Abstract
본 연구는 전래동화를 활용한 집단미술치료가 경증치매노인의 인지기능에 미치는 영향을 검증하고자 실행되었다. 연구대상은 I시 소재의 치매노인 주간보호소 입소자 28명 중 MMSE-DS의 점수가 11점에서 23점 사이의 경증치매노인들 20명으로, 각각 실험집단과 통제집단에 10명씩 무선배치 했으나 실험집단에서 2명이 건강악화와 개인사유로 회기 진행 도중 퇴소 했고, 통제집단에서 2명이 개인사유로 퇴소하여 최종적으로 실험집단 8명, 통제집단 8명이 본 연구의 대상이 되었다. 연구기간은 2014년 6월 3일부터 2014년 7월 24일까지로 주 2회 매주 화요일과 목요일 오후 1시부터 약 50분간 실시됐다. 전래동화를 활용한 집단미술치료가 경증치매노인의 인지기능에 미치는 영향을 검증하기 위해 본 연구자는 사전-사후에 치매선별검사인 MMSE-DS와 레이복합도형검사를 실험집단과 통제집단에 실시해 그 결과를 양적으로 분석 했다. 또한 실험집단에 한하여 동적 집-나무-사람(K-HTP)검사를 사전과 사후에 실시하였고 그림의 필수 구성부분을 점수화 해 양적 분석과 인지적 관점으로 질적 해석 하였다. 본 연구의 결과를 요약하면 다음과 같다. 첫째, 실험집단의 MMSE-DS의 사전-사후 결과에 대해 대응표본 Wilcoxon검정으로 분석한 결과, 사전 점수의 평균이 16.75점이고, 사후점수의 평균은 22.63점으로 5.88점이 증가하여 통계적으로 유의미한 차이를 보였으며(p<.05), 총 8개의 하위영역 중 지남력, 기억력, 주의집중력, 실행능력의 4개의 하위 영역에서 점수의 유의미한 향상이 있었다. 둘째, 실험집단의 레이복합도형검사의 사전-사후 결과에 대해 대응표본 Wilcoxon검정으로 분석한 결과, 사전점수 평균이 13.44점이고 사후점수 평균은 17.88점으로 4.44점이 증가하였으며, 유의미한 차이가 있는 것으로 나타났다(p<.05) 셋째, 실험집단의 K-HTP 그림검사의 필수 구성부분 묘사 여부를 점수화하여 사전∙사후 결과에 대해 대응표본 Wilcoxon검정으로 분석한 결과 집과 나무 그림에서 유의미한 점수의 상승이 있었으나 전체적으로는 유의미한 차이가 없었다. 이러한 결과로 전래동화를 활용한 집단미술치료가 경증치매노인의 인지기능에 긍정적인 효과를 가져 온다는 결론을 도출해 낼 수 있었으며 전래동화를 활용한 집단미술치료가 전래동화의 익숙함을 통해 미술에 대한 거부감을 줄이고 옛날 우리 문화와 소재에 대한 기억과 회상을 자극시키는 매개체 역할을 함으로써 경증치매노인에게 인지기능 향상에 영향을 주었다는 것을 알 수 있었다.| This study was performed to identify the effect of group art therapy with Korean folk tales on cognitive function in the elderly with mild dementia. Twenty older adults, whose MMSE-DS score ranged between 11-23 points, with early-stage dementia were recruited from among 28 residents at an elderly day-care facility in I city. Ten elderly people were randomly assigned to a control or an experimental group. Two subjects were withdrawn from the study due to health problems or personal circumstances from each group. A total of 8 subjects in each group were included in the study. The study was performed twice a week on Tuesday and Thursday for about 50 minutes per session starting at 1 p.m. between June 3 and July 24, 2014. To verify the effect of group art therapy with Korean folk tales in the elderly with mild dementia, the Mini-Mental Status Examination for screening dementia (MMSE-DS) and Rey Complex Figure Test (RCFT) were performed on both groups before and after the study period, and a quantitative analysis was done to measure differences between pre- and post-test scores. Furthermore, pre- and post-K-HTP tests were only performed on the experimental group, and essential drawing elements were scored using a rating scale. These scores were analyzed quantitatively and interpreted qualitatively from a cognitive perspective. The results of this study are as follows: First, according to the analysis of pre- and post-MMSE-DS test results using the paired sample Wilcoxon test, the mean score increased by 5.88 points from the mean pre-test score of 16.75 to the mean post-test score of 22.63, showing a statistically significant difference (p<.05). Significant improvement was found on four subscales of orientation, memory, attention-concentration and practical ability among 8 subscales. Second, according to the analysis of pre- and post-RCFT results using the paired sample Wilcoxon test, there was an increase by 4.44 points, from the mean pre-test score of 13.44 to the mean post-test score of 17.88. A significant difference was found in scores (p<.05). Third, drawings of K-HTP test were scored using a rating scale, and pre- and post-test scores were analyzed with the paired sample Wilcoxon test. The results showed that there were significant differences in scores of house and tree drawings. However, no significant difference was found in overall scores. The study findings suggest that group art therapy with Korean folk tales has positive effects on cognitive function of the elderly with early-stage dementia. Moreover, this group art therapy reduces a sense of rejection toward arts by applying familiar Korean folk tales and serves as a medium for stimulating memory and recollection of the Korean traditional culture and ideas, thereby leading to improvement in cognitive function of the elderly with mild dementia.; This study was performed to identify the effect of group art therapy with Korean folk tales on cognitive function in the elderly with mild dementia.
