TY - THES AU - 이수연 DA - 2019. 8 PY - 2019 UR - https://repository.hanyang.ac.kr/handle/20.500.11754/109411 UR - http://hanyang.dcollection.net/common/orgView/200000436080 AB - 배경 및 목적: 침 치료는 대중적인 관심과 인기에도 불구하고, 통증 등 만성 질환 관리에 효과적인 치료법임을 보여주는 확고한 증거는 부족하다. 이 연구의 목적은 만성 질환자들의 침 치료 이용률과 그 이용에 미치는 영향요인을 결정하는 데 있다. 연구 방법: 이 연구는 서울·경기 지역에 소재하고 있는 병원 및 기관에서 만성 통증에 대한 치료를 받은 400명의 환자 데이터를 구조화된 설문지를 이용하여 수집했다. 이 연구에 사용된 설문지는 자기기입방식으로, 설문항목은 총 52항목으로 사회 인구학적 특성 및 질병관련 특성, 침 지식수준, 침 치료 태도 및 이용특성 등을 구성하였다. 이 연구를 위하여 수집된 자료는 SPSS Statistics 24.0 Network Version프로그램을 이용하여 분석되었다. 사용된 분석기법은 빈도분석과 기술통계, Chi-square test, t-test이었고, 이용에 미치는 영향요인 결정은 로지스틱 회귀분석을 통해 시행되었다. 연구 결과: 대상자들의 침 치료 이용률은 44.3%였다. 이용자들의 침 치료 이용 이유 중 ‘치료효과 증대’에 44.5%에 가장 높은 응답률을 보였다. 침 치료 이용률이 가장 높은 질병군은 신경계질환 군(60.5%)이었고, 대부분이 ‘한의원’(82.9%(136명)에서 이용하였다. 침 치료에 대한 정보는 주로 가족이나 지인(71.6%)으로부터 구하였다. 침에 대한 정확한 지식수준율은 65.4%였고, 침 이용군의 지식수준이 높았다(p<0.05). 대상자들의 침 이용 예측요인으로는 연령(OR=2.676, p<0.01), 월수입(OR=1.592, p=0.05), 질병의 수면영향(OR=1.821, p>0.05), 질병심각도(OR=1.825, p>0.05)와 침 지식수준(OR=2.491, p<0.05), 질병 관리 방법 선호(OR=3.003, p<0.01) 등이었다. 결론 및 제언: 이 연구는 만성질환자들의 건강증진 및 질환치료를 목적으로 침 치료를 이용하는 데 있어 침의 지식수준과 침의 이용률에 대한 연관성을 규명하고자 연구가 수행되었다. 만성질환자들의 침 치료 이용은 10명 중 약 5명이 이용하였으며, 이들 침 이용자들의 침의 효과와 효능에 대한 정확한 지식수준은 낮았다. 침에 대한 정보원 대부분은 비 의료인이었으며, 의사와의 이용에 대한 상담률도 10명 중 약 4명 정도만 상의한 것으로 조사되었다. 이러한 결과로 미루어 현재 우리나라는 침의 이용에 있어 침에 대한 구체적 지식이나 침의 효용성, 적응증에 대한 특별한 이해 없이 침의 무분별한 오남용 및 과잉치료가 우려되고 있어, 이에 대한 대처 방안이 요구되어짐을 시사한다. ※핵심단어 : 침 치료, 만성질환, 보완대체의료, 지식 ※Key word : acupuncture, perception, knowledge, chronic disease, Complementary and Alternative Medicine; Background and Objectives: Although acupuncture treatment has been steadily gaining public interest and popularity, the evidence of its effect on treating a chronic condition is inconclusive. The aim of this study is to examine the pattern of acupuncture use among chronic disease patients and to determine factors associated with the use of acupuncture. Methods: A cross-sectional survey was conducted in 2018 with patients receiving treatment and care for chronic diseases in four health care facilities in Seoul and Gyeonggi provinces. To assess the use of acupuncture treatment, we used self-administered questionnaire consisted of 52 questions on the socio-demographic and disease-related characteristics of respondents, and the level of knowledge, attitudes, and its utilization pattern. Using the SPSS Statistics 24.0 Network Version program, a logistic regression analysis was performed to identify factors associated with the use of acupuncture treatment. Results: Among 400 respondents, 44.3% of respondents have used acupuncture treatment, and the most popular reason for acupuncture use was to increase the effectiveness of current treatment (44.5%). The patients with neurological disease had the highest utilisation rate of 60.5%. The main source of information for acupuncture use was “family and friends” (71.6%). The average score for the level of knowledge on acupuncture treatment among the respondents was 65.4%. Moreover, the knowledge level on the effects and efficacy of acupuncture treatment was low. Disclosure of the use of acupuncture treatment to medical professional was 40%. Potential predictors of acupuncture use included the age (OR=2.676, p<0.01), monthly household income (OR=1.592, p=0.05), and the level of knowledge on acupuncture treatment (OR=2.491, p<0.05), and preferred disease management methods (OR=3.003, p<0.01). Discussion and Conclusion: The finding of this study show that the use of acupuncture treatment was common. The patients characteristics and their perception on their health and disease association with thie use of acupuncture treatment.
