노인 병원 간호사의 호스피스 간호 교육이수 및 수행 현황

Abstract

본 연구는 노인병원에서 근무하는 간호사들의 호스피스 간호 교육이수 및 수행정도를 알아봄으로써 노인 호스피스의 중요성을 재인식하고 나아가 노인 호스피스의 질적 향상을 위한 방안을 모색하고, 노인 간호과정에 반영할 기초자료를 제공하기 위해 시도되었다. 본 연구는 서술적 조사 연구로 자료 수집을 위해 서울, 경기도 및 충청지역에 소재한 12곳의 노인 병원에서 근무하는 141명의 간호사를 대상으로 한 설문지를 분석에 활용하였고 수집된 자료는 SPSS for Windows 12.0 통계 프로그램을 이용하여 분석하였다. 본 연구의 결과는 다음과 같다. 노인 병원 간호사들의 호스피스 교육이수 정도를 점수화하여 분석한 결과 평균 1.99점으로 교육정도가 매우 낮은 것으로 나타났으며, 노인 병원 간호사의 호스피스 간호 교육 이수 방법을 보면, 총 설문 자 141명 중 호스피스교육 미 이수자 23명을 제외한 118명 중 학과수업을 통해 호스피스 교육을 받은 간호사(71.1%), 보수교육을 통해 호스피스 교육을 받은 간호사(19.5%), 원내교육을 받은 간호사(9.3%)로 순으로 나타났다. 이수방법에 따른 교육이수 정도를 알아본 결과 원내교육을 받은 대상자의 교육이수 정도는 평균 3.27점으로 매우 높은 수준이었으며, 보수교육을 통해 호스피스 교육을 받은 간호사는 평균 2.30점의 교육이수 정도를 보였고, 학과수업을 통해 호스피스 교육을 접한 간호사는 2.01점이었다. 즉, 호스피스교육 방법과 교육이수 정도간의 차이(F=59.563, p=0.000)는 통계적으로 유의하여 원내교육이 더욱 활성화 되어야 함을 보여준다. 노인 병원 간호사의 호스피스 간호 수행정도는 4점 만점에 평균 3.06점으로 높은 편이었다. 영역 별 간호 수행에 있어서는 신체적 간호가 평균 3.50점으로 매우 높은 수준이었으며, 정서적 영역 3.12점, 영적 영역 2.85점, 사회적 영역 2.79점 순으로 나타났다. 노인 병원 간호사의 호스피스 간호 교육이수와 수행은 전체 영역 간에는 순 상관관계를 보였다(r=0.31, p=0.000). 교육 이수정도와 간호수행의 상관관계를 살펴보면 신체적 영역(r=0.15,p=0.082)및 정서적 영역(r=0.06, p=0.508)은 통계적으로 유의한 관계가 없었으나, 영적 영역(r=0.26, p=0.002) 및 사회적 영역(r=0.33, p=0.000)은 교육이수정도와 간호수행간의 상관관계가 높게 나타났고, 이는 호스피스 간호수행에 있어 교육이 중요한 영향을 미친다는 것을 의미한다. 일반적 특성에 따른 노인 병원 간호사의 호스피스 교육이수 정도는 대부분 통계적으로 유의한 차이를 보이지 않았으나, 종교가 있고, 종교의 중요도가 높은 군에서 (t=3.906, p=.000) 호스피스간호 교육이수정도가 높게 나타났고, 4년제 졸업 이상(M=2.51, SD=0.88)의 학력을 가진 간호사가 교육의 이수정도가 높았으며 유의한 차이를 나타냈다(t=-4.358, p=0.000). 노인 병원 간호사의 호스피스 간호 수행 정도에 유의한 차이를 보인 일반적 특성은 전체영역에서의 종교의 유무(t=2.85, p=.005)와 영적 영역에서의 종교의 중요도였다(t=3.265,p=0.002). 이는 간호사의 영적간호 수행에 있어 종교의 중요도가 상당한 영향을 미치는 것으로 해석된다. 노인 병원 간호사의 교육 이수방법에 따른 간호수행 정도의 차이를 보면 원내교육을 받은 간호사에 있어서 전 영역의 간호수행 정도가 평균 3.39점으로 높은 수준을 보였으며, 교육 이수방법에 따른 호스피스 간호수행 정도의 차이분석에서 통계적으로 매우 유의하였다(F=9.425, p=.000). 본 연구결과, 노인 병원 간호사의 호스피스 간호 교육 이수정도는 매우 낮으며, 호스피스 간호수행에 있어서 신체적·정서적 간호의 수행은 높으나 영적·사회적 간호 수행은 부족하다는 것을 알 수 있었고 호스피스 간호 교육과 수행간의 상관관계 분석을 통해 호스피스 간호수행에 있어 교육이 매우 큰 영향을 미침을 알 수 있었다. 이는 호스피스 간호 수행을 위해서는 전문적인 호스피스 교육이 이루어져야 함을 시사한다. 또한 원내교육을 통해 호스피스 교육을 받은 간호사의 수행정도는 매우 높으나, 원내교육을 받은 간호사는 9.3%로 나타나, 노인 병원내의 호스피스 교육이 충분치 못함을 알 수 있었으며,병원 내 교육이 더욱 활성화 되어야 함을 보여준다. 이상의 결과를 바탕으로 전문적인 호스피스 간호수행과 후속 연구를 위한 제언을 하면 다음과 같다. 1.노인 병원 간호사들의 호스피스 간호에 대한 지식체계 및 인식의 재정립이 필요하며, 노인 호스피스 환자의 삶의 질 개선을 위해 영적·사회적 간호에도 보다 적극적인 간호 수행이 필요하다. 2. 질 높은 호스피스 간호 수행을 위해 간호 관리부서 차원에서의 체계적인 프로그램 개발 및 지속적인 원내 호스피스 간호 교육이 이루어져야 한다. 3. 노인 병원 내에서의 전문적인 호스피스 간호 수행을 위한 병원의 제도적 뒷받침과 호스피스 팀, 자원 봉사자등의 인력확보가 필요하다. 4. 노인 간호에서의 호스피스 간호의 중요성에 대한 인식 개선이 필요하며, 노인호스피스 대상자의 특성을 고려한 전문적이고 총체적인 호스피스 간호 수행이 이루어져야한다. 5. 노인 병원 간호사의 호스피스 교육이 체계적으로 이루어지고 간호 수행 및 평가가 시행되어야하며, 재교육, 심화교육을 통하여 양질의 호스피스 케어가 이루어져야 한다. 6. 본 연구의 결과에 나타난 노인 병원 간호사의 호스피스 간호수행과 교육과의 상관관계 및 교육 이수방법에 따른 간호수행 정도의 차이에 대한 추후 연구가 필요하다. 7. 노인의 특성을 고려한 노인호스피스 환자를 대상으로 하는 호스피스 간호 수행과 교육을 위한 체계적인 프로그램 개발에 대한 연구가 계속적으로 이루어져야 할 것이다.; This Study purports to examine old man hospice nursing achievement degree and hospice nursing education experience of nurses who work in the Geriatric hospital, and grasps interrelation between hospice nursing achievement degree and hospice nursing education . Source data for this study was collected from 141 nurses who work in Geriatric hospital in 12 places located in Seoul, Kyonggi and Chungchong area through questionnaire. Total questionnaire collected is 141 copies out of 170 copies, and utilized them in analysis of this study. Collected data was processed by using SPSS 12.0 K for Windows statistics program through computer and used real number, frequency, percentage, mean, order, standard deviation of descriptive statistics method and Pearson Correlation, Coefficient , T-test, ANOVA . The summary of this study is as follows: 1. The Geriatric hospital nurses 141 people was participated in this study. 