- 비만은 에너지의 불균형에 의한 단순한 생리적 현상이 아니라 생체 신진대사, 내분비 교란, 유전자 발현 및 후생학적 요인, 장내 세균의 변화 등과 같은 다양한 병인으로 유발되며, 고혈압, 심근경색증, 당뇨병과 같은 질환을 야기시킨다. 또한 비만에 의해 지방조직에서 염증 반응과 특정 면역세포들이 증가하면서 만성적 면역반응이 일어나고 다양한 자가 항체를 유도 생산한다. 선행 연구를 통해 비만 마우스에서 아포지단백-B100을 인식하는 자가 항체가 존재함을 확인하였다.
본 연구에서는 마우스와 인간에서 아포지단백-B100을 인식하는 자가 항체의 특성 분석 및 아포지단백-B100 항체 유도에 의한 치료 효과를 확인하였다. 먼저, 비만 마우스에서 아포지단백-B100을 인식하는 자가 항체가 유도 생성되어 혈중 내 농도가 증가한 후 고도 비만으로 이행될 때 감소하는 것을 확인하였으며, 인간에서도 비만이 진행됨에 따라 자가 항체의 농도가 증가한 후 고도 비만으로 이행될 때 감소하는 2상 패턴의 양상을 확인하였다. 이는 과도한 지방 흡수로 체내 축적된 지방 입자를 제거하기 위한 자연 치유적 메커니즘으로 아포지단백-B100을 인식하는 자가 항체가 생성되고 소모되는 것을 의미한다. 또한 이들 자가 항체는 IgG 형태로 특이적으로 아포지단백-B100의 p210 부분을 인식하는 것을 확인하였다. 동맥경화 환자에서 다양한 에피토프를 가지는 자가 항체가 생성되는 것과 다르게 비만 환자에서 특이적으로 p210을 인식하는 자가 항체만 생성되는 것은 선행적으로 체내에서 (초)저밀도 지단백질을 제거하기 위해 아포지단백-B100-p210에 대한 자가 항체가 생성된 후 고도 비만으로 악화되면서 추가적으로 체내에 축적된 자연적/변형된 (초)저밀도 지단백질에 의해 다양한 에피토프를 가지는 자가 항체가 생성되는 것을 의미한다. 이를 이용하여 3HOTP 접종을 통해 인위적으로 아포지단백-B100을 인식하는 항체 생성을 유도하였을 경우 3HOTP 접종에 의해 효과적으로 체중 억제가 되는 것을 확인하였으며, 혈중 항체 농도에 따라 접종 주기를 조절하여 지속적인 접종에 의해 비만을 효과적으로 차단하는 것을 확인하였다.
결론적으로 비만이 유도되면 체내에서 치료를 위해 특이적으로 아포지단백-B100의 p210 부분을 인식하는 자가 항체가 생성되어 증가하지만 고도 비만에서 더 이상 증가하지 않으며, 3HOTP의 접종은 생체 내 면역체계를 자극하여 아포지단백-B100을 인식하는 항체 생성을 인위적으로 유도 증가시켜 체내 지방의 축적을 차단하여 체중 증가를 효과적으로 억제하며, 이는 새로운 기전의 백신 형태의 비만 치료의 가능성이 있음을 보여준다.
|Obesity is caused by various etiologies such as changes in metabolism,
endocrine disturbances, gene expression and epigenetic factors, and
changes in the intestinal microbiome, and in turn causes pathologies such
as high blood pressure, myocardial infarction, and diabetes. Of potential
importance, as obesity is induced, inflammatory reactions and specific
immune cells are increased in the adipose tissue, resulting in a chronic
immune response and the production of various autoantibodies. Notably,
our laboratory found in previous studies that some of these autoantibodies
recognize Apolipoprotein B-100 (ApoB100), the primary organizing
protein of the lipid-carrying particles of the blood, in obese mice.
In the present Ph. D. Thesis, I studied the characteristics of
autoantibodies that recognize ApoB100 in obese mice and humans, and
then I investigated the potential therapeutic effects in mice by inducing
Anti-ApoB100 antibodies via the injection of a synthetic peptide mimotope
named 3HOTP (3H Obesity Immuno-Therapeutic Peptide). First, I
confirmed that autoantibodies recognizing ApoB100 were induced in mice
that received a high-fat diet. Remarkably, the autoantibody titers initially
increased, but then decreased in the transition to severe obesity. Similarly,
in humans, the concentration of autoantibodies increased as obesity
progressed and then decreased when transitioning to severe obesity. This
suggests that autoantibodies recognizing ApoB100 are produced and
become involved in a natural therapeutic mechanism to remove fat
particles that accumulated in the body due to excessive fat absorption. I
further found that the ApoB100 autoantibodies are of the IgG isotype and
specifically recognize the p210 epitope of ApoB100. It has been known
that arteriosclerotic patients produce autoantibodies against various
ApoB100 epitopes; my new data now indicate that obese patients
specifically produce autoantibodies that recognize the epitope p210. The
findings suggest a scenario of obesity progression in which first
ApoB100-p210 autoantibodies are generated that remove (V)LDL, and
then autoantibodies against various epitopes are generated by the
continuously accumulating native/modified (V)LDL. Importantly, I found
that when 3HOTP was repeatedly injected to induce anti-ApoB100
antibodies using this autoantibody mechanism, the body weight increment
was inhibited in high fat diet-fed mice, and the effect was maintained over
the injection period and was paralleled by the ApoB100 antibody titers.
These findings confirm the observations we have previously made with a
recombinant vaccine. Further, obesity was effectively blocked by adjusting
the injection cycle according to the antibody concentration in the blood.
In conclusion, I found that (i) both humans and mice show a biphasic,
ApoB100-directed autoantibody response to obesity, suggesting a
physiological role of the immune system in body weight control; (ii)
injection of the peptide 3HOTP induces the production of ApoB100-
specific antibodies, which are most likely responsible for the concomitantly
observed suppression of weight gain. Compared with the previous,
recombinant, version of an anti-obesity vaccine from our laboratory, the
novel 3HOTP, also from our laboratory, presents a safer and more
economical version with at least equal effect. We suggest that 3HOTP has
a significant potential utility in a novel, vaccination-based anti-obesity
treatment.
