의사-환자 간 공유된 의사결정이 류마티스 질환자의 진료만족도에 미치는 영향

Abstract

Background: Patient's participation in treatment planning is being increasingly advocated in chronic disease management. There is good evidence that most patients wish to be involved in medical decisions concerning their treatment. It is however still unclear to what extent patient's with rheumatic diseases can be engaged in medical decisions and whether this procedure has positive effects on treatment outcomes and their satisfaction.

Objective: The objective of this study is to examine the relationship between shared decision making and patient's satisfaction during the consultation. Then, this study determined the influencing factors related to the satisfaction of patient's with rheumatic diseases.

Method: A survey was conducted in patient's with rheumatic disease who visited a university specilaised hospital for rheumatic disease in Seoul, Korea, from June 1 to June 30, 2011. The design of the study was descriptive cross-sectional, and data were collected using a 37-items questionnaire. The subjects were three hundred seventy two patients. Data were analyzed using 'SPSS Statistics 18.0 Network Version(on release 18.0.1 of PASW Statistics)' program. Various statistical methods were applied for analyzing the data to obtain a profile for the patients and to identify the factors which influence patient's preference for shared decision making. Logistic regression analysis was employed to determine the predicting variables of the satisfaction of the patients.

Results: 84% of participants have a desire to participate in treatment process. 72.7% of participants have experienced on shared decision making. The level of education and religion are influencing factors on the difference in the degree of involvement in decision making. The level of satisfaction of patient's with rheumatic diseases was positively related to the level of expectation on participation in decision making on treatment. Patient preference for shared decision making, decrease in pain level, and improvement in social role were all significant predictors of patient's satisfaction.

Conclusion: Participation in shared decision making in the patient's with rheumatic diseases during the consultation is related to patient's satisfaction. The affective quality of the consultation seems to be the most important factor in determining the outcomes of treatment. For improving the outcomes, a tool for identifying the patient's perception of shared decision making needs to be developed.|환자의 치료과정에 있어서 환자의 의사결정 참여가 최근 중요한 이슈로 부각되고 있다. 의사-환자 간 공유된 의사결정에 대한 연구는 외국에서는 지난 20여 년 동안 많은 논의와 연구가 진행되어왔으나, 국내 연구는 최근 몇 년 전부터 외국의 문헌을 바탕으로 연구되어 지고 있는 상황이며 아직은 단순분석 정도에 머물고 있다. 국내의 의료 환경의 변화에 적응하기 위해서는 진단 및 치료과정에 있어서 환자가 의학적 의사결정과정에 어느 정도 참여하고 있으며 그 참여정도에 따른 치료 결과와 진료만족도에 어떠한 영향을 미치는지를 파악해 볼 필요가 있다.

이 연구의 대상자는 서울 소재 류마티스 전문병원을 이용하는 외래 환자들이며, 조사방법은 설문지를 이용한 조사의 방법으로 총 400부를 배부하여 설문의 응답이 충실하지 않은 28명을 제외한 372명을 연구대상자로 하였다.

분석 방법은 연구대상자들에 대하여 인구사회학적 특성, 의료기관 이용 행태, 류마티스 질환별 특성, 질환 및 치료관련 특성, 질환의 심각성 요인별 특성 등을 독립변수로 분류하고 공유된 의사결정 참여도 및 진료만족도를 종속변수로 두어 빈도분석, t-test, 분산분석, 상관관계분석 및 회귀분석의 방법을 적용하였다.

연구결과 의사결정 참여도에는 모든 연구대상자의 84%가 의사 결정 시 참여하기를 기대한다고 하였으며 75.7%가 진료 시 의사 결정에 참여하였다고 하였다. 인구사회학적 특성에 따른 의사결정 참여도는 특별히 유의한 차이를 보이지 않았지만, 학력에서 고졸이하가 대학교이상보다 참여 정도가 높고, 종교를 가지고 있는 대상자보다 종교가 없는 대상자의 의사결정 참여 정도가 낮은 점에는 유의한 차이를 보였다. 류마티스 질환별로는 강직성 척추염 환자가 다른 류마티스 질환자보다 공유된 의사결정 참여도가 높은 유의한 차이를 보였으며, 각 진단 기간별 공유된 의사결정 참여도의 차이에 대해서는 유의함을 보이지 않았다. 류마티스 질환으로 인한 사회적 역할 수행 장애, 통증의 정도, 신체활동손상, 경제적 손실, 심리적 불안감의 항목에서는 심각하지 않게 이닉하고 있는 대상자가 심각하게 인식하고 있는 대상자보다 공유된 의사결정 참여도가 유의하게 높게 나타났다.

