dc.description.abstract국문요약 본 연구는 노인환자의 회복실 체류시간에 영향을 미치는 요인들을 파악하기 위한 서술적조사연구로서 서울시 소재 S 대학병원에서 2009년 4월 1일부터 2010년 3월 31일까지 전신마취 및 부위마취 하에 수술을 받고 회복실에 입실하였던 환자 중 65세 이상인 환자 538명을 대상으로 후향적인 조사방법으로 실시하였다. 회복실 체류시간에 영향을 미치는 요인으로 31가지 요인을 측정하였으며, 세부적으로는 대상자의 일반적 특성 3가지, 수술 전 요인 4가지, 수술 중 요인 12가지, 수술 후 요인(회복실 요인) 12가지이다. 분석방법으로는 SPSS/WIN 12.0 program을 이용하였으며, 평균과 표준편차, ANOVA, 다중회귀분석방법을 사용하였다. 본 연구의 결과는 다음과 같다. 회복실 체류시간은 평균44.6분으로 나타났으며, 30-59분이 69.7%로 가장 많았다. 대상자의 일반적 특성에 따른 회복실 체류시간은 성별에 유의한 차이를 보였다. 수술 전 요인에 따른 회복실 체류시간은 과거병력, 수술부위에 유의한 차이를 나타냈다. 수술 중 요인에 따른 회복실 체류시간은 마약제제사용, 수혈, 수액공급량, 요 배설량, 비정상 ABGA에 따라 유의한 차이를 보였다. 수술 후 요인에 따른 회복실 체류시간은 회복실에서의 고혈압, 통증점수, 수혈, 입실 시 SPO2, 입실 시 회복지수(PAR score)에 따라 유의한 차이를 나타냈다. 수술 전‧ 중‧ 후 요인들에 대한 다중회귀분석을 시행한 결과 회복실 입실 시 SPO2, 수술 중의 비정상 ABGA, 회복실에서의 수혈, 회복실에서의 고혈압, 회복실 입실 시 회복지수(PAR score), 수술 중 수액공급량, 회복실 입실 시 통증점수, 수술 중의 수혈 순으로 회복실 체류시간에 영향을 미치는 것으로 나타났으며, 전체 설명력은 45.1%였다. 회복실 입실 시의 SPO2가 가장 유의한 변수로 나타났는데, 이는 회복실에서 측정되었으므로 회복실 요인으로 분류하였지만, 수술 전 요인 중 과거병력, 수술 중 요인에서의 마약제제사용, 실혈량, 마취방법, 근이완제 사용 유무 등의 중요한 영향요인들의 상호작용의 결과로 대표될 수 있는 의미 있는 요인이라 생각된다. 또한 8개의 영향요인들 중 회복실에서의 요인들이 5개로 이들이 수술 전‧ 중의 요인들에 비해 중요한 영향을 미치는 것으로 나타나 회복실에서의 영향요인들을 주의 깊게 모니터링하고 신속하게 처치하여 교정하는 것이 회복실 체류시간의 단축에 기여할 것으로 여겨진다.; ABSTRACT Influencing factors on Length of Stay in the Post-anesthesia Care Unit in Elderly Choi, Ji Yeon Dept. of Gerontology & Dementia Nursing The Graduate School of Information in Clinical Nursing Hanyang University Directed by Prof. Lim, Nan Young As a descriptive study based on the research designed to figure out major factors that influence the length of the elderly’ staying in PACU, this study was carried out in the S university hospital in Seoul from April 1, 2009 to March 31, 2010 to a subject of 538 patients more than 65 years old who were hospitalized in a recovery room after having had operation under general or local anesthesia with anterograde survey methods. There are in total 31 factors that influence the length of the patients’ staying in PACU; to be specific, 3 general characteristics of them, 4 factors before operation, 12 factors during operation, and 12 factors after operation (in other words, in PACU). For an analysis measures, SPSS and WIN 12.0 program were utilized. We sought for average and standard deviation, t-test, ANOVA and multiple regression analysis. The result of this study is as follows. The length of patients’ staying in PACU on average was 44.6 minutes with a range of 30 – 59 minutes marked as the highest occupying approximately 69.7%. The length of the time according to general characteristics of the patients showed that there are some differences between male and female patients. The length of the time according to factors before operation showed some differences owing to patients’ medical history of having operation in the past or their operation area. According to factors during operation, it indicated that some differences are dependent on drug materials, (blood) transfusion, supply of infusion solution, urine output, and abnormal ABGA. In the matter of factors after operation, it revealed that some differences are up to patients’ high blood pressure in PACU, pain score , (blood) transfusion, and SPO2, PAR Score when they were moved in. The result of an application of multiple regression analysis to factors before, during and after operation proved that what mainly influence on the length of the time are, form the most important to the least, SPO2 when they were moved in a recovery room, abnormal ABGA during operation, (blood) transfusion, hypertension in it, PAR Score when they were moved in, the supply of infusion solution during operation, pain score, (blood) transfusion during operation: its total indicator was 45.1%. The most important thing in influencing the length of patients’ staying in PACU was SPO2 when they were moved in it; although it was categorized into one of factors in PACU, it is possible to consider it as a result of interactions between important influential factors such as patients’ medical history in the past among factors before operation and whether or not drug materials, a Placebo in Decreasing Blood Loss, methods of anesthesia, and skeletel muscle relaxants were used for operation. Moreover, the fact that out of 8 influential factors, 5 belong to factors in PACU means that the 5 factors are much more important than other factors before and during operation. In order to shorten the length of patients’ staying in PACU, what is at issue most is how to monitor each physical condition of the patients and promptly take action to them in case of emergency.-
