소화기계 암환자 배우자의 부담감 관련요인

Abstract

본 연구는 소화기계 암환자와 환자의 간호제공자인 배우자의 부담감을 감소시킬 수 있는 방안을 모색하기 위하여, 소화기계 암환자와 배우자의 관계의 질, 사회적 지지가 배우자가 경험하는 돌봄으로 인한 부담감에 어떤 영향을 미치는지 밝힘으로써 소화기계 암환자 배우자의 돌봄 행위를 증진시킬 수 있는 간호전략을 개발하기 위한 기초 자료를 제공하기 위해 시도되었다. 연구대상은 서울시내 1개 대학병원의 일반외과 병동과 외래에 소화기계 암으로 진단 받고 치료중인 환자 배우자 76명이었다. 연구 도구는 서혜미와 오가실(1993)이 개발한 부담감 측정도구를 사용하였으며, 관계의 질 측정도구는 Archbold 와 Stewart(1986)의 상호성(15문항) 도구를 양영희(1992)가 번역 · 보완하여 사용한 도구를 사용하였고, Brandt 와 Weinert(1985)가 개발한 Personal Resource Questionnaire(P. R. Q) Part Ⅱ 를 성미혜(2000) 가 번역한 도구를 수정 · 보완한 사회적 지지 측정도구를 사용하였다. 자료수집은 2006년 9월부터 11월 28일 까지 이며, 자료수집 방법은 본 연구에 참여를 수락한 대상자에게 설문지를 배부하여 직접 기록하게 한 후 회수하였으며, 수집된 자료의 분석은 SPSS 12.0을 이용하여 전산 처리하였고, 대상자의 일반적 특성은 실수와 백분율, 평균을 이용하여 분석하였고, 변수와의 상관관계는 Pearson's correlation coefficient를, 일반적 특성에 따른 부담감, 관계의 질, 사회적 지지는 independent t-test 와 ANOVA 를 이용하여 분석하였다. 연구의 결과는 다음과 같다. 1. 소화기계 암환자 배우자의 부담감 정도는 125점 중에 평균 74.8(평점 2.99 점±0.55) 점 이었으며, 관계의 질은 65점 중 평균 43.6(평점 2.91±0.24)점 이고, 사회적 지지는 65점 중 49.1(평점 3.27±0.78)점 이었다. 2. 소화기계 암환자 배우자의 관계의 질, 사회적 지지와 부담감과의 관계에서 관계의 질이 높을수록 부담감이 낮은 역상관 관계(r=-.232, p<0.05), 사회적지지가 낮을수록 부담감이 높은 역상관 관계(r=-.290, p<0.05)를 보였으며, 관계의 질이 높을수록 사회적 지지가 높은 순상관관계(r=.540(p<0.01)를 나타냈다. 3. 부담감 정도는 환자의 진단명 (F=3.824, p<0.05), 돌봄기간 (F=2.889, p=.042) 배우자의 하루 중 돌봄시간(F=4.659, p<0.01)에서 통계적으로 유의한 차이를 나타냈다. 그러나 관계의 질과 사회적 지지 모두에서는 통계적으로 유의한 차이가 없었다. 이상의 결과를 종합하면 소화기계 암환자 배우자가 느끼는 부담감은 비교적 높은 것으로 나타났으며, 사회적 지지가 높을수록, 관계의 질이 높을수록 부담감이 낮았다. 그러므로 배우자의 사회적 지지와 관계의 질을 높일 수 있는 간호제공자의 역할을 분담할 수 있는 효율적인 전략과 이를 위한 간호 중재 개발이 필요할 것이다.; The purpose of this study is to describe the perceived burden of the gastrointestinal cancer patient's spouses and related factors to analyze relationship between perceived burden and quality of relationship and social supports. The data were collected from 4th of September to 28th of November 2006. The convenient sampling was done from the 76 spouses who take care for a gastrointestinal cancer inpatients and outpatients at H hospital in Seoul. The spouses's perceived burden tool of O, Ga-Sil, quality of relationship tool of Yang, Young-Hee and social support measurement tool of Sung, Mi-Hye which adjusted and complimented PRG-part II developed by Brandt and Weinert were used as research tool. Collected data were analyzed using SPSS win 12.0. The general characteristics of the subjects were analyzed by frequency, percentage and mean. Correlation with variable was analyzed using Pearson's correlation coefficient and the relation of the spouse's burden, quality of relationship and social supports according to general characteristics were analyzed using independent t-test and ANOVA. The result of this study are as follow. 1. The spouse's burden, quality of relationship and social support for gastrointestinal patient's spouses were 2.99(±0.55), 2.91(±0.24) and 3.27(±0.78) each. 2. Concerning to the relation of spouses's burden, quality of relationship and social support, quality of relationship was low with high burden(r=-.232), social support was high with low burden(r=-.290). Also, quality of relationship was high with high social support(r=.540). 3. Spouse's burden were considered according to general characteristics. The level of burden differed according to patient's diagnosis, during of caregiving(month) and time of caregiving. However, the level of quality of relationship and social support did not differ according to general characteristics. In conclusion, burden of gastrointestinal cancer patient's spouses was high score. Nevertheless, the burden was low with high quality of relationship and high social support. For the that reasons, efficiency nursing intervention need to make high quality of relationship and high social support for lower burden of the spouse.
