전립선암 환자들의 보완대체의료 이용 영향 요인

Abstract

배경 및 목적: 현재 급증하고 있는 전립선암 환자들은 병원의 긴 치료 과정과 삶의 질을 저하시키는 문제들을 겪는다. 이러한 질병의 특성은 보완대체의료 이용과 관련이 많으나 이에 대한 연구가 부족하다. 이 연구의 목적은 전립선암 환자들의 보완대체의료 이용과 연관 요인 결정 및 통합 의료적 암 관리 정책개발에 대한 기여이다. 연구 방법: 이 연구는 중앙보훈병원에 등록된 전체 전립선암 환자 8833명 중 303명을 대상으로 설문조사로 진행되었다. 이 연구는 중앙보훈병원 기관생명윤리위원회의 승인을 받아 진행되었다. 연구 결과: 본 연구에서는 45.9%의 높은 보완대체의료 이용률을 보였다. 이용 빈도가 가장 높은 보완대체의료 종류로는 식이/약물 요법(종합비타민, 영양제, 검은콩, 녹차)이었다(80.5%). 통증이 가장 심한 대상자들 군에서 보완대체의료 이용률이 높았으며(p=0.014) 통증이 일상생활을 방해하는 정도가 가장 높은 군에서(p=0.004), 전립선암에 대한 심각도가 높은 군(p=0.001), 의료진과의 상담시간이 짧다고 느끼는 군(p=0.021), 병원 치료의 효과가 없다고 느끼는 군(p=0.011), 치료 과정에 불만이 있는 군(p=0.022), 의료진에게 자신의 의견 제시를 잘 하는 군(p=0.001)에서 보완대체의료 이용률이 높았다. 결론 및 제언: 의료인들은 전립선 암 환자의 CAM 사용한다는 사실을 알고 있어야 한다. 그들의 보완대체의학 사용의 높은 빈도는 더 큰 표본 집단에 대한 추가 연구가 필요함을 시사한다.; Background and Objectives: The characteristics of the symptoms among prostate cancer patients are closed related with complementary and alternative medicine. However, there is a lack of scientific research on the subject. The aims of this study is to determine the pattern of and the factors on the use of complementary and alternative medicine by them. Method: This study has targeted registered patients for prostate cancer in VHS medical center. Among 8833 patients, 303 patients are surveyed. This survey obtained approval from the institutional life ethics commission in a medical center in Seoul, Korea. Results: 45.9% of prostate cancer patients had used complementary and alternative medicine. The most frequent method is a diet and food therapy (80.5%). Patients with severe pain were more likely to use complementary and alternative medicines. The many characteristics of the patients perception on their disease and health service experienced are related to the patterns of the use of complementary and alternative medicine. Conclusions and Discussion: Physicians need to be aware of the fact that prostate cancer patients’CAM use. High prevalence of CAM use among them warrants further studies using larger sample population.
