dc.description.abstract척추 질환 환자들의 다양한 치료계획을 얻기 위해서는 척추에 대한 생체역학적 이해와 해석은 매우 중요한 일이다. 직립보행을 하는 인류의 80%가 한번은 경험한다는 허리장애와 통증은 노인의 경우 유병인의 20%내외를 무력하게 만드는 위험을 지니고 있다. 척추증(spondylosis)과 후관절의 변형성 관절증(osteoarthosis)으로 대표되는 노화(aging)는 나이에 따른 퇴행증상이며, 생화학적 변화와 반복하중에 의한 것으로 정상기능이 저하되어가는 상태를 이름이다. 이런 과정중에서 통증의 유발은 등통증(back pain)외에도 사지에서 느껴지는 좌골신경통(sciatica) 같은 연관통(referred pain)을 가져오며 진단과 처방을 필요로 한다. 본 논문에서는 수술의 길잡이(surgical guide)로써 검증된 해부학적 유한요소 모델링 및 해석을 수행하였고 아울러 요통의 대표격인 심각한 추간판질환을 치료해온 골융합(fusion)을 대치하고 있는 인공디스크의 보다 효과적인 설계 및 대치에 따른 영향을 연구하였다. 우선, DICOM 파일로부터 골의 형상자료를 검토하여 정상여부를 판단하였다. 이미지 및 CAD 모델러를 사용하여 해부학적인 형태를 갖는 장골, 천추, L1부터 L5의 요추를 포함한 추체를 만든 후, 장요추 인대, 요추인대, 디스크, 후관절 등을 추가하고, 물성처리 및 유한요소의 사용으로 모델을 완성하였다. 각 단관절의 검증을 위하여, 하중 ±10Nm에서 시상면의 전후 굽힘 평균운동영역, 관상면의 좌우 굽힘 평균운동영역, 수평면의 좌우 비틀림 평균운동영역, 400N에서 평균수직변위, L12에서 전종과 후종인대의 하중, L45에서 시상면의 인대의 변형율, L45에서 디스크 화이버의 최대 변형율 등을 시험결과와 비교하였다. 다중관절의 검증을 위하여, 장요추 인대가 있고 없는 경우에 대하여 L1-S와 L5S 분절의 운동영역을 해당 실험의 운동영역과 비교하였다. 검증된 모델의 해석으로부터 다음의 결과를 얻었다. 1) 장요추 인대는 주로 앞 굽힘시 후방인대가, 좌우 굽힘시 좌우측 전후방인대가 안정화 역할을 하였다. 2) 장요추 인대는 전체모델의 운동조건에 따라 1-8%의 전체 운동영역을 감소시켰으며 또한 운동 조건에 따라서는 L5S 분절에서 후관절 하중은 0.8-1.4배, 디스크 하중은 0.9-1.5배까지 변경시켰다. 3) 장요추 인대중 후방인대는 앞 굽힘시 최대 16%, 비틀림시 최대 10%의 변형율이 예측 되었다. 4) 후방 장요추 인대의 강도 증대는 모멘트하중시, 주로 L5S 디스크 변형에 영향을 미치며 전체 시스템의 운동영역 감소 및 시상면 이동 변위의 감소를 유발하였다. 그러나 압축시 L45의 활주와 L5S의 ROM에 영향을 주었다. 5) 후방 장요추 인대의 높은 강성변화는 L5 전방돌출증의 원인인 전방 이동변위 및 그 운동영역을 제한할 수 있었다. 현재 시술되고 있는 수핵대치 인공디스크인 PDN을 해부학적인 모델에 맞도록 유한요소 모델링하였다. L45 분절에서 대치된 유한요소 모델을 동일한 실험조건의 하중하에서 운동영역을 비교하여, 적합한 모델임을 검증하였다. 단 모델 L45의 거동 외에도, 입증된 L3-S 또는 L1-S 장요-천추 모델의 L45분절에 PDN을 대치한 후 생체공학적인 문제를 예측하였다. 탈수된 PDN이 절개된 수핵의 동공에 위치될 때 PDN은 수화가 시작되어 점차 폴리에틸렌 팩안의 부풀어짐은 남아 있는 수핵과 접촉하게 될 것이며, 이 때 잔존 수핵은 역으로 탈수가 되어 탄력성은 감소되지만 탄성계수는 증대할 것이라는 체계적인 개념을 가정하였다. 연구 결과로 얻은 결론은 다음과 같다. 1)검증된 L45에 PDN이 대치된 유한요소 모델은 비교적 실험오차 내에서 적합한 모델이었다. 2)가정된 완전 접촉 상태와 주어진 하중 상태에서, PDN은 30-125㎛(L45분절), L3S 다중관절에서는 40-140㎛, 그리고 L1S 다중관절에서는 약 25-125㎛ 의 이동량을 나타냈다. 그러나 초기 이동량 이후 급격한 증가의 이탈량은 없었다. 3)대치 분절에서 운동영역은 건강한 인체보다 작았지만 위와 아래의 이웃 분절에서는 컸다. 4)PDN 옆면과 접하는 잔존수핵의 역할은 분절의 이동량과 ROM은 직접적인 관련이 있었다. 5)잔존 수핵의 탈수와 PDN의 부풀어짐이 진행됨에 따라서, 고립된 PDN 보다는 수핵과 접하는 PDN이 탈출된 수핵을 효과적으로 대치할 수 있음이 예측되었다. 향후, 개발된 장요추 인대를 갖는 L1S 유한요소모델과 대치된 PDN의 해석 기술은 수술 길잡이 뿐아니라 다양한 인공디스크 설계 및 FE 해석 기술에 응용 될 것이다.; The biomechanical comprehension and analysis to obtain various medical treatment designs which are related with the spine in order to cure and diagnose patients suffered from LBP, are very important subject. The low back disorder and low back pain which were once experienced in 80% of human beings to walk a standing, have a risk to be disable in about 20% of elder patients. The aging namely, regression change with aging which represents spondylosis and osteoarthrosis of apophysial joints, ultimately reflects in degradation of normal spinal function caused by change of biochemical level and biomechanical properties. Those process were involved by the referred pain i.e. sciatica which is perceived in legs and buttock or by the back pain, should be in need of a diagnosis and prescription. In this paper, the verified anatomical finite element modeling and its analysis was carried. And also the effect after implantation and better effective design of the artificial discs as sutable alternatives of bony fusion that have treated severe disc diseases i.e. typical disease of LBP, were studied. First, we investigated whether the geometrical configuration of vertebrae displayed by DICOM slice files was regular and normal condition. After constructing spinal vertebrae including a partial ilium, a sacrum and five lumbars (from L1 to L5) with anatomical shape reconstructed using softwares such as image modeler and CAD modeler, we added iliolumbar ligaments, lumbar ligaments, discs and facet joints, etc.. And also, we assigned material property and discretized the model using proper finite element types, thus it was completely modeled through the above procedure. For the verification of each segment, average