환자의 진료만족도에 대해서는 우선 모든 연구대상자의 치료 전 통증세기가 평균 6.73에서 치료 시작 후 평균 3.48의 통증세기로 완화되었고, 일상생활에 어려운 정도에 대해 치료 시작 전 잘 지냄 평균 3.48 에서 치료시작 후 평균 6.25로 높아졌다고 하였다. 모든 연구대상자의 진료만족도에 대한 분석결과는 의사와 환자가 의사결정을 공유하는 것이 환자만족도에 매우 중요하다는 것이 95.2%(354명), 의료기관과 의료진에 대한 만족에 대해 89.8%(334명), 의사가 본인의 질병을 잘 알고 진료를 한다는 응답률은 85.5%(318명), 검사선택의 적절성에 대한 만족은 82.8%(308명), 건강상태 호전에 대해서는 79.6%(296명)가 만족한다고 하였다. 인구사회학적 특성에 따른 진료만족도에 대한 분석결과는 각 변수에 따른 진료 만족도에서 유의한 차이를 보이지 않았지만, 학력수준이 고졸이하 보다 대학이상의 학력에서 공유된 의사결정의 중요성에 대한 항목에서 더 높은 평균적인 만족도를 나타내 유의한 차이점을 보였다. 연구대상자들의 류마티스 질환별 진료만족도는 유의한 차이를 보이지 않았으며, 각 진단 기간에 따른 진료만족도의 차이에 대해서도 유의함을 보이지 않았다. 류마티스 질환으로 인한 사회적 역할 수행 장애, 통증의 정도, 신체활동손상, 경제적 손실, 심리적 불안감의 항목에서는 심각하지 않다고 인식하는 연구 대상자가 심각하다고 인식하는 연구대상자보다 진료만족도가 유의하게 높았다.

의사와 환자의 공유된 의사결정 참여도에 따른 진료만족도에 대해서는 진료만족도에 대한 10개항의 질문에 8개 항목에서 공유된 의사결정 참여도가 높은 연구대상자의 진료만족도의 평균이 높은 유의한 차이를 보였으며, 진료만족도와 공유된 의사결정 참여도 간의 상관관계 분석 결과 순상관 관계를 나타냈다. 따라서 공유된 의사결정, 의사결정 참여정도와 의사결정 참여 기대수준이 높을수록 진료만족도가 높다는 사실을 알 수 있었다. 회귀분석결과 공유된 의사 결정은 진료만족도에 유의한 영향요인으로 , 의사-환자 간 공유된 의사 결정 참여도가 높을수록 진료에 대한 만족도가 높아지는 것으로 분석되었다.

이 연구는 서울소재 류마티스 전문병원에 내원하는 외래환자에 국한하였기 때문에 연구의 결과를 모든 환자에게 적용하거나 환자의 의사결정과 민접한 관련이 있는 환자의 가족 등의 공유된 의사결정 참여에 대해서는 한계가 있다. 이 연구는 국내에서는 처음으로 류마티스 질환자를 대상으로 공유된 의사결정이 진료만족도에 미치는 영향에 대한 연구이며, 공유된 의사결정 참여도를 측정하기 위한 도구는 선행연구들에 비해 세분화되고, 공유된 의사결정 참여도에 직접 관련된 요소로 진료만족도를 구성하였다는 점에서 의의가 있다. 이 연구가 하나의 기초 자료가 되어 다른 질환자들과 환자들의 의사결정에 영향을 미칠 것으로 생각되는 환자의 가족 등을 대상으로 한 연구에도 활용될 수 있기를 기대한다. 아울러 환자의 공유된 의사결정 참여는 환자의 의학 지식과 역할이 수반되므로 의사-환자 간의 의사소통과 의사의 설명을 통한 환자의 의학지식에 대한 이해를 높일 수 있는 교육 등 제도 보완과 환자의 역할을 결정하는 연구가 진행되길 제언한다.; Background: Patient's participation in treatment planning is being increasingly advocated in chronic disease management.
