간호사의 노인에 대한 지식, 태도, 공감력이 노인간호행위에 미치는 영향

Abstract

This study is a descriptive correlation study to provide basic data on the development and application of nursing practices and education programs for senior citizens by identifying knowledge, attitudes, and empathy of nurses working at superior general hospital. The subjects of the study were nurses working at the upper general hospital in Anyang, and the data was collected for a week from April 13, 2021 after IRB approval. A total of 145 copies were distributed and 141 copies were collected. Of these, 134 copies were used for the final analysis, excluding three that did not meet the selection criteria and four unfaithful responses. The knowledge of the elderly was measured by FAQ1(Factson Agging Quiz1) developed by Palmore(1998) and attitude toward the elderly was measured by tools developed by Sanders (1984) . Empathy toward the elderly was measured by Empathy Construct Rating Scale(ECRS) developed by La Monica (1981) and geriatric nursing practice was measured using tools by developed Choi Hee-ja (2002). The collected data were analyzed using the IBM SPSS 26.0 statistical program was used to analyze frequency and percentage, mean, standard deviation, t-test, ANOVA, Scheffetest, Pearson correlation coefficients, Multiple regression analysis. Research results, The general characteristic that showed a difference the attitude toward the elderly and nursing practice for the elderly was the work department. it was showed that special departments(4.05±0.53) have a more positive attitude toward the elderly than Integrated Nursing Care Service Wards(3.64±0.50) and showed that Integrated Nursing Care Service Wards(3.38±0.39) have higher geriatric nursing practice than general wards and special departments. According to a survey of factors that affect geriatric nursing practice, the higher the knowledge(r=.190, p=.028) and empathy for the elderly (r=.531, p<.028), the better the geriatric nursing practice. The most influential factor in nursing for the elderly was empathy for the elderly(β=.49, p<.001). Therefore, in order to promote geriatric nursing practice, it is necessary to develop and apply education courses and practical programs that can identify nurses' nursing needs to provide knowledge related to nursing for the elderly and enhance empathy for the elderly.| 본 연구는 간호사의 노인에 대한 지식, 태도, 공감력이 노인간호행위에 미치는 영향을 확인하여, 노인간호 실무와 교육프로그램 개발 및 적용에 대한 기초자료를 제공하기 위한 서술적 조사연구이다. 연구 대상은 안양시 소재 H 상급종합병원에 근무 중인 노인 환자를 간호하는 근무경력 1년 이상인 간호사 134명을 대상으로 2021년 4월 13일부터 일주일간 자료를 수집하였다. 노인에 대한 지식은 Palmore(1977)에 의해 처음 개발되었고, 1998년 수정 보완된 True-False version의 FAQ1(Facts on Aging Quiz1)을 사용하였고, 노인에 대한 태도는 Sanders et al(1984)이 개발한 노화의미분별척도를 사용하였다. 노인에 대한 공감력은 La Monica(1981)가 개발한 Empathy Construct Rating Scale (ECRS)를 사용하였으며, 간호사의 노인간호행위 실천 정도는 최희자(2002)가 개발한 노인간호실천 도구를 사용하였다. 자료 분석은 IBM SPSS 26.0 통계 프로그램을 사용하여 빈도와 백분율, 평균, 표준편차, t-test, ANOVA, Scheffe test, Pearson correlation coefficients, Multiple regression analysis로 분석하였다. 연구 결과 간호사의 노인에 대한 태도와 노인간호행위에 유의한 차이를 나타낸 일반적 특성은 근무부서였다. 노인에 대한 태도는 특수부서(4.05±0.53)가 간호⋅간병통합서비스 병동(3.64±0.50)보다 노인에 대한 태도가 긍정적인 것으로 나타났으며(F=7.19, p=.001), 노인간호행위는 간호⋅간병통합서비스 병동(3.38±0.39)이 일반 병동(3.18±0.34) 및 특수부서(3.15±0.33)보다 더 잘하는 것으로 나타났다(F=5,28 p=.006). 간호사의 노인간호행위에 영향요인을 살펴본 결과, 노인에 대한 지식이 높고(r=.19, p=.028), 노인에 대한 공감력이 높을수록(r=.53, p<.001) 노인간호행위를 더 잘하는 것으로 나타났으며, 노인간호행위에 가장 큰 영향을 주는 요인은 노인에 대한 공감력(β=.49, p<.001)으로 나타났다. 따라서 노인간호행위 증진을 위해 간호사들의 노인간호 교육요구도 분야를 파악하여 노인간호 관련 지식을 제공하고, 노인에 대한 공감력을 증진 시킬 수 있는 교육 과정 및 실습프로그램의 개발 및 적용이 필요할 것으로 생각된다.
