임상간호사의 근무환경과 건강문제와의 관계

Abstract

The purpose of this study is to identify the relationship between health problems, working environment, presenteeism, absenteeism and productivity of clinical nurses. Data was collected from structured questionnaires from January 7th 2008, to March 6th 2008, on nurses working in 2 university hospitals of the same foundation in Seoul and Gyunggi area. Information of total of 638 nurses was analyzed by SPSS 16.0 program. Study result is as below. 1. Average age of clinical nurses is 31.43. Their average working period in the current workplace is 7.86 years and 34.8% of them had departed from previous work place. Their average departure frequency is 1.35 times. They work night shifts 5.15 times a month on the average and 16.26 hours of over time a month on the average. 2. The clinical nurses had 4.92 health problems out of 16 for the last one month. They had most shoulder, back, lower back and neck pain, accounting for 79.8 %, followed by fatigue and edema of feet with 64.9% and gastrointestinal disorders with 57.5%. Out of health problems that they experienced, 28.7% of them were most concerned with their shoulder, back, lower back and neck pain, followed by 14.6% of them with gastrointestinal disorders and 11.6% with fatigue and edema of feet. 3. Level of working environment recognition was 7.43 out of 16 points. The average score out of 100 for presenteeism due to most concerning health problem out of those experienced for the last one month was 44.35. Average absenteeism was 2.04 hours and average productivity was 71.31 out of 100. 4. There was a negative correlation of Pearson Correlation Coefficient -.205 between working environment recognition level and number of health problems. There was a negative correlation of Pearson Correlation Coefficient -.181 between working environment recognition level and presenteeism, and a negative correlation of Pearson Correlation Coefficient -.143 between working environment recognition level and productivity. These study results all together indicate various strategies to reduce number of health problems of clinical nurses and their presenteeism as well as to improve working environment to heighten recognition level of their working environment in order to raise nurses' productivity are needed.; 의학의 발달과 지속적인 경제 성장으로 보건의료분야에 대한 국민들의 욕구가 팽배해짐에 따라 질 높은 의료서비스를 효율적으로 충족시키기 위해 최 일선에서 의료서비스를 제공하는 간호사의 역할과 책임이 매우 중요하다. 간호사의 근무환경이나 직무수행과정에서 긍정적이든 부정적이든 간에 여러 가지 신체적, 정신적 건강문제가 발생하며, 이러한 건강문제는 간호사 개인의 문제 뿐 아니라 환자 간호의 서비스 질 저하와 의료기관의 경제적 손실이라는 결과를 가져온다. 이에 본 연구는 임상간호사를 대상으로 근무환경과 건강문제에 초점을 두어 근무환경 인식정도와 건강문제, 건강문제로 인한 직무손실, 결근율, 생산성 정도의 관계를 파악하는데 목적이 있다. 서울·경기지역 소재 동일 재단인 2곳의 대학병원에 근무하는 간호사를 대상으로 2008년 1월 7일부터 3월 6일까지 구조화된 설문지를 이용해 자료를 수집하였다. 총 638명의 자료를 SPSS 16.0 Program을 이용하여 분석하였다. 연구결과는 다음과 같다. 1. 임상간호사의 평균 연령은 31.43세였다. 현 근무지 평균 재직기간은 7.86년이었고, 이직 경험이 있는 임상간호사는 34.8%로 평균 이직 빈도는 1.35번이었다. 월 평균 야간근무 횟수는 5.15번, 월 평균 초과근무 시간은 16.26시간이었다. 2. 지난 1개월간 임상간호사들이 경험한 평균 건강문제수는 총 16가지 중 4.92였다. 어깨, 등, 허리, 목의 통증이 79.8%로 가장 많았고, 다음이 발의 피로 및 부종 64.9%, 위장장애 57.5% 순이었다. 경험한 건강문제 중 가장 염려되는 건강문제 역시 어깨, 등, 허리, 목의 통증이 28.7%로 가장 많았고, 다음이 위장장애 14.6%, 발의 피로 및 부종 11.6% 순이었다. 3. 근무환경 인식정도는 총 16점 만점 중 7.42점이었다. 지난 1개월간 경험한 건강문제 중 가장 염려되는 건강문제로 인한 직무손실의 전체 총점은 100점 만점 중 평균 44.35점이었다. 결근율은 평균 2.04시간이었으며, 생산성 정도는 100점 만점 중 평균 71.31점이었다. 4. 근무환경 인식정도와 건강문제수 사이에는 피어슨 상관계수 -.205의 음의 상관관계가 있었다. 근무환경 인식정도와 직무손실 사이에는 피어슨 상관계수 -.181의 음의 상관관계, 근무환경 인식정도와 생산성 정도 사이에는 피어슨 상관계수 .143의 양의 상관관계가 있었다. 이상과 같은 결과를 종합하여 볼 때, 임상 간호사의 건강문제수와 직무손실을 감소시키고, 간호의 생산성을 높이기 위해서 근무환경에 대한 인식정도를 높일 수 있는 근무환경 개선을 도모할 다양한 전략들이 필요하다.
