- 육종은 지방, 근육, 혈관, 신경 등의 mesenchymal tissue에 발생하며 아형간
종양 미세 환경을 포함한 다양한 면에서 매우 이질적인 특성을 가지는 악성 희귀
암이다. 이러한 이질적 특징으로 인해 육종암의 정확한 진단, 아형의 분류 및 그
에 따른 적절한 치료 계획 수립이 어렵다. 육종암의 이질성은 예후 예측과 종양의
행동 예측의 어려움도 야기한다. 이러한 이질적 특성을 반영한, 아형 별 맞춤형 치
료가 필요하다. 본 연구에서는 육종암의 이질적 특성을 아우르는 새로운 아형 분
류 방법을 제시하여 기존의 분류 방법의 한계를 보완하고자 하였다. 기존의 육종
암 분류에 따른 아형은 서로 다른 아형간 분자적 지표가 대부분 일치하여 각 아형
의 분자적 특징에 맞춘 정밀한 표적 치료에 어려움이 있었다. 문제 해결을 위해 많
은 연구자들이 기존의 조직병리학적 분류에서 벗어나, 분자 지표에 근거한 새로운
아형 분류 방법을 개발하였다. 그러나 분자 지표 기반 아형 분류방법 또한 조직 병
리학적 아형 분류에 의존하는 한계를 지닌다.
따라서, Tumor Microenvironment(TME)를 기반으로 한 새로운 분류 체계
를 도입하여 육종암의 정밀 의학 치료를 가능케 하고자 했다. 각각 Genomic Data
Commons(GDC), NCBI Gene Expression Omnibus(GEO)에서 수집한
TCGA-SARC(The Cancer Genome Atlas Sarcoma), GSE30929,
GSE71121데이터를 통합하여 메타데이터를 구성한 뒤 분석을 진행하였다. 육종
암의 TME 정보를 망라하는 분류 체계의 확립을 위해 면역 세포의 구성 및 발현
정도를 담은 CIBERSORTx 분석 결과 데이터를 NMF 클러스터링하여 육종암을
세 개의 아형(클러스터)으로 분류하였다. 각 아형은 독자적인 분자적 특징을 가지
며, 이 분자적 특징을 이용하여 각 아형에 특화된 정밀 의학 맞춤 치료 전략을 제
안한다. 클러스터 1번과 3번은 각각 CDK amplification, PTEN deletion을 분자
적 특징으로 갖는다. 이들 클러스터에 해당하는 육종 아형 환자에게는 각각 CDK,
PTEN deletion관련 경로를 표적으로 하는 약물로 치료하는 것이 효과적일 것이
다. 클러스터2번은 TLR amplification과 CD274 amplification을 분자적 특징으
로 갖는다. 따라서 클러스터 2로 분류되는 육종암 아형 환자는 면역 체크 포인트
억제제(면역 관문 억제제)로 치료하는 것이 효과적일 것이다. TCGA-SARC 데
이터에 대한 생존 분석을 통해 TME를 기반으로 한 새로운 분류 체계의 유효성을
통계적으로 확인했다. TME기반 육종암 아형 분류법의 개발이 육종암의 정밀한
진단과 예후 예측, 그리고 육종암 아형 별 맞춤 정밀 의학 치료의 발전에 기여할
것으로 기대한다.|Sarcomas, a diverse group of rare malignancies originating from
mesenchymal tissues, present significant classification challenges due to
their heterogeneity. Pathologically, they are divided into over 50 subtypes,
including osteosarcoma and liposarcoma, distinct from epithelial-origin
carcinomas. Notably, undifferentiated sarcomas constitute around 20% of
soft tissue sarcomas. Globally, sarcomas account for 1% of all cancer
diagnoses, with an annual incidence of 5-6 per 100,000 in the U.S., as
reported by the National Cancer Institute.
This study introduces a pioneering classification system for sarcomas,
pivoting on the tumor microenvironment (TME), which complements
traditional molecular marker-driven methodologies. Employing
CIBERSORTx-derived immune cell expression scores, we classified
sarcomas into three TME-based subtypes. This novel stratification,
congruent with precision medicine paradigms, effectively represents the
diversity inherent in sarcoma subtypes.
For our analysis, we utilized datasets from the Genomic Data Commons
(GDC) and NCBI Gene Expression Omnibus (GEO). Following
normalization and batch effect elimination, we conducted clustering using
Non-negative Matrix Factorization (NMF) and performed pathway
enrichment analysis, delineating unique molecular signatures for each
cluster.
Our results demonstrate distinct molecular and immunological features
across the TME-based sarcoma subtypes, underscoring the potential for
subtype-specific therapeutic interventions. The prognostic relevance of
this classification is further validated by survival data from TCGA-SARC,
affirming its utility in guiding clinical decision-making. This study marks
a significant advancement in sarcoma research, suggesting a paradigm
shift in classification strategies that could potentially extend to other 80
cancer types. Further validation through clinical studies is essential to
establish the broader applicability of this TME-based classification
system in precision oncology.
