Abstract
As job stress was known to be associated with various diseases and psychiatric conditions, we aimed to estimate the job stress-attributable burden of disease in Korea based on the concept of disability-adjusted life years (DALY). We selected ischemic heart disease, stroke, major depressive disorder, and suicide as health outcomes from job stress, because of the easy accessibility to data estimating burdens and of their important meaning in Korean occupational background. Approximately 21% of Korean workers were exposed to high job strain, which was attributable for approximately 6.7% of ischemic heart diseases, 6.9% of strokes, 13.6% of major depressive disorders, and 4% of suicides. In terms of job stress-attributable DALY, the burdens of disease per 100,000 people were 38 for ischemic heart disease, 72 for stroke, 168 for major depressive disorders, and 44 for suicides. The present study would be useful to improve workers’ health in that the findings can be used as a reference data for establishing policies to manage the job stress related diseases.; 직무 스트레스는 정신질환을 포함한 다양한 질병과 관련된 것으로 알려져 있다. 이에 본 연구에서는 ‘장애보정생존년수(Disability-adjusted life years, DALY)’의 개념을 바탕으로 직무 스트레스로 인한 질병의 부담을 추정하였다. 직무 스트레스에 의한 건강 결과로서 허혈성 심장 질환, 뇌졸중, 주요 우울 장애 및 자살을 선택하였다. 이는 이러한 질환들이 한국의 직업 환경에서 차지하는 중요성과 질병부담을 추정하는데 필요한 데이터에 대한 접근성과 용이성을 모두 고려하여 결정한 것이다. 본 연구 결과 허혈성 심장 질환의 6.7%, 뇌졸중의 6.9%, 주요 우울 장애의 13.6%, 자살의 4%는 직무 스트레스에 의한 것으로 추정하였다. DALY로 평가하였을 때 근로자 10만 명당 질병 부담은 허혈성 심장 질환이 약 38년, 뇌졸중이 약 72년, 주요 우울 장애가 약 168년, 자살이 약 44년이 직무 스트레스에 의한 것으로 추정하였다. 본 연구의 결과는 직무 스트레스 관련 질환을 관리하기 위한 정책 수립의 근거 자료로서 사용될 수 있으며, 따라서 근로자의 건강을 증진시키는데 기여할 것이다.