20~29 age was more than 2/3 by 68.1% and maiden was higher ratio than married by 67.4%. Nurses with religion 48.2% out of 58.9% answered religion is important, 3 years graduates are more than 84.4%. The career experience with more than 3 years and less than 6 years is 41.1% and majority was general nurse (87.2%). The nurses with more than 10 times' experience on deathbed nursing was more than half by 53.2%. Also, all of the respondents answered hospice nursing education is needed. 2. As a result of analyzing degree which completes curriculums of hospice education for old man hospital nurse by order, it shows standard of education is very low by average 1.99 points and mental and social nursing, deathbed nursing were exposed by 2.0 points. And it appeared as 1.99 points in hospice nursing theory, pain administration, symptoms administration, physical nursing, spiritual nursing, subject for education, bereaved family nursing. 3. The methods how Geriatric hospital nurses could complete curriculums of hospice nursing education was examined. Total 118 persons out of 141 persons except 23 who didn't complete curriculums of hospice education were answered. The nurses who took hospice education through curriculums is 71.1% and the nurses who took hospice education through continuous education is 19.5%. And 9.3% took education in hospital. The nurses' education degree who took education in hospital was shown high level by 3.27 points and the nurses' education degree who took continuous education was shown at average 2.30 points. It was 2.01 points that the nurses got the hospice education through curriculums. That is, survey results are statistically meaningful in difference between hospice education method and education experience degree(F = 59.563, p = 0.000). 4. Geriatric hospital nurse's hospice nursing achievement was high by average 3.06 points in 4 points perfect score. Physical nursing was very high level by average 3.50 points, and was exposed in sentimental area by 3.12 points, spiritual area by 2.85 points, social area by 2.79 points in order for the measurement of nursing achievement in different area. 5. Education and achievement about Geriatric hospital nurse's hospice nursing are shown interrelation in Total province.(r = 0.31, p = 0.000) But, was exposed that nursing achievement of two provinces has no interrelation with education experience because there is in physical province (r = 0.15, p = 0.082) and sentimental province (r = 0.06, p = 0.508). Interrelation of nursing achievement and education experience of two areas in spiritual province(r = 0.26, p = 0.002) and social province (r = 0.33, p = 0.000) appears high and it shows that education experience is much influenced on general nursing achievement. 6. Geriatric hospital nurse's hospice standard of education by general special quality did not affect because it is not meaningful. But the religious group having more importance in religion (t = 3.906, p = .000) were appeared high in hospice nursing education experience. The nurses having scholarship of (M = 2.51, SD = .88) 4 years graduates and master of nursing than 3 years graduates (M = 1.90, SD = 0.54) were high in education degree, and displayed meaningful differences about education experience degree(t = -4.358, p = 0.000). 