요양시설의 치료적환경이 치매 노인의 삶의 질에 미치는 영향

Abstract

노인의 인구 증가로 고령화 사회로의 진입이 급속도로 이루어지면서 그에 따른 여러 노인 문제도 끊임없이 대두되고 있다. 그 중 고령인구의 질병 또한 피해갈 수 없는 문제이며, 특히 세기의 질병(Century Disease)이라 불리는 치매에 대한 심각성이 부각되고 있다. 그와 더불어 현대 사회 변화의 특성상 전통적으로 이어져내려 온 자녀세대의 부모 부양 기능의 약화로 치매노인들을 모시는 요양시설의 중요성은 점점 더 커지고 있다. 따라서 치매노인이 거주하는 시설의 환경에 대한 연구는 그 필요성이 더욱 부각되고 있다. 이에 본 연구는 치료적 환경이 노인의 삶의 질에 중요한 영향을 끼친다는 가설아래 구체적 분석을 중심으로 살펴봄으로써 요양시설의 치료적 환경과 삶의 질의 관계에 대해 모색하려 한다. 본 연구의 목적은 치매노인을 위한 요양시설의 물리적· 공간적· 사회적 환경범주를 정리한 치료적 환경을 바탕으로, 치매노인의 특성과 니즈(needs)에 적합한 치료적 환경을 제공하는 요양시설과 치매노인의 삶의 질의 관계분석을 통해 향후 최소한의 법적 시설기준만을 충족하는 요양시설에서 벗어나, 치매노인에게 조금 더 긍정적인 영향을 미칠 수 있는 바람직한 치료적 환경을 계획하는 것에 있다.

설문 방식의 조사 결과에서 응답자의 생활만족 수준에 통계적으로 유의한 영향을 미치고 있는 시설 관련 변수는 공간적 환경과 사회적 환경인 것으로 나타났으며, 이러한 요양시설의 치료적 환경(공간적 특성과 사회적 관계에 관한 조건들)은 치매노인의 삶의 만족도에 의미 있는 영향을 미치고 이것이 치매노인의 삶의 질에 긍정적인 영향을 주었다는 점을 알 수 있었다. 본 연구를 바탕으로 분야 간의 경계를 두지 않고 모두 아우른 치료적 환경에 대한 꾸준한 관심과 보강이 요구되며 치료적 환경의 범주에 따른 좀 더 세밀한 지표 개발 및 다양한 치료적 환경을 고안하고 연구한다면 요양시설에 거주하는 치매노인의 삶의 질이 향상되어 조금 더 유의미한 영향을 미칠 수 있을 것이다. 또한 본 연구가 치매노인을 대상으로 하는 연구이기 때문에, 치매노인의 증상이 각기 다른 점을 감안 하여 모든 치매노인에게 일반화하여 해석하기에는 무리가 있으나, 본 연구를 토대로 치매노인과 치료적 환경 간의 관계에 대한 연구의 가능성을 발견하였음에 의의를 두고, 앞으로 치매노인의 주관적 지표화의 개발이 무엇보다도 선행되어야 할 것이라 본다. 앞으로 치료적 환경의 발전을 위해 치료적 환경과 치매노인의 관계에 중추적인 역할을 하고 있는 시설의 전문 인력 충원의 원활을 돕는 다양한 방법이 개발되도록 정책마련이 필요할 것이다. 덧붙여 궁극적으로 요양시설의 치료적 환경의 질을 높이기 위해 아직까지 우리 사회에 만연한 요양시설의 부정적 이미지 쇄신에 관한 인식 개선 계획이 이루어져야 할 것이라 본다.|The new problems and issues are surfacing as the population ages due to the increased number of seniors. Increased prevalence of chronic diseases among the older population became evident while Alzheimer gained its title as the ‘disease of the century’ and the concerns for it have been magnified. Moreover, the changes in the social structure and the tradition weakened younger generation’s supports for seniors and it has emphasized the importance and significance of care facilities of the demented elderly. As a result, there is a growing need of study on Therapeutic Environments of Care Facilities of The Demented Elderly. The study examines the relation between therapeutic environment of care facilities and quality of life with the hypothesis; therapeutic environment affects the quality of life for the demented elderly. The purpose of the study is to analyze the relationship between the care facility environment and the quality of life to understand the therapeutic environmental factors including their physical, sptial, and social needs. In the end, the study aims to provide better therapeutic environment of care facilities that goes beyond the minimum legal requirements for people with dementia. Our survey showed the quality of life is significantly associated with the spatial and social environment among the therapeutic environmental variables. These factors positively influenced the life satisfaction of the demented elderly. Further research and stable support for the study field is required to better understand the environmental variables in detail and appropriate therapeutic environment should be developed to pursue higher quality of life for people with dementia. Additionally, it must be understood the study cannot be generalized over the spectrum of dementia diseases but the importance of the study should be given to the discoveries of the relationship between the environment variables to the quality of life. Consequently, the future research should focus on the development of objective measurements for people with dementia. To improve therapeutic environment, there should be new measures to increase the number of professionals who bridges the gap between the therapeutic environment and the people with dementia.
