Abstract
ABSTRACT Impact Professionalism on Job Stress among Medical Insurance Review Nurses Yong Suk Joung Dept. of Health Insurance Review The Graduate School of information in Clinical Nursing Hanyang University Directed by Professor Jung A Kim This research is a descriptive correlation study on confirming the professionalism and occupational stress for medical insurance review nurses and finding out the correlation between several variables of occupational stress. The subjects of this study are 130 medical insurance review nurses being in charge of medical insurance review at two coporate hospitals with more than 600 beds and four university hospitals, who agreed about the purpose of this study and permitted their participation in this study and I analyzed the collected datas using self-reporting surveys. Professionalism was estimated by the tool which is Korean standardization version of Professional Inventory Scale(HPl) developed by - 79 - Hall(1968), corrected by Snizeck(1972), standardizated by Paek Hee-jung and Kim-Godwin(2007). Occupational stress was estimated by the contracted form of Korean Occupational Stress Scale(KOSS) which was developed by Jang Se-jin etc(2005) and confirmed its reliability and validity. The collected datas were analyzed by the program SPSS WIN 21.0: technological statistics, t-tests, ANOVA, Scheffe test, Pearson's Correlation Coefficient, Cronbach's alpha coefficient. The main result of this study are as in the following: 1. As a general characteristic of the subjects, age of 41-50 was the highest rate, 48.3% and about marriage, the married was 81.4%, much higher than single people. About last school, 3-year course nursing college graduates was the highest as 41.65%, and about the scale of working organization, hospital with more than 1000 beds was the highest as 53.4%. 2. About the job characteristic of the subjects, 6-10 year was the highest as 40.7% in clinical career and 10-20 years was the highest as 43.2% in medical insurance review career. As the motivation about being a medical insurance review nurse, the largest percent, 39.0% responded 'new field of nursing'. 3. The total score of medical insurance review nurses in professionalism appeared to be 80.75±9.59. In low-scored field, 'Job - 80 - satisfaction' was the highest as 17.44±2.93 and 'reference of professional organization' and 'Faith in self-regulation' was the lowest recording 16.11±2.79 and 16.11±2.78. The occupational stress of medical insurance review nurses appeared to be 35.67±7.83, In low-scored fields, 'Job demand' was the highest as 54.93±11.38 and 'Interpersonal conflicts' was the lowest as 28.97±9.69. 4. As the result of verifying the difference between professionalism depending on general characteristic and job-related characteristic, there was significant difference in sick-bed-scale(F=6.37, P=.002), clinical career(F=3.32, P=.040), workload(F=3.83, P=.024). Professionalism was significantly higher in the case of working in 800-1000 bed-scale hospital than in 600-800 bed-scale hospital and in the case of 1-5-year clinical career than 6-10-year clinical career. 5. As a result of verifying the difference between occupational stress depending on general characteristic and job-related characteristic, it turned out that there was significant difference in sick-bed-scale(F=4.73, P=.011) and clinical career(F=4.23, P=.017), in occupational career. It turned out that occupational stress was significantly higher in the case of working in 800-1000 bed-scale hospital than in 600-800 bed-scale hospital and in the case of over-11-year clinical career than 1-5-year clinical career. - 81 - 6. It turned out that there was a negative correlation between the professionalism and occupational stress of medical insurance review nurses. Professionalism had a significant negative correlation with all sub-fields of occupational stress. Among the sub-fields of professionalism, 'job satisfaction' and 'reference of professional organization' had a significant negative correlation of r=-162 ~ .390 with all sub-fields of occupational stress. As a conclusion, considering that as professionalism of medical insurance review nurses increases, occupational stress decreases, the improvement of the professionalism of medical insurance review nurses would reduce occupational stress and as a result, it would lead to excellent work performance and improved systemecity. Considering that professionalism is formed and reinforced by systemic support of the organizations to which individuals belong(Ko Sun-hee, Jung Bok-rye, 2014: Min A-ri, 2012: Cha In-ja, 2008, Wynd, 2003), systemic and efficient education system of organizations should be seeked for the improvement of professionalism.
