노인 입원환자가 인식하는 간호 서비스에 대한 간호 요구도와 만족도

Abstract

본 연구에서는 종합병원에 입원하고 있는 60세 이상 노인환자들의 특성에 따라 간호사가 제공하고 있는 서비스에 대해 노인환자가 바라는 간호요구가 무엇이고, 만족감이 어느 정도인지를 파악하며 노인환자의 간호요구와 간호만족 사이에 어느 정도의 상호관련성이 있는지에 대해 알아보고자 시도 하였다. 서울시내 소재한 일개 종합병원 신경과의 입원환자 94명을 대상으로 2006년 9월 1일 ~ 2007년 2월 30일까지 설문지를 통하여 자료수집 하였다. La-Monica 등이 개발한 Patient Satisfaction Scale(1986)을 양은진(2002)이 수정?보완한 도구를 36문항으로 재구성한 설문지를 이용하여 자료를 수집하였다. 자료분석은 SPSS 프로그램을 사용하였으며 빈도와 백분율, 평균, 표준편차, Pearson correlation coefficient, t-test, ANOVA, Scheffe의 사후검정, stepwise multiple regression analysis를 적용하였다. 연구의 결과는 다음과 같다. 1) 노인 입원환자의 간호 요구도 총 5점 만점에 평균 4.04이었다. 각 영역별 간호 요구도는 교육적 간호, 사회정서적 간호, 치료적 간호, 신체적 간호 순으로 나타났다. 노인환자의 일반적 특성에 따른 노인환자의 간호 요구도는 성별, 종교, 결혼상태, 월평균소득, 직업, 건강상태, 다른 건강 문제, 주 돌봄 제공자, 간병빈도가 유의한 차이를 보여주었다. 2) 노인 입원환자의 간호 만족도는 총 5점 만점에 평균 3.67이었다. 영역별 간호 만족도는 사회정서적 간호, 치료적 간호, 신체적 간호, 교육적 간호 순으로 나타났다. 노인환자의 일반적 특성에 따른 간호 만족도는 성별, 종교, 월평균 소득, 직업, 건강상태, 다른 건강문제, 주 돌봄 제공자, 간병빈도가 유의한 차이를 보여주었다. 3) 전체 간호 요구도와 전체 만족도 사이는 순상관관계를 보였으며 간호서비스에 대한 요구도가 높을수록 간호서비스의 만족도도 높게 지각하고 있는 것으로 나타났으며 이는 통계적으로 유의하였다 (r=.369, p=.000). 영역별 간호요구도 간의 상관관계 중 교육적 간호 요구도는 치료적 간호 요구도와 강한 순상관관계(r=.797, p=.000)를 나타냈다. 영역별 간호만족도 간의 상관관계 중 사회정서적 간호 만족도는 치료적 간호만족도와 강한 순상관관계(r=.793, p=.000)를 보였다. 영역별 사이의 간호 요구도와 만족도의 상관관계에서는 치료적 간호 요구도와 치료적 간호 만족도가 가장 높은 유의한 중등도의 순상관관계(r=.541, p=.000)를 나타냈다. 4) 간호 요구도와 만족도에 영향을 미치는 요인을 분석한 결과, 전체 간호 요구도에서는 성별, 종교, 월평균소득, 다른 건강문제 유무의 변수가 39.6% 영향력을 나타냈으며 하부영역에서는 성, 입원횟수, 다른 건강문제 유무의 변수가 신체적 간호 요구도에 45.9%의 가장 큰 영향력을 나타냈다. 전체 간호 만족도에 있어서는 성별, 입원횟수, 재원일수의 변수가 31.8%의 영향력을 나타냈으며 하부영역에서는 성, 종교, 입원횟수, 월평균 소득, 재원일수의 변수가 치료적 간호 만족도에 있어서 51.5%의 가장 큰 영향력을 나타냈다. 간호서비스의 질은 환자만족도에 가장 깊게 영향을 미치며 병원에 대한 선호도 증가와 함께 좋은 이미지 형성으로 병원을 다시 찾게 하는데 영향을 미치므로, 간호사들은 요구도에 대한 인식과 함께 만족도가 낮은 영역에 보다 더 관심을 가져야 할 것으로 본다. 또한 노인환자의 간호 요구도와 만족도는 노인의 일반적인 특성과 건강관련 특성에 영향을 받으므로 특히 성별, 다른 건강문제 유무, 주 돌봄 제공자, 주 돌봄 제공자와 보내는 시간에 유의하게 나타났으며 이를 고려한 간호서비스 개발이 요구된다.; The purpose of this study was to examine the level of the elderly inpatients’nursing needs and to analyze the relationship between two in order to provide some basic data useful to development of quality nursing services for the elderly inpatients. Ninety four elderly patients aged over sixty hospitalized at A medical center in Seoul were interviewed with questionnaire directly by the researcher from 1st September 2006 to 30th February 2007. Research instrument, Patients Satisfaction Scale developed by Yang Eun Jin(2002) and reconstructed to 36 questions by this researcher was used. Data collected by the questionnaire were analyzed by SPSS WIN 12.0 program. The results of this research are as follows: 1. The percentage of male inpatients was 58.5%, female was of 41.5% with mean age of 68.06 years old. 84% of participants had other disease such as hypertension, diabetes mellitus and so on and 45.7% of main care giver is their husband or wife. 2. Mean score of nursing needs was 4.04 out of 5. The score of educational nursing needs was the highest with 4.31 followed by socio-affective 4.30, therapeutic 4.25, physical 4.04 in rank. 3. Mean score of nursing satisfaction was 3.67 out of 5. The score of socio-affective was the highest with 3.95 followed by therapeutic 3.94, physical 3.74, and educational 3.63 in rank. 4. The universal factors giving statistically significant difference in their nursing needs and nursing satisfaction, totally and categorically were only sex among 8 demographic factors; and the other health problems, main lay care giver, and the time of tending the patient among 5 health related factors. 5. There was a positive correlation between total nursing needs and satisfaction(r=.369, p=.000). The strongest relationship was shown between therapeutic needs and satisfaction(r=.541, p=.000). 6. The influencing factors on nursing needs totally were sex, religion, monthly income and the other health problems. The total variance was 39.6%(adjusted R2 =.396). The influencing factors on nursing satisfaction totally were sex, the frequency of hospitalization, length of hospitalization. The total variance was 31.8%(adjusted R2 =.318). This results show the relationship between nursing needs and satisfactions, and the importance of the sex, female or male, as the very factor in making a difference elderly inpatients’ nursing needs and satisfaction. For the developing the nursing service for the elderly, besides, the present illness, other health problems originally they have had, main lay care giver, and the time of tending the patient should be considered.
