Abstract
질편모충(Trichomonas vaginalis, TV)은 성병(non-viral sexually transmitted disease)을 일으키는 가장 흔한 원충으로 남성에서는 거의 무증상으로 진단 및 치료가 되지 않아 만성감염을 일으킬 수 있다. 질편모충은 양성전립선비대, 전 립선암 환자의 전립선 조직과 전립선염 환자의 소변에서 발견되었다. 비만과 같은 대사장애는 지방조직에 chronic low-grade inflammation을 유발하며, 지방 세포에서 분비되는 아디포카인은 내분비계 뿐만아니라 면역반응에도 영향을 미친다고 보고되었다. 또한 비만에 의한 염증상태는 종양을 유발하는 인자로서 전립선암 등 여러 종류의 암과 관련되어 연구되고 있다. 그러나 비만과 전립선비대증의 상관관계를 본 실험적 연구는 드물다. 렙틴(leptin)은 아디포카인의 일종으로 식품섭취와 체중조절의 핵심물질로 혈청렙틴수준은 전립선의 증식과 관련이 있는 것으로 알려져 있으며, 전립선비대증의 고 위험인자로 보고되었다. 그러나 렙틴이 양성전립선비대증의 발달에 관여할 수 있는지에 대한 실험적 연구는 없다. 이 연구에서는 질편모충에 감염된 전립선 세포의 증식에 있어서 지방세포에서 분비된 렙틴의 역할을 알아보고자, 질편모충에 의한 전립선비대상피세포의 염증반응이 지방세포에서 렙틴의 분비를 증가시키는지 알아보고 렙틴이 전립선세포(전립선기질세포, 전립선비대상피세포)의 증식을 유도하는지 알아보았다. 양성전립선비대상피세포주(BPH-1), 전립선기질세포주(WPMY-1) 및 지방전구 세포주(3T3-L1)를 사용하였으며, 질편모충은 T016 분리주를 사용하였다. 전립선상피세포주에 질편모충을 반응시킨 TV-conditioned medium(TCM)에는 IL-6, CXCL8, CCL2, IL-1β가 포함되어 있으며, TCM은 지방전구세포의 이동(chemotaxis)을 증가시켰다. 성숙한 지방세포를 TCM과 반응시켰을 때 adipokine인 CXCL1, IL-6, CCL2와 렙틴의 생산이 증가하였다. 지방세포에 TCM을 넣어 만든 adipocyte-TCM(ATCM)을 전립선기질세포주(WPMY-1) 및 전립선비대상피세포주(BPH-1)와 반응시켰을 때 두 세포주 모두에서 증식이 유의하게 증가하였고, 렙틴수용체(OBR)의 발현이 증가하였다. 또한 ATCM에 의해 유도된 증식에서 렙틴신호분자들의 발현을 확인하였을 때 JAK2/STAT3, Notch1, Jagged1, survivin의 발현이 유의하게 증가되었다. ATCM으로 유도된 증식이 렙틴에 의한 것인지 확인하기 위해 ATCM 처리 시 렙틴수용체에 대한 항체를 함께 추가하였을 때 전립선세포주들의 증식은 감소하고 렙틴신호분자 들의 발현도 함께 줄어들었다. 따라서 지방세포에서 분비된 렙틴은 전립선세포의 증식에 관여할 수 있다는 것을 알 수 있었다. 요약하자면 질편모충에 감염된 전립선비대상피세포에서 분비된 염증물질은 지방세포의 이동 및 활성화를 일으켜서 아디포카인을 생산하는데, 렙틴은 전립선기질세포와 전립선비대상피세포의 렙틴수용체와 결합하여 렙틴신호분자들의 발현을 통하여 전립선세포의 증식을 유도하였다. 이 연구결과는 질편모충감염에 의한 전립선세포의 염증반응은 지방세포의 렙틴 생산을 증가시켜 전립선세포의 증식을 일으킬 수 있음을 제시한다.; Trichomonas vaginalis (TV), a common sexually transmitted disease, is a protozoan parasite. The majority of infections are asymptomatic in men and remain undiagnosed and untreated, which has been hypothesized to result in persistent prostatic infection. TV has been observed in the prostate tissue of prostate cancer, and in the urine of patients with prostatitis as well as in the prostate tissue of benign prostatic hyperplasia (BPH). Obesity induces chronic low-grade inflammation and increases the risk of BPH. Adipocytes influence not only on the endocrine system but also on the immune response through several cytokine-like mediators known as adipokines including leptin. Leptin is a kind of adipokines that was originally identified as a key molecule in the regulation of food intake and body weight. Abdominal obesity and high serum leptin level are known to associate with prostate growth and reported as high-risk factors for clinical BPH. However, it is not known that leptin could induce proliferation of prostate cells. In this study, we investigated whether adipocyte reacted with the culture supernatant of BPH epithelial cells infected with TV may induce the proliferation of prostate cells via leptin signaling pathway. To investigate the effect of crosstalk between adipocyte leptin and inflamed epithelial cell in proliferation of prostate cells, adipocytes (3T3-L1) were incubated in conditioned medium of BPH epithelial cells infected with TV (TCM) and then the adipocyte-conditioned medium (ATCM) was identified to cause proliferation of prostate cells. To measure the proliferation of prostate stromal and BPH epithelial cells, the BrdU assay and wound healing assay were used. ELISAs, RT-PCR, Q-PCR, western blotting and immunofluorescence assay were used to measure the production and expression of leptin signaling molecules. BPH epithelial cells incubated with live TV released pro-inflammatory cytokines such as IL-6, CXCL8, CCL2 and IL-1β, and induced the migration of pre-adipocytes. When the TCM was added to mature adipocytes, they produced adipokines such as CXCL1, IL-6, CCL2 and leptin. Prostate stromal cell and BPH epithelial cell incubated with the ATCM or recombinant leptin showed increased proliferation and expression of leptin receptor (OB-R). Also, leptin signaling molecules including p-JAK2, p-STAT3, Notch1, NICD, Jagged1, survivin showed increased expression in the prostate cells incubated with ATCM or recombinant leptin. However, blocking of the leptin receptor reduced the proliferation and the expression of leptin signaling molecules in the prostate cells. Therefore, leptin from adipocytes may be involved in proliferation of prostate cells. In summary, the inflammatory mediators released by BPH epithelial cells in response to infection with TV induced the migration and activation of adipocytes. The activated adipocytes induce the proliferation of prostate cells via leptin signal pathway. Our findings showed that BPH epithelial cells infected by live TV induce the proliferation of prostate cells through the leptin signaling. Therefore, it is proposed that adipocyte leptin may contribute to the enlargement of BPH caused by TV infection.