7. Achievement degree about Geriatric hospital nurse's hospice nursing by general special quality did not affect because it is not shown meaningful difference and general special quality shown meaningful difference in achievement degree was importance of religion(t=2.816, p=.006) and religion(t=2.85, p=.005) in Total nursing achievement. Geriatric hospital nurses who think religion importantly were shown (M = 3.06, SD = ± . 31) about high achievement in spiritual nursing achievement, and this is analyzed that importance of religion exerts fair effect in nurse's spiritual nursing achievement. 8. Nursing achievement degree of total province is average 3.39 points in a nurse who took education in hospital when it comes to analyze difference nursing achievement by geriatric hospital nurse's education method, so persons who fairly high level was seen, and who completes hospice nursing education through continuous education by average 3.11 points, who learned through class by 3.02 points, and which is very statistically meaningful by F (p) = 9.425 (. 000) in difference analysis in hospice nursing achievement by the method of education's completion. That hospice nursing achievement degree appears high in nurses who took education in hospital than who took hospice education through continuous education and curriculum etc. It was examined that they attend to nurse on person through education in hospital continuously with high attention on nursing. The suggestions are as follows for special hospice nursing achievement and successive study with result of above. 1. Need re-thesis of body of knowledge and realization about hospice nursing of Geriatric hospital nurses, and need more active nursing achievement to spiritual· social nursing for old man hospice patient's quality of life improvement. 2. Need quality systematic program development in nursing administration department dimension for high hospice nursing achievement and persistent hospice nursing education in hospital. 3. Need manpower security of systematic backing and hospice team of hospital for special hospice nursing achievement in geriatric hospital, resource servant etc.. 4. Need to improve the recognition about importance of hospice nursing in old man nursing, and need to implement systematic and general hospice nursing achievement by considering the characteristics of old man hospice. 5. Researcher will go study continuously and repeatedly to increase number of samples for generalization of study finding and representative-character security of sample because there is limitation that subject persons of this study represent the geriatric hospital nurses. 6. To estimate hospice nursing achievement degree and education experience degree, an used tool is not generalized and normalized, and an education experience estimation tool developed by investigator for this study is needed continuous and repeated research for tool's verification of validity and reliability. 7. Need study later that analyze interrelation with old man hospital nurse's hospice nursing achievement and education and cause about difference nursing achievement by education irrigation method which appeared in result of this study. 8. Research should be continued to develop he systematic program for hospice nursing achievement and education subject to old man hospice patients with consideration of old man's special quality.