sagittal ROM, average coronal ROM and average transversal ROM under various loading conditions(±10Nm), average vertical displacement under compression(400N), ALL(Anterior Longitudinal Ligament) and PLL(Posterior Longitudinal Ligament) force at L12 level, strains of seven ligaments on sagittal plane at L45 level and maximal strain of disc fibers according to various loading conditions at L45 level, etc., they were compared with experimental results. For the verification of multilevel-lumbosacrum spine, ROMs of L1-S1 and L5S in the cases with and without iliolumbar ligaments were compared with their ROMs of those experiments. The results were obtained from analysis of the verified FE model as follows: 1) Iliolumbar ligaments played a stabilizing role as mainly posterior iliolumbar ligaments under flexion and as both posterior and anterior iliolumbar ligaments of one side under lateral bending. 2) The iliolumbar ligaments decreased total ROM of 1-8% in total model according to various motion conditions, which changed facet contact forces of L5S level by approximately 0.8-1.4 times and disc forces of L5S level by approximately 0.9-1.5 times more than case without ilioligaments, under various loading conditions. 3) It was predicted that strains of posterior ligaments among iliolumbar ligaments would produce the maximum 16% under flexion and the maximum 10% under twisting. 4) As the elasticity of posterior iliolumbar ligaments changed, they produced mainly disc change at L5S level and the global reduction of ROM and the translation on sagittal plane under mement conditions. But under compression, they influence not only translation of L45 level but also ROM of L5S level. 5) It was confirmed that high stiffness of posterior iliolumbar ligament could put restriction upon higher anterior displacement and its ROM that was caused of L5 spondylolythesis. The two type PDNs which have been used in operating, were modeled to fit in the present disc size. Under the same condition as the reported experimental loading; moment of ±7.5Nm and compression of 200N, the verification of L45 segmental motion with three implements such as intact disc, nucleotomy and implanted PDNs was carried out. After the PDN were inserted at L45 level among the verified L1S(or L3S) multijoint as well as only L45 level, the biomechanical problems were predicted. It was assumed as a schematic concept that when the hydrated PDN is positioned in the cavity of the excised nucleus, the hydration of PDN begins, gradually the swelling of PDN in polyethylene pack comes in contact with the remained nucleus, the remainder nucleus just dehydrates reversely, and its elasticity increases but its flexibility decreases.The results were obtained by using the verified finite element model as follows; 1) The verified FE model with an inserted PDN of L45 level was compatible model within experimental result range.2) Under assumed perfect bonding condition, the PDNs showed migration 30-125 micrometer (at L45 single level), 40-140 micrometer (at L3S multi level) and about 20-125 micrometer (at L1S multi level), under given loading condition. But the steeply increasing migration was not shown after initial migration.3) The ROMs were more small than those of intact at the operated level, but they showed more big than those of intact at the adjacent of upper and lower level.4) The roles of the remainder nucleus that was contacted with PDN sides, were directly related with migrations and ROMs of the spinal segment.5) As the dehydration of the remainder nucleus and the swelling of PDN were processing, it was predicted that the behavior of the PDN which was contacted with side fats should be effectively replaced with herniated nucleus, better than an isolated PDN. For further study, the developed L1-S FE model with iliolumbar ligament and analysis technique after implantation will be applied to the design and FE analysis of various artificial discs as well as the function of surgical guide.-
