 간호 수행과 죽음 불안은 음의 상관관계이었다. 회복이 어려운 노인환자 대상의 임종 간호를 수행하는 요양병원 간호사의 질적인 임종 간호 수행을 위해 긍정적 죽음 인식 향상과 죽음 불안 조절 능력 향상을 위한 요양병원 간호사 대상의 전문적이고 체계적인 임종 간호 교육프로그램이 필요할 것이다. |Abstract The Influence of Death Perception and Terminal Care Stress on the Terminal Care Performance of Nurses Working in Long-term Care Hospitals. Kim Ja Young Graduate School of Clinical Nursing Hanyang University Directed by Professor Lee Hanyi This study is a descriptive research study to understand the effects of death perception and terminal care stress on terminal care performance of long-term care hospital nurses. For data collection, a self-report questionnaire was used from July 12, 2021 to August 13, 2021 for 179 nurses working at an elderly nursing hospital with 150 beds or more in Gyeonggi-do, and the final 160 copies were analyzed. As a research tool, death perception was conducted using the death view scale developed by Inumiya Yoshiyuki (2002) reflecting Korean culture, the terminal care stress measurement tool developed by Lee Yeon-ok (2004), and the terminal care performance tool developed by Park Soon-ju (1996). The collected data were analyzed by frequency and percentage, mean and standard deviation, independent sample t-test, Pearson correlation, and multiple regression analysis using SPSS WIN 25.0 statistical program. The results of this study are as follows. 1. The subject's death perception was an average of 4.59±0.40 on a 7-point scale, and by sub-domain, positive of death 4.89±0.77, negative of death 5.06±0.69, death anxiety 3.88±1.15, death concern 3.38±0.97 points, will to respect life 4.66±0.81 points, and terminal care stress averaged 3.74±0.46 points on a 5-point scale, and by sub-area, expert knowledge and skills 3.91±0.66 points, conflict of medical care limits 3.93±0.59, overload 3.09±0.50 points, spend much time for patients 3.73±0.57 points, burden of terminal care 3.24±0.78 points, human conflict with patients 3.83±0.55 points, negative attitude towards the terminal of patients and guardians 3.86±0.61 points, and the average of terminal care performance was 2.36±0.45 points, and by sub-domain, it was 2.67±0.55 points in the physical area, 2.80±0.53 points in the psychological area, and 1.61±0.64 points in the spiritual area. 2. The difference in terminal care performance according to the general characteristics of the subjects was significantly higher in the female subjects than in the male subjects by gender (t=-2.08, p=.039). 3. Terminal care stress did not show a significant correlation between death perception and terminal care performance, and there was a significant positive correlation between terminal care performance and death perception in the positive of death (r=.274, p<.001). There was a significant negative correlation in death anxiety (r=-.160, p=.044). 4. Factors affecting the terminal care performance of subjects were total clinical history (β=.19, p=.017) and positive of death (β=.22, p=.004). The explanatory power of terminal care performance for variables was 13.8%. As a result of the above study, factors affecting terminal care performance had a positive of death on death perception, and terminal care performance and death anxiety had a negative correlation. A professional and systematic terminal care nursing education program for Long-term care hospital nurses will be needed to improve positive death perception and improve ability to control death anxiety for qualitative terminal care performance of nursing hospital nurses who terminal care performance for elderly patients who cannot recover.-
