지방자치단체 노인의료복지서비스 공급체계의 효율화 방안

Abstract

First, it is necessary to divide the roles appropriately among service providers. The central government is obliged to provide for indirect supports and coordinate the medical welfare service programs, while rearranging relevant laws, regulations and guidelines and developing the medical welfare service programs at the national level and establishing an appraisal system. In addition, the central government should secure the budgets and support other medical welfare service providers. On the other hand, the local governments are requested to play actual supporting roles, cooperate with markets and private organizations and guide and supervise them, while regulating them to a minimum degree. In addition, local government should coordinate the roles among other service providers, provide them with information, secure the budget and support them and arrange some incentive systems. Private organizations and markets are obliged to cooperate with each other and share information, perform the functions of education and training, monitor the medical welfare service activities as public actors and construct a local community cooperative system. Second, it is essential to enhance linkage between service providers and relevant services. Above all, administrative sectors should be linked with each other basically for integration and linkage among service providers. And in order to facilitate such integration and linkage by geographic unit, it is necessary to construct a primary net of linkage for each local community and then, expand such nets at the lower level of local autonomy. Namely, it is necessary to construct a community care system for the elderly medical welfare services at the level of community. Third, it is necessary to enhance the leadership for coordination. A connection pin or leadership is required to coordinate roles and activities among plural service providers. In case of coordination in the public sector, who will take initiative is not important. Instead, it is necessary to approach the public sector from the perspective of easy coordination among public and private service providers as well as efficiency of services. All in all, it is necessary to arrange an appropriate incentive system in consideration of budget, human resources and administrative capacity and thereby, operate the leadership coordinating the roles among administration, private organizations and markets.; WHO를 비롯하여 OECD, ILO, EU 등에서는 1990년대 초반부터 고령화 사회를 대비하여 ‘활기찬 고령화(active ageing)’, 또는 ‘건강한 노화(healthy ageing)’라는 기본이념을 가지고 건강증진, 질병예방, 일차보건의료에 대한 공평한 접근 및 장기요양서비스에 대한 균형적 제공, 노인의료복지시설 확충에 중점을 둔 생애전반적인 측면의 노인의료복지체계를 추구하여야 한다고 권고하고 있다. 한국의 경우 노인인구 2020년에는 65세 이상의 노인인구가 총인구 대비 13.2% 수준으로 높아질 전망이며, 평균수명 또한 78.1세로 늘어나 2020년경에는 고령화 사회(Aging Society)에서 고령사회(Aged Society)로 전이될 것으로 예상된다. 더욱이 한국의 경우 고령화 수준은 선진국에 비하여 상대적으로 낮은 편이나 고령화 진전의 속도가 사상 유례가 없을 정도로 빠르다는 점을 감안할 때, 이에 대한 대책이 시급하다 하겠다. 그러나 이러한 고령화의 문제는 중앙정부의 힘만으로는 점증하는 노인의료복지 수요에 대응하는데 한계가 있다, 따라서 정부, 시장, 자원부문, 비공식부문 등 다양한 복지서비스 공급주체들을 총체적으로 활용할 수 있는 공급체계를 구축하는 것이 필요하다. 본 연구는 이러한 문제의식에서 출발하여 ‘복지다원주의(welfare pluralism)'의 관점에서 지방자치단체인 송파구를 중심으로 노인의료복지서비스 공급체계의 효율화 방안을 다음과 같이 모색하였다. 첫째, 서비스 공급자들 간의 적절한 역할 설정이 필요하다. 중앙정부는 간접적인 지원과 조장자의 역할 그리고 전국적인 기준설정(관련법규의 제정 및 정비, 전국적 차원의 프로그램 개발 및 평가체계의 확립, 예산의 확보 및 지원 등이 필요하다. 그리고 지방정부는 실질적 지원자의 역할, 시장 및 민간단체 등과의 협력 및 지도감독, 필요한 최소한의 규제자, 공급자들 간의 역할 조정, 정보제공, 예산의 확보 및 지원, 인센티브제도 마련 등이 요구된다. 또한 민간단체 및 시장은 상호 협조 및 정보공유, 교육 및 훈련기능, 공익단체로서의 감시기능 및 이익투입기능, 지역사회 협력체제 구축 등의 역할이 필요하다. 둘째, 공급주체 및 관련 서비스의 연계성을 강화해야 한다. 우선 서비스 공급자들 간의 통합 및 연계성과 관련하여 기본적으로는 행정부문간의 연계가 먼저 되어야 한다. 그리고 통합 및 연계의 지리적 단위와 관련해서 작게는 지역공동체 단위에서 일차적인 연계망을 구축하고, 그 다음 단계로는 기초자치단체 수준에서 연계망을 구축하는 것이 현실적일 것이다. 즉, 지역사회 단위에서는 노인의료복지서비스관련 커뮤니티 케어(community care) 시스템을 구축해야 한다. 셋째, 조정을 위한 리더십 강화가 요구된다. 다원화된 공급주체들 간에는 이들의 역할과 활동을 조정해주는 연결핀 내지는 리더십이 필요하다. 공공부문 내에서의 조정의 경우는 누가 주도권을 갖느냐는 차원에서 접근해서는 안되며, 업무효율성 및 민간부문을 포함하는 통합적인 조정의 용이성 측면에서 접근을 해야 할 것이다. 따라서 예산, 인력, 행정 등을 고려하여 적절한 인센티브제도를 수행함으로서 행정부문, 민간단체, 시장으로 역할이 가능하도록 조정기능을 수행하게 하는 리더십이 필요하다.; Various international organizations including WHO, OECD, ILO and EU have recommended since early 1990's that with the basic ideology 'active aging' or 'healthy aging', nations should, in preparation for the aging society, establish a medical welfare service system for the elderly focusing on improvement of health, prevention of diseases, equal access to the primary health and medical services, balanced long-term rehabilitation services and expansion of the medical welfare service facilities. In case of Korea, the people aged 65 or older are expected to account for 13.2% of the entire population by 2020, while the average life span would be expanded to 78.1 years. In a nutshell, Korea will proceed from the aging society to an aged society by 2020. Moreover, although Korea's aging level is relatively low compared with advanced nations, her aging speed is very fast unprecedently, requiring urgent measures. Nevertheless, the central government alone cannot afford to respond to the problems of the rapidly aging society, particularly the gradually increasing needs for the elderly medical welfare services. So, it is deemed necessary to construct a medical welfare service delivery system encompassing diverse sources of welfare services such as government, markets, resources and unofficial sectors. From the perspective of 'welfare pluralism', this study sampled Songpa Gu, Seoul, a lower level local autonomous administration to explore the ways to construct an efficient medical welfare service delivery system for the elderly, and thereby, suggested the following reform measures
