- 엡스타인-바바이러스 (EBV) 감염 종양 세포는 대식 세포 같은 주위 염증 세포와 상호작용을 하여 시토카인 환경 (Cytokine milieu)을 형성하고, 이는 결절외 NK/T 세포 림프종에서 중요한 역할을 담당한다. 본 연구를 통해 결절외 NK/T 세포 림프종에서 적혈구포식성 림프조직구증식증 및 예후와 연관있는 시토카인을 밝혀내고자 한다.
2008년 9월부터 2015년 12월 사이 단일 기관에서 결절외 NK/T 세포 림프종으로 진단된 환자 114명을 대상으로 치료 전 혈청 인터루킨-18 (IL-18), 인터페론-감마 (IFN-γ), 인터페론감마 유도 단백-10 (IP-10), 종양괴사인자-알파 (TNF-α)를 측정하였다.
환자들은 병기에 따라 동시화학방사선조사 또는 비안트라사이클린계 항암화학요법으로 치료받았다. 병기 I 또는 II 환자는 72명 (63%), 병기 III 또는 IV 환자는 42명 (37%) 였다. IL-18, IP-10, 그리고 TNF-α의 중앙값은 다음과 같다: IL-18 (20.5pg/mL), IP-10 (21.4pg/m) and TNF-α (6.9pg/mL). IL-18, IP-10, 그리고 TNF-α의 중앙값을 기준으로 각각 높은 군과 낮은 군으로 환자를 분류하였다. 이들 수치가 높은 군의 환자들은 그렇지 않은 환자 군에 비해서 전체 생존률 (OS) 및 무진행생존률 (PFS) 이 열등하였다. 특히, IL-18로 구분한 환자군에서 OS 및 PFS의 차이는 병기 III 또는 IV 환자군에 비해 병기 I 또는 II 환자군에서 두드러졌다.
본 연구에서는 골수 흡인 또는 골수 생검에서 병리학자에 의해 확인된 적혈구포식성 대식세포가 존재할 경우 적혈구포식성 림프조직구증식증라고 정의하였다. 11명의 적혈구포식성 림프조직구증식증 환자가 있었고 이들은 모두 혈청 IL-18이 중앙값보다 증가되어 있었다. 이들 환자에서의 전체 생존률 및 무진행생존률은 불량하였다 (OS, 3.8 months; PFS, 3.1 months).
본 연구를 통하여 결절외 NK/T 세포 림프종 환자에서 IL-18가 예후 예측 인자로서의 가치가 있음을 밝혀내었고 또한 적혈구포식성 림프조직구증식증 및 염증성 시토카인들과 연관이 있음을 확인하였다. 진단 시 IL-18 측정은 특히 병기 I 또는 II 환자에서 더욱 유용할 것이다.; Cytokine milieu is important for extranodal NK/T-cell lymphoma (ENKTL) because
EBV-infected tumor cells could interact with surrounding inflammatory cells such as
macrophages. In this study, we evaluated the prognostic value of cytokines related with
hemophagocytosis including serum IL-18 in ENKTL.
We measured pre-treatment serum levels of IL-18, interferon-gamma (IFN-γ), interferon-
γ-inducible protein-10 (IP-10) and tumor necrosis factor-α (TNF-α) in 114 patients with
ENKTL who were enrolled into the prospective cohort study between September 2008 and
December 2015 at single Center.
Patients were treated with non-anthracycline-based chemotherapy or concurrent
chemoradiotherapy according to the stage. The stage I/II and III/IV were 72 (63%) and 42
(37%), respectively. The median values of IL-18, IP-10 and TNF-α were as follows: IL-18
(20.5pg/mL), IP-10 (21.4pg/m) and TNF-α (6.9pg/mL). Patients were classified into high
and low group with IL-18, IP-10 and TNF-α according to the median value. Accordingly,
patients with elevated level of serum IL-18, IFN-γ, IP-10 and TNF-α had inferior overall
survival (OS) and progression-free survival (PFS) to patients without them. Furthermore,
this association of serum IL-18 with OS and PFS was more significant in stage I/II patients
than stage III/IV.
We defined hemophagocytic lymphohistocytosis (HLH) as macrophages showing active
hemophagocytosis were observed in bone marrow aspirates and/or biopsy by pathologists.
There were eleven patients with HLH and all of them were included in the group of higher
serum IL-18 level than median value. They were associated with both worse OS and PFS
(3.8 and 3.1 months, respectively).
Our study demonstrated the prognostic value of serum IL-18 in ENKTL as well as its
association with HLH and other inflammatory cytokines. Especially, serum IL-18
measurement at diagnosis might be useful for predicting the prognosis for stage I/II patients.
