Abstract
배경 및 목적 불면증은 성인이 흔하게 경험하는 중요한 건강문제로 공중보건의 관점에서 볼 때 불면증과 사회인구학적 변인 및 임상적 요인들과의 연관성을 이해하는 것이 필요하다. 본 연구는 상이한 사회경제학적 인구구조를 지닌 경기도 내 두 지역인 구리시와 양평군의 불면증 유병률을 다양한 변인들과 관련하여 조사함으로서 지역차이 뿐만 아니라 연령, 성별, 소득수준 등의 요인들이 불면증 유병률 상승에 미치는 영향을 확인하고자 하며 신체 및 정신질환이 불면증 유병률과 맺는 관련성을 탐색하고자 한다. 방법 연구는 경기도 내 구리시와 양평군에 거주하는 937명(구리시 333명, 양평군 604)을 대상으로 설문을 통해 이루어졌다. 불면증 여부는 ISI(Insomnia Severity Index)를 통해 확인하였으며 정신질환의 진단을 위해 K-MINI(Korean version of the Mini-International Neuropsychiatric interview)를 사용하였다. 모든 통계분석은 SAS 9.2 프로그램을 이용하였고 인구 가중치를 반영하여 이루어졌으며 불면증 유병률의 사회인구학적 변인에 따른 차이를 검증하기 위해 카이제곱 검증을 실시하였다. 또한 불면증 유병률과 다양한 요인들의 관련성을 지역별로 살펴보기 위해 관련요인의 범주에 따라 다변량 로지스틱 회귀분석을 실시하였다. 결과 전체 응답자의 불면증 유병률은 9.26%였다. 양평군의 불면증 유병률은 11.75%로 구리시 6.87%(p<0.001)에 비해 현저하게 높게 나타나 지역별 차이가 유의하였다. 고령, 여성, 낮은 소득수준, 낮은 교육수준, 민간의료보험 미가입, 미취업군에서 불면증 유병률이 높았고, 신체질환과 정신질환에 이환된 경우 불면증 유병률이 높게 나타났다. 결론 불면증 유병률의 지역차이는 연령과 성별을 포함한 사회인구학적 변인들을 통제한 이후에도 유의하였으며 향후 후속연구를 통해 불면증에 영향을 미치는 지역적 변인들에 대한 탐색이 이루어져야겠다. |Objectives Insomnia is an important issue in health which adults suffer from in common, and it is necessary to understand the correlation of the socio-demographic variable and clinical factors with insomnia in public health. The purpose of this study is to figure out how the socio-demographic factors like not only regional differences but also age, sex, an income level, etc have an effect on increasing the prevalence rate of insomnia, and also to explore the relationship of the physical and mental illness with the prevalence rate by researching the prevalence rates of insomnia in two regions with different socioeconomic structures of considering various variable. Methods A survey was conducted targeting 937 residents(over 40 years of age) of Guri-si(n=333) and Yangpyeong-gun(n=604) in Gyonggi Province of South Korea. The ISI(Insomnia Severity Index) was used to verify whether a person surveyed is insomnia or not. The K-MINI(Korean version of the Mini-International Neuropsychiatric interview) was used to diagnosis whether a person surveyed has mental diseases or not. All statistical analysis was performed using SAS v. 9.2 and all data were weighed. To identify the unique association between socio-demographic characterics and prevalence of insomnia, chi-square test was conducted, and to study the association between the prevalence of insomnia and various factors by region, the multivariate logistic regression analysis was conducted by categories of related factors Results The prevalence rate of insomnia of all the respondents was 9.26 %. The rate of Yangpyeng-gun was 11.75% and it is significantly higher than that of Guri-si which was 6.87% of the respondents(p<0.001). Increased age, female, unmarried and widowed/divorced/separated marital status, less educational level, lower income, none private health insurance, unemployment were risk factors for insomnia. Prevalence rate of insomnia was significantly associated with physical and mental conditions, especially mental disorders, urologic diseases, risk of fall. Conclusions The prevalence rates of insomnia in two regions were still remarkably different even after controlling soico-demographic characteristics including age, sex, education, income level.
