호흡기 질환을 가진 미숙아 부모의 정보 요구 분석

Abstract

본 연구는 국내 인터넷 포털사이트에서 신생아호흡곤란증후군과 기관지폐이형성증을 주제로 검색하여 수집한 데이터를 분석하고 이를 토대로 호흡기 질환을 진단받은 미숙아를 돌보는 부모가 가장 필요로 하는 정보에는 무엇이 있는지 파악하기 위한 서술적 조사연구이다. 본 연구에서는 2013년 1월 1일부터 2022년 12월 31일까지 국내 N 포털사이트에서 사용자 간 질문과 답변하며 정보를 교류하는 ‘지식iN’ 서비스와 커뮤니티형 소셜네트워크 서비스인 ‘카페’에서 작성되어있는 게시글을 대상으로 데이터 수집하였으며 총 850건을 연구 대상으로 하였다. 지식iN에서 수집된 게시글을 제외한 수집된 데이터는 신생아와 미숙아를 돌보는 부모를 대상으로 개설한 ‘**홀릭’, ‘**도담’ 외에도 여러 맘카페에서 수집된 것을 확인할 수 있었다. 수집된 게시글을 바탕으로 TEXTOM 프로그램을 활용하여 다빈도 키워드 분석, TF-IDF 분석, 토픽모델링 분석 방법을 활용하여 호흡기 질환을 가진 미숙아를 돌보기 위해 부모가 필요로 하는 정보 요구를 파악하였고 이와 더불어 대상자가 직접 작성한 게시글의 특성을 확인하여 부모의 정보 요구에 대해 알아보았다.

연구 결과는 다음과 같다. 1. 신생아호흡곤란증후군을 주제로 올린 게시글을 분석한 결과 질병과 관련된 질문보다 의료비와 관련된 ‘산정특례’에 관한 질문이 많았으며 키워드의 중요도 또한 높게 나타났다. 2. 기관지폐이형성증을 주제로 올린 게시글을 분석한 결과 질병과 관련된 질문과 더불어 호흡기융합세포바이러스의 감염을 예방하기 위한 ‘시나지스’ 예방접종과 관련된 정보 요구가 높았으며 키워드의 중요도 또한 높게 나타난 것을 확인할 수 있었다. 3. 토픽모델링 분석 결과 신생아호흡곤란증후군은 2가지 주제 ‘신생아호흡곤란증후군 질병 관련 정보’, ‘산정특례 신청 관련 정보’로 나눌 수 있었으며 기관지폐이형성증은 ‘기관지폐이형성증 관련 증상’, 그리고 ‘기관지폐이형성증 관련 합병증’ 두 가지로 구분되었다. 4. 호흡기 질환을 가진 미숙아를 돌보는 부모는 아기의 질병에 대한 정보 요구와 더불어 산전특례나 보험, 시나지스와 같은 의료비 지원에 대한 정보 요구가 많은 것을 알 수 있었다.

본 연구를 통해 부모는 환아의 질병과 관련된 정보를 꾸준히 요구하고 있다는 사실을 확인할 수 있었다. 이러한 결과를 바탕으로 가장 가까이에서 부모와 이야기할 수 있는 간호사는 부모의 정보 요구를 해결하기 위해 아기의 질병과 관련된 증상에 대해 정확한 정보를 바탕으로 교육을 제공할 필요가 있다. 그리고 의료비 지원과 관련된 정보 요구도 높은 것을 확인할 수 있었으며 간호사가 호흡기 질환을 진단받은 미숙아를 대상으로 한 의료비 지원 제도와 관련하여 더욱 관심을 가지고 부모에게 알맞은 정보를 제공할 것을 제언한다. 또한 본 연구 결과를 바탕으로 정책적인 측면에서는 보험 적용기준과 의료비 지원받는 대상자의 기준이 적절한지 확인해 볼 필요가 있다. | This study is a descriptive survey research aimed at identifying the information needs of parents caring for infants diagnosed with respiratory distress syndrome and bronchopulmonary dysplasia. The research analyzed data collected by searching and collecting information on the topics of respiratory distress syndrome and bronchopulmonary dysplasia from domestic internet portal sites. The data was collected from January 1, 2013, to December 31, 2022, using the "Jisik-iN" service, which allows users to exchange questions and answers, and from community-based social networking services called "Cafe." A total of 850 posts were included as the research sample. The collected posts were analyzed using the TEXTOM program, employing methods such as frequency keyword analysis, TF-IDF analysis, and topic modeling analysis to identify the information needs of parents caring for infants with respiratory diseases and to examine the characteristics of the posts written by the subjects.

The research findings are as follows:

1. Analyzing the posts related to respiratory distress syndrome revealed that there were more questions about the "special calculation" of medical expenses rather than disease-related questions, and these keywords were also found to be highly important. 2. Analyzing the posts related to bronchopulmonary dysplasia showed that along with disease-related questions, there was a high demand for information about "Synagis" vaccination to prevent respiratory syncytial virus (RSV) infection, and these keywords were also found to be highly important. 3. The results of topic modeling analysis revealed that respiratory distress syndrome could be divided into two topics: "disease-related information about respiratory distress syndrome" and "information about applying for special calculation." On the other hand, bronchopulmonary dysplasia was categorized into two topics: "symptoms related to bronchopulmonary dysplasia" and "complications related to bronchopulmonary dysplasia." 4. Parents caring for infants with respiratory diseases have a high demand for information not only about their baby's illness but also about prenatal special insurance, and ‘Synagis’ vaccination for medical expense support.

Through this study, it was possible to confirm that parents consistently demand information related to their infant's illnesses. Based on these findings, it is necessary for nurses, who are in closest contact with parents, to provide education based on accurate information about symptoms related to the baby's disease in order to address their information demands. Additionally, it was observed that there is a high demand for information regarding financial support for medical expenses. Therefore, it is recommended that nurses take a greater interest in providing appropriate information to parents regarding the medical expense support system targeting infants diagnosed with respiratory diseases. Furthermore, based on the results of this study, from a policy perspective, it is necessary to assess whether the criteria for insurance coverage and eligibility for medical expense support are appropriate.
