dc.description.abstract오늘날 비침습적산전검사(Non-Invasive Prenatal Testing; NIPT)는 대규모 병렬염기서열분석(Massively Parallel Sequencing; MPS)이 적용되어 태아의 염색체 이수성 진단에 널리 사용되고 있다. 모체혈장 내 태아의 DNA 비율인 Fetal Fraction(FF)은 비침습적산전검사의 정확성을 좌우하는 중요 요소이므로, FF를 보다 정확하게 측정하기 위한 다양한 방법들이 개발되어 적용되고 있다. Y 염색체 기반의 FF 추정방법은 정확하지만 남자태아에게만 수행할 수 있으며, SNP 기반의 FF 추정방법인 FetalQuantSD는 모든 성별에 사용 가능하지만 SNP 유전형검사에 추가 비용이 필요하다. 반면에 통계모델 기반 FF 추정방법인 SeqFF는 대량의 데이터와 다변량회귀모형을 이용하여 계산하는 방법으로, 추가비용 없이 모든 성별 태아의 FF를 추정할 수 있는 장점이 있지만, 낮은 FF의 경우에는 부정확한 추정결과가 나올 수 있다는 단점이 있다. 본 연구에서는 약 2,100명의 임신부 혈액시료에서 생산된 기존 자료를 사용하여 대표적인 Y 염색체 기반, SNP 기반, 통계모델 기반의 FF 추정방법들의 결과를 서로 비교해 보았다. Y 염색체 기반 FF 추정방법(YFF)은 2,058명의 남자태아를 임신한 모체의 혈액시료에서 생산된 기존 자료를 사용하여 각 태아의 Y 염색체에 해당하는 DNA 비율을 계산하였다. SNP 기반 FF 추정방법(SFF)은 90명(남녀 각 45명 태아 임신부)의 모체 혈액시료에서 생산된 기존 자료를 사용하여 SNP 유전변이를 검출한 후, 태아의 SNP 유전변이 비율을 구하여, 태아의 DNA 비율을 측정하였다. 통계모델 기반 FF 추정방법은 SeqFF 논문에 제시된 사전학습모델을 그대로 사용하여 각 구간 내 리드(read)의 개수를 세어 태아의 DNA 비율을 계산하였다. 위의 세 가지 방법으로 계산된 FF들은 상관분석(correlation analysis)을 통해 서로 비교하였다. SNP 기반 FF 추정방법(SFF)과 Y 염색체 기반 FF 추정방법(YFF)의 상관관계는 남자태아 시료인 45명 대상에서만 비교가 가능하였고, r=0.929인 높은 상관관계를 보였다. SNP 기반 FF 추정방법(SFF)과 SeqFF FF 결과의 상관관계는 남자태아 시료인 45명 대상인 경우 r=0.800, 90명의 남녀태아 전체 대상인 경우는 r=0.813을 보였다. Y 염색체 기반 FF 추정방법(YFF)과 SeqFF FF 결과의 상관관계는 r=0.798(남자태아 45명 대상)과 r=0.832(남자태아 2000명 대상)을 보였다. 결론적으로 통계모델 기반의 SeqFF 방법은 염기서열분석자료 만을 가지고 모든 성별 태아 검사에 적용할 수 있다는 장점은 있으나, 일반적으로 Y 염색체 기반 및 SNP 기반 방법에 비해서는 상대적으로 정확도가 떨어지며, 특히 5% 이하의 FF을 추정하는 경우 현저하게 정확도가 떨어질 수 있음을 알 수 있었다. 따라서 높은 정확성의 장점을 가진 Y 염색체 및 SNP 기반의 FF 측정의 더 많은 실험자료를 수집하여, SeqFF와 같이 저비용으로 태아 성별에 무관하게 적용할 수 있는 보다 더 정확한 FF 추정방법의 개발이 비침습적산전검사의 임상적 유용성을 증가시킬 것으로 생각된다.; Today, non-invasive prenatal testing (NIPT) by massively parallel sequencing (MPS) is widely used in detecting fetal chromosomal aberrations. Since fetal fraction (FF) of maternal plasma is a key factor affecting the accuracy in NIPT analysis, various methods have been developed and applied to measure FF more accurately. Y chromosome-based FF estimation is accurate, but can only be performed on male fetuses. A SNP-based FF estimation (i.e. FetalQuantSD) can be applied to both male and female fetuses, but requires additional cost for SNP genotyping. A statistical model based FF estimation (i.e SeqFF) is applicable to both male and female fetuses without any additional cost. SeqFF uses a multivariate regression model based on a large amount of data, and it has a disadvantage of providing inaccurate estimation when FF is low. In this study, the estimated FFs by the representative three methods mentioned above were compared. The total data set used for the three methods was produced from approximately 2,100 pregnant maternal samples in the previous studies, but in this study the same subset of data was used for each pair of FF comparison. Y chromosome-based FF estimation(YFF) was performed with 2,058 male fetus pregnant sample data, and the DNA proportion of each fetal Y chromosome was calculated. SNP-based FF estimation (SFF) was done with previously generated SNP genotyping and sequencing data in 90 fetus pregnant samples (male 45 and female 45 each). The fetal DNA proportion was measured by calculating the SNP genetic variation ratio of each fetus. Statistical model based FF estimation (SeqFF FF) was done with the published pretrained model which was based on the regional block based read counts. The FF estimation results by the three methods were compared with correlation analysis. The correlation analysis between SFF and YFF was done for 45 male fetus pregnant sample data, and shows high correlation (r=0.929). The correlation between SFF and SeqFF FF shows r=0.800 for 45 male fetus pregnant sample data, and r=0.813 for 2000 male fetus pregnant sample data. The correlation between YFF and SeqFF FF shows r=0.798 for 45 male fetus pregnant sample data, and r=0.832 for 2000 male fetus pregnant sample data. Generally, in spite of its advantage in clinical application, SeqFF estimation is relatively less accurate than that of the Y chromosome method (YFF) or SNP method (SFF), and cannot be applicable in case of FF less than 5% because of its very low correlation. Therefore, collection of large amount of data from high accuracy methods like Y chromosome-based and SNP-based FF estimation may promote development of more accurate gender independent and low cost FF estimation methods. Thereby, new methods may increase clinical usefulness of NIPT even in case of low FF cases.-
