3차원 디지털 모형을 이용한 성인 골격성 Ⅲ급 부정교합자의 전후방 부조화와 치열궁 폭경의 상관성에 관한 연구

Abstract

성인 골격성 Ⅲ급 부정교합자의 안정적인 치료 결과를 위해서는 상하악 치열궁 폭경에 대한 면밀한 평가와 함께 횡적 부조화를 파악하는 것이 중요하다. 본 연구는 29명(남자 14명,여자 15명)의 성인 골격성 Ⅲ급 부정교합자를 대상으로 “Wits” appraisal 계측과 3차원 디지털 모형을 이용한 치열궁 측정을 통하여 전후방적 부조화와 치열궁 폭경의 상관성을 조사하여 다음과 같은 결론을 얻었다. 1. 상악 제1대구치 부위에서 “Wits” appraisal이 감소함에 따라 기저골에 대해 치아가 더 협측으로 경사되는 뚜렷한 음적 선형 관계를 나타내었으며(γ=-0.464), 통계학적 유의성을 보였다(p<0.05). 2. 상악 견치 부위(γ=-0.256)와 상악 제2대구치부위(γ=-0.180)에서는 “Wits” appraisal이 감소함에 따라 기저골에 대해 치아가 더 협측으로 경사되는 약한 음적 선형관계가 나타났으나 통계학적 유의성은 없었다.(p＞0.05). 3. 하악 견치(γ=+0.196), 하악 소구치(γ=+0.270), 하악 제2대구치 부위 (γ=-0.277)에서는 “Wits” appraisal이 감소함에 따라 치아가 설측 경사 되는 약한 양적 선형관계가 나타났지만 통계학적 유의성은 없었다. (p＞0.05). 4. 하악 제1대구치 부위의 폭경 비율의 변화는 거의 무시될 수 있는 선형관계(γ=-0.091)로서 모든 치아 부위 중 가장 낮은 상관성을 보였다.; This study was conducted to evaluation of correlation between anterior-posterior discrepancy (“Wits” appraisal ) and arch width of adult skeletal Class Ⅲ malocclusion using a three-dimensional digital model. The sample consisted of 29 adult patient with skeletal Class Ⅲ malocclusion. The data were evaluated by Spearman's rank order correation. The result were as follows: 1. There were significant difference and distinct negative correlation in Mx. 1st molar area. 2. There were no difference and reluctant negative correlation in Mx. canine, Mx. 2nd molar area. 3. There were no difference and reluctant positive correlation in Mn. canine, Mn. premolar, Mn. 2nd molar area. 4. There were no difference and negligible correlation in Mx. premolar, Mn. 1st molar area.
