dc.description.abstractBackground: The mechanisms underlying the effects of growth hormone(GH) on fat metabolism in GH-deficient (GHD) patients are not fully understood. This study was to evaluate the effects of GH replacement before the body fat changes and their relevant parameters in GHD patients.Methods: In total, 14 (5 males and 9 females) GHD adults (mean 34.4 ± 8.2 yr, BMI 23.1 ± 1.0 kg/m2) received GH treatment for 12 weeks. Body composition by dual-energy X-ray absorptiometry and fasting serum analyses were assessed before and after treatment.Results: GH replacement did not affect body weight (60.9 ± 3.0, vs. 60.3 ± 3.3 kg, P = 0.30), body fat mass (19.7 ±1.5, vs. 18.4 ± 1.8 kg, P = 0.10) or HOMA IR (1.65 ± 0.3,vs. 1.94 ± 0.4, P = 0.34). However, serum adiponectin and leptin levels were distinctly reduced after GH administration (6.86 ± 1.2, vs. 5.51 ± 0.8 mg/L, P = 0.03; 8.7 ± 1.1, vs. 6.5± 1.0 ng/mL, P < 0.01, respectively). Interestingly, correlations between body fat mass and adipokines were stronger (adiponectin, before r = -0.502, P = 0.07 vs. after r =-0.634, P = 0.02; leptin, before r = 0.378, P = 0.18 vs. after r = 0.711, P = 0.004) after GH administration.Conclusion: After 12 weeks-GH replacement, the reductions in serum adiponectin and leptin levels without significant body fat changes and the stronger correlations between body fat mass and these adipokines are reflective of the metabolic effects of GH probably due to the amelioration of compensatory mechanism of adiponectin like GHRH to overcome GH deficiency and leptin resistance in GHD patients.연구배경: 성장호르몬의 체지방 대사 기전에 대해서 아직완전히 규명되고 있지 않다. 본 연구는 성장호르몬 결핍 환자에서 성장호르몬 보충 후 체지방 변화 이전의 효과에 대해 알아보고자 한다.방법: 총 14명(5 남성, 9 여성)의 성장호르몬 결핍 환자(평균 34.4 ¡± 8.2세, BMI 23.1 ¡± 1.0 kg/m2)를 대상으로 12주간성장호르몬 치료를 시행하였다. DEXA (dual-energy X-ray absorptiometry)를 이용한 체지방 측정과 공복 혈장 분석을 치료 전후로 시행하였다.결과: 12주간의 성장호르몬 치료는 체중(60.9 ¡± 3.0, vs.60.3 ¡± 3.3 kg, P = 0.30) 및 체지방(19.7 ¡± 1.5, vs. 18.4 ¡±1.8 kg, P = 0.10), HOMA IR (1.65 ¡± 0.3, vs. 1.94 ¡± 0.4,P = 0.34)에¡ 영향을 미치지 못하였으나, 혈액의 아디포넥틴과렙틴은 의미있게 감소하였다(adiponectin 6.86 ¡± 1.2, vs. 5.51¡± 0.8 mg/L, P = 0.03; leptin 8.7 ¡± 1.1, vs. 6.5 ¡± 1.0ng/mL, P < 0.01). 흥미롭게, 성장호르몬 치료 이후 체지방량과 아디포카인간의 상관관계가 더 강해졌다(adiponectin, 전 r = -0.502, P = 0.07, 후 r= -0.634, P = 0.02; leptin, 전 r = 0.378, P = 0.18, 후 r = 0.711, P = 0.004)결론: 의미있는 체지방 변화 이전, 아디포넥틴과 렙틴의 현저한 감소와 체지방량과 아디포카인간의 강한 상관관계는, 성장호르몬결핍환자에서 성장호르몬 부족을 극복하기 위한 성장호르몬자극호르몬(GHRH) 상승과 비슷하게 아디포넥틴의 보상적 상승의 약화와, 렙틴 저항성을 극복하기 위한 렙틴 상승의 해소를 야기하는 성장호르몬의 보충 효과로 사료된다.en_US
