분리불안장애 아동의 특성과 부모-자녀 놀이치료 프로그램의 효과

Abstract

The purpose of this study was to analyze the characteristics of children with separation anxiety disorder and was to examine the effects of the program based on Filial Therapy. The subjects of the study 1 were the 33 SAD(separation anxiety disorder) children and 33 normal children. The subjects of the study 2 were the 8 experimental group and 8 comparison group. The results of the study 1 were as follow: 1. The SAD children showed insecure-avoid attachment style than the normal children. 2. The SAD children showed a lower level of an adaptability and a physical rule than the normal children. 3. The mothers of the SAD children showed a higher level of depression and separation anxiety than the mothers of the normal children. 4. The mothers of the SAD children showed a lower level of an affection than the mothers of the normal children. The mothers of the SAD children showed a higher level of overprotection and noninterference than the mothers of the normal children. 5. The parents of the SAD children showed a higher couple conflict than the parents of the normal children. 6. 1. The SAD children experienced a current trauma than the normal children. The results of the study 2 were as follow: 1. After Filial therapy program, The level of withdrawn, somatic complaints, anxious/depressed, and social problem of the experimental group children decreased. 2. After Filial therapy program, The level of empathy ability of the experimental group children increased. 3. After Filial therapy program, The level of parental stress of the experimental group mothers decreased. 4. After Filial therapy program, The level of depression and separation anxiety of the experimental group mothers decreased. 5. After Filial therapy program, The level of PLATO of the experimental group mothers increased. 6. The result of the qualitative analysis was as result : The level of children's problem behavior, mother's empathy ability, parenting stress, mother's depression and separation anxiety reported the positive change. 7. The result of home play session showed to omit a primary level and to represent a negative emotion.; 이 연구는 크게 2가지로 나눠서 연구되었으며 연구 1의 목적은 분리불안장애 아동의 특성을 알아보고자하는 것이다. 연구 2의 목적은 분리불안장애 아동을 대상으로 부모-자녀 놀이치료 프로그램을 실시한 후 프로그램의 효과를 검증하는 것이다. 연구 1을 위해, 분리불안아동 33명과 일반아동 33명을 대상으로 아동의 애착유형, 기질, 어머니의 우울, 어머니의 분리불안, 어머니의 양육태도, 부부갈등 그리고 최근의 스트레스 원에 대해 비교 분석하였다. 연구 2를 위해, DISC 진단 준거에 부합되며 프로그램에 참여의사를 밝힌 분리불안장애 아동의 어머니를 대상으로 실험집단을 구성하였고, 참여의사가 있으나 현실적인 여건으로 프로그램 참여가 어려운 분리불안장애 아동의 어머니를 대상으로 통제집단을 구성하였다. 각 집단은 최종 8명이 분석에 이용되었다. 프로그램은 10회가 실시되었고, 프로그램의 효과를 검증하기 위해 사전, 프로그램 종결 후 사후, 12주 후 추후 검사를 실시하였다. 연구 1의 연구결과는 다음과 같다. 첫째, 분리불안장애 아동은 일반 아동보다 불안정-회피 애착 유형이 많았다. 둘째, 분리불안장애 아동은 일반 아동보다 기질적으로 적응도가 낮으며 생리적 주기가 덜 규칙적인 것으로 나타났다. 셋째, 분리불안장애 아동의 어머니는 일반 아동의 어머니보다 우울이나 분리불안 수준이 높았다. 넷째, 분리불안장애 아동의 어머니는 일반 아동의 어머니보다 덜 온정적인 양육태도를 보이며, 과보호를 하거나 방임적인 양육태도를 더 보이는 것으로 나타났다. 다섯째, 분리불안장애 아동의 부모가 일반 아동의 부모보다 부부갈등이 더 높은 것으로 나타났다. 여섯째, 분리불안장애 아동은 스트레스를 경험한 후에 분리불안을 보였으며 스트레스 내용은 아동 사고, 어머니의 임신 및 출산, 기관입학, 이사 및 전학, 부부갈등, 어머니의 입원 등으로 나타났다. 연구 2의 연구결과는 다음과 같다. 첫째, 부모-자녀 놀이치료 프로그램 실시 후 실험집단 아동의 위축, 신체증상, 우울/불안과 사회적 미성숙의 문제행동을 감소시켰으며 추후에도 그 효과는 지속적이었다. 둘째, 실험집단 아동 어머니의 공감능력이 증가하였고 추후에도 그 효과는 지속적이었다. 셋째, 실험집단 아동 어머니의 양육스트레스에 변화가 나타났는데 어머니의 양육스트레스 점수가 낮아졌고, 추후에는 더 감소하였다. 넷째, 부모-자녀 놀이치료 프로그램 실시 후 실험집단 아동의 어머니의 우울과 분리불안의 심리적 문제도 감소되었으며 추후에는 더 감소하였다. 다섯째, 보조치료자로써의 부모 수행 수준도 유의하게 증가하였다. 여섯째, 대부분의 어머니들이 아동의 문제행동, 어머니의 공감능력, 어머니의 양육스트레스에서 변화 정도에 따른 개개인의 차이는 있으나 모두 긍정적 변화를 보고하였다. 어머니의 우울의 변화는 사례 3, 사례 6, 사례 8의 어머니가 보고하였고 어머니의 분리불안은 사례 1, 사례 2, 사례 4, 사례 8의 어머니가 보고하였으며 자신의 심리적 어려움에 대한 인식과 조금씩의 변화를 보고하였다. 이러한 어머니의 보고는 이 연구의 양적분석을 지지하는 결과로 볼 수 있다. 일곱째, 가정놀이세션의 놀이를 분석한 결과, 실험집단 아동이 공통적으로 놀이과정의 변화된 모습을 보였다.
