Abstract
소득 불평등은 개인의 건강수준에 큰 영향을 미치며, 결핵 발병률과 유행에 중요한 요인인 것으로 밝혀졌다. 미디어 캠페인은 건강에 대한 지식 불균형을 해소하고, 건강행동에 긍정적인 영향을 미쳐 사회경제적 격차를 감소시키는 것으로 밝혀졌지만, 사회경제적 지위(SES)가 낮은 사람들의 건강행동을 촉진하는데 효과가 적은 것으로 나타났다. 본 연구에서는 전체 인구 집단을 소득수준에 따라 60대 저소득층과 60대 고소득층, 20-50대 저소득층, 20-50대 고소득층으로 구분하고, 결핵예방 미디어 캠페인이 결핵에 대한 지식수준과 결핵에 대한 건강신념(지각된 개연성, 지각된 심각성), 결핵검진 행동의도에 어떠한 영향을 미쳤는지 통제변인을 통해 검증하였다. 질병관리본부의 결핵예방 미디어 캠페인 평가 2차 자료(2012년~2015까지)를 활용하였으며, 분석 대상자는 총 4,234명으로 다중회귀분석을 실시하여 결핵예방 미디어 캠페인 효과를 검증하였다. 연구 결과, 60대 저소득층의 결핵에 대한 지식수준에 영향을 미치는 변인은 없었으며, 지각된 개연성에는 결혼여부와 2014년, 2015년 캠페인이 정(+)적인 영향을 미쳤다. 또한, 지각된 심각성과 결핵검진 행동의도에는 유의수준은 낮았으나, 결혼여부가 영향을 미친 것으로 나타났다(p<.1). 60대 고소득층의 경우에는 결핵에 대한 지식수준과 검진행동의도에 영향을 미치는 변인이 없었으며, 지각된 개연성에는 2014년과 2015년 캠페인이, 지각된 심각성에는 결혼여부가 영향을 미쳤다. 캠페인 노출여부는 60대 저소득층과 60대 고소득층에는 유의미한 영향을 미치지 않았다. 20-50대 저소득층에는 결핵에 대한 지식수준과 지각된 개연성에 정적(+)인 영향을 미쳤으며, 20-50대 고소득층에는 결핵에 대한 지식수준과 지각된 심각성, 결핵검진 행동의도에 긍정적인 영향을 미친 것으로 나타났다. 따라서 결핵예방 미디어 캠페인은 TV 총 시청률이 높은 60대 저소득층보다는 20-50대 고소득층에 더 효과적인 것으로 나타났다. 이것은 60대와 저소득층은 미디어 캠페인 노출에 있어 전략적인 접근이 필요하다는 것을 의미한다. 향후 노인층과 저소득층의 결핵 유병률과 건강 불평등을 줄이기 위한 새로운 캠페인 전략과 정책이 보완되어야 한다.; Income inequality greatly affects an individual's health level and has been found to be an important factor in the incidence and prevalence of tuberculosis. Media campaign has been found to address the knowledge imbalance about health and reduce socioeconomic gaps by having a positive impact on health behavior, but has been found to be less effective in promoting health behaviors of people with low socioeconomic status (SES). In this study, the entire population was divided by 4 groups according to their income levels, 60s low-income and high-income groups and 20-50s low-income and high income groups. Control variable was used to verification how the TB prevention media campaign affected the knowledge level of TB and the health belief(perceived susceptibility, perceived severity) and TB screening behavior intention. The secondary data on the assessment of the TB prevention media campaigns by the KCDC(from 2012 to 2015) were utilized, and total 4,234 participants were analyzed for multiple regression analysis to verify the effectiveness of the TB prevention media campaign. In result have shown that there were no variables that affected the level of knowledge about tuberculosis among low-income people in their 60s. And the perceived susceptibility of marital status and the 2014 and 2015 campaigns had a positive impact. In addition, it was shown that marital status affected the perceived severity and the level of TB-screening behaviour intention, although significant levels were low (p<.1). There were no variables that influenced the knowledge level of tuberculosis and screening behaviors intention of 60s high-income brackets. The perceived susceptibility was influenced by campaigns of 2014 and 2015, and marital status affected perceived severity. The Media campaign exposure status not affect low-income people in their 60s and high-income people in their 60s. Low-income group in their 20s and 50s had a positive(+) effect on the level of knowledge about tuberculosis and perceived susceptibility while high-income group in their 20s and 50s had a positive effect on the level of knowledge about tuberculosis, perceived severity, and TB screening behavior intention. Thus, the TB prevention media campaign was more influential for high-income group in their 20s to 50s, rather than low-income group in their 60s, who have a high total TV viewing rate. These results means that in their 60s and low-income groups need a strategic approach to media campaigns exposure. Future campaign strategies and policies should be complemented to reduce tuberculosis prevalence and health inequality in the elderly and low-income groups.
