DVD를 이용한 수술 전 폐암환자교육이 수술 후 자가간호지식과 수행율에 미치는 효과

Abstract

통계청(2005)에 따르면 2005년 암은 사망원인 1위를 차지했으며 그 중 폐암은 2000년에는 위암(24.3%)보다도 높은 사망률(24.4%)을 보임으로써 국내암 사망률 1위를 차지하고 있다. 폐암의 증상은 암의 위치에 따라 다르게 나타나지만, 대부분의 경우 기침, 객혈, 폐렴이나, 가슴통증, 객담, 호흡곤란 등이 나타난다. 폐암의 치료방법으로는 수술, 항암 화학요법, 방사선 요법이 있다. 수술방법으로는 전폐절제술, 폐엽절제술, 쐐기절제술이 있으며, 폐엽을 잘라내면서 같이 임파절도 잘라내며 폐암의 위치와 크기에 따라 수술방법은 달라진다(강민종 외, 1996). 수술 후 부동은 혈류를 느리게 하고, 혈전증을 유발하므로 수술 후 가능한 빠른 조기이상과 체위변경을 하도록 권장해야 한다. 조기이상에 대하여 수술 전에 미리 여유를 두고 가르쳐주어 연습하도록 하는 것이 궁극적으로 빠른 회복과 재발방지 및 합병증 예방에 도움이 된다(전시자 외, 1996). 폐암수술환자가 증가하는 것에 비해 체계적 교육프로그램은 개발되어 있지 않고 그에 대한 연구도 미비한 실정이다. 따라서 본 연구는 폐암수술을 한 환자의 교육프로그램을 개발하고, 교육프로그램의 적용을 통한 환자들의 자가 간호 지식을 증가시킴으로써 그 수행율을 높이고 궁극적으로는 환자들의 빠른 회복과 합병증 예방에 실질적 도움을 주기위한 간호중재를 개발하기 위함이다. 연구 대상자는 2007년 3월1일부터 3월31일까지 경기도 N병원에 입원해서 폐암수술을 받은 22명이며, 수집된 자료는 SPSS 12.0을 이용하여, 백분율, 실수, 평균평점, paired t-test, Pearson's Correlation으로 분석하였다. 본 연구에서 사용된 도구로 자가 간호지식 측정도구는 이혜영(2000)의 도구를 연구자가 수정 보완한 16문항을, 자가 간호수행 측정도구는 이현숙(1994)의 도구를 연구자가 수정 보완한 15문항을 사용하였고, 신뢰도는 자가 간호지식 측정도구는 Chronbach' a = 0.663이고, 자가 간호 수행 측정 도구는 Chronbach' a = 0.864이다. 1. DVD 교육프로그램 시행 전 자가 간호지식의 평균은 21.36±3.82 점이었 고, DVD 교육 프로그램 시행 후의 자가 간호 지식의 평균은 29.18±2.17 점이며, 수술 전 DVD 교육 프로그램 시행 전 후의 자가 간호 지식의 점수 차이는 7.81±3.54 점으로 수술 전 DVD를 이용한 교육은 자가 간호 지식을 증가시킨다는 가설은 유의하다(p=0.00(p<0.05)). 2. 수술 전 DVD를 이용한 교육 후 자가 간호 지식의 증가는 자가 간호수행 율에 영향을 준다는 가설은 다소 높은 상관관계가 있는 것으로 나타났 다(r=0.445, p=0.038). DVD 교육 프로그램 시행 전 후의 자가 간호 지식의 증가는 자가 간호 수행율에 영향을 준다. 궁극적으로는 합병증예방, 건강증 진, 빠른 회복 등 환자의 수술 후 회복에 영향을 준다 할 수 있다. 이상의 결과를 종합해 볼 때 DVD를 통한 수술 전 교육은 환자에게 자가 간호 지식을 증가시켜주며, 지식의 증가는 환자의 수술 후 자가 간호 수행율의 증가와 관계가 있음이 확인되었다. 본 연구를 통해 DVD를 폐암수술환자교육에 적용함으로서 효과를 검증할 수 있는 기회가 되었으며, 다른 수술환자를 대상으로 한 효과검증에 관한 반복연구가 필요하며, 유용한 간호중재가 될 수 있으리라 사료된다.; Cancer is highest mortality in 2005 by Korea National Statistical Office. Lung cancer's symptom is cough, hemoptysis, chest pain, sputum, dyspnea etc. Management of cancer is Radiation therapy, Chemotherapy, surgery(Lobectomy, wedge resection, pneumonectomy) We must encouraged ambulation because of immobility is made embolism. Education about ambulation is helped recovery and complication prevention. But, education program is deficiency. This study is education program development that use DVD and effect of program apply to lung cancer patient. And relationship between self care knowledge and performance. Data was collected from March 1 to March 31, 2007. And among the 25 in-patients who has lung cancer in general hospital which has is located in K city. This study was used the self care knowledge instrument modified by Lee Hye Young(2000), self care performance instrument modified by Lee Hyun Sook(1994). This tool were remodified and complemented to fit to this study. Data analysis was used SPSS 12.0 program for percentage, mean, average score, Pearson's correlation coefficient, t-test. The results of this study are as follows; 1. The pre self care knowledge was average score 21.36±3.82 , post self care knowledge was average score 29.18±2.17. Pre and post average difference score is 7.81±3.54. DVD program was significantly different by self care knowledge(p=0.00(p<0.05)). 2. Self care performance were significantly different by Self care knowledge increasing(r=0.445, p=0.038). Ultimately DVD program have an effect on recovery, complication prevention. Consequently, self care knowledge is increased, and self care performance according to self care increasing self care knowledge is increased when DVD program use lung cancer patient. There is a need of repeated research with more patients and development of program on education of lung cancer patient and comparison research among them are also required.
