보조생식술의 임상가이드라인에 대한 연구

Abstract

연구배경 : 불임 치료에 있어서 보조생식술은 지난 30년 간 매년 증가되는 추세로 전 세계적으로 시술되고 있으며, 기술적인 발달은 거의 믿을 수 없을 정도로 놀라운 발전을 거듭해 왔다. 그러나 많은 새로운 기술들이 앞으로의 문제점에 대하여 잘 예측할 수 없는 것처럼 이 역시 환자에게 해가 될 수 있는 부분에 대하여는 완전히 알려져 있지는 않다. 그러므로 보조생식술을 시행하는 데 있어서 근거 중심의 의학(evidence-based medicine)에 입각하여 시술을 해야 하며, 가장 잘 제시된 가이드라인을 따라서 각각의 환자에 맞는 치료를 하는 것이 매우 중요하다 하겠다. 최근 보조생식술에 대하여 세계 여러 나라에서도 가이드라인을 제시하여 보조생식술의 남용을 막고, 적절한 진료를 할 수 있도록 시도하고 있다. 연구 목적 : 세계 여러 나라 보조생식술 가이드라인의 근간이 되는 영국의 NICE 가이드라인과 이의 모체인 RCOG 가이드라인을 분석하고, 이를 우리나라의 가이드라인 즉 대한산부인과 학회에서 만들어진 보조생식술 가이드라인과 비교하며, 향후 우리나라의 가이드라인을 만드는 데 도움이 되고자 한다. 연구 방법 : 영국의 NICE 가이드라인과 RCOG 가이드라인을 우리나라의 대한산부인과 학회에서 만들어진 보조생식술 가이드라인과 비교, 분석하였다. 연구 결과 및 고찰 : 보조생식술 영역에서의 NICE 가이드라인, RCOG 가이드라인 및 우리나라의 가이드라인을 비교 분석하였으나 우리나라 가이드라인이 임상에 관한 것은 매우 부족한 실정으로 이에 저자는 우리나라의 보조생식술 임상 가이드라인을 우리나라의 실정에 따라 만들어야 한다는 의견을 제시하는 바이다. NICE 가이드라인은 불임상담, 불임의 위험인자, 불임 시술, 불임 시술의 경제적 측면 등 다양한 방면에서 폭넓은 가이드라인을 제시하고 있어 이를 근거로 우리나라 실정에 맞는 가이드라인을 만든다면 향후 불임 진단과 치료에 대한 좋은 이정표가 될 것이며, 이는 불임 환자에게도 많은 도움이 될 것으로 사료된다.; In 1978, Louise Brown, the first baby conceived following fertilization in vitro, was born in England. This is often attributed as the starting point of IVF. Since that moment, the field of IVF has been transformed and developed rapidly; a steady stream of discoveries and technological progress has led to an expansion of the assisted reproductive techniques(ART). For these reasons, the clinician who treats infertile couples should know the appropriate management of the infertility under the best guidelines. Guidelines are 'systematically developed statements to assist decisions about appropriate care for specific clinical circumstances'. Guidelines have been shown to improve the process of care and improve outcomes for patients. The Royal College of Obstetrics and Gynecologists(RCOG) have produced three guidelines in ART areas. The RCOG guidelines provide recommendations to help clinicians and other staff in managing infertile couples. In 2001, the National Institute for Health and Clinical Excellence commissioned the National Collaborating Centre for Women and Children's Health to produce clinical guidelines for clinically effective, cost-effective and appropriate infertility treatment. These guidelines were to build on the existing RCOGs' guidelines on the management of infertility, incorporating any new published scientific evidence available, in order to produce a seamless guide to the management of the infertile couple. This guideline has been used in many countries in the world. The aim of this study is to compare the Korean guidelines with NICE guidelines and RCOG guidelines in the field of ARTs, and to help produce a new Korean clinical guidelines of ART that could be used by all professional groups involved in providing fertility services, including policy-makers, as well as couples seeking advice and possible treatment for infertility. This study reviewed and compared three guidelines. The results was that there were no clinical guidelines of ARTs in Korea such as NICE or RCOG guidelines. The NICE guideline document was heralded as an essential prerequisite to establishing a fair national policy for the investigation and treatment of infertility. NICE guideline will help develop the new guideline in Korea and potentially, this document could act as a landmark in the approach to a comprehensive infertility service, and be milestones in making new Korean clinical guidelines of ARTs.
