정신병원 정신건강의학과의 중장기적 발전방향 조사를 위한 연구

Abstract

비록 지역사회모델, 전문화모델, 확대모델이 실제로 현실에 적용되기 위해서는 보다 구체화되어야 하고 이를 위해 요구되는 부분에서 차이가 있으나 모두 정신병원 정신건강의학과의 중장기적인 발전방향으로 고려될 수 있을 것 같다. 지역사회모델은 당위적으로 옳고 현실적으로도 반드시 필요한모델로서 이를 실현하고 꾸준히 발전시키기 위해서는 지역사회와의 긴밀한 연계를 통해 공식적인 지역사회 정신건강서비스에 대한 다양한 프로그램을 개발하고 제공함으로써치료의 연속성을 확보하는 전략이 유효할 것으로 생각된다. 전문화모델은 중독정신의학과 노인정신의학에서 특히 효과적일 것으로 보이고, 특히 대형정신병원의 경우 교육 및 수련을 포함한 보다 다양한 역할을 수행할 필요가 있을 것으로 보이며, 이 모델은 일정한 조건만 갖춰진다면 가장 빨리 현실에적용되고 발전할 수 있는 장점이 있을 것으로 생각된다. 확대모델의 경우 본 연구에서 정의했던 것보다 더 세분하고 구체적이 될 수 있도록 개념을 재정립할 필요가 있을 것으로 보이나, 이 분야에 적절히 준비된 사람들은 이 모델을 중장기적인 발전방향으로 충분히 고려해 볼 수 있을 것이며, 향후 이모델을 중요한 발전의 축으로 삼기 위해서는 전문의를 양성하는 과정에서 생물학적 치료에 대한 강조뿐만 아니라 이모델에 적합한 프로그램을 교육하고 수련하는 데에도 더 큰관심을 기울일 필요가 있을 것 같다. 본 연구를 토대로 정신병원의 중장기적 발전발향을 확립하기 위한 활발한 논의의시작이 될 수 있기를 기대한다. Objectives: The aim of this study was to investigate medium- and long-term developmental directionsof mental hospitals. Methods: The preliminary investigation is composed of unstructured interviews with 10 psychiatrists,literature and case investigations, and development of models for medium- and longtermdevelopmental model. The qualitative investigation consisted of constructed interviews withseven professionals. The quantitative investigation was conducted using questionnaires, whichwere sent to members of the Korean Neuropsychiatric Association, etc. Data were analyzed usingfrequency analysis. Results: Through the preliminary investigation we were able to make three models : communitymodel emphasizing linkage between mental hospitals and community, specialization modelstressing specializations of mental hospitals, and extension model highlighting extensions of theroles of the mental hospital. In the qualitative investigation, two of the seven professionals gave aspecial preference for the community model, one for the specialization model, and two for theextension model. In the quantitative investigation, the community model and the specializationmodel were the most favorite models, and the extension model was the least favorite model. Conclusion: Three developmental models could be used in planning of the medium- and longtermdevelopmental directions of mental hospitals, although each model needs to be embodiedin order to work in practice.